What You Will Do

In this role, you will:
Identify and engage caregivers of adults who are 65 and older through admission/rounds flags and referrals; obtain consent and explain program scope.
Conduct brief structured screens using evidence-based tools (e.g., PHQ‑2/9, GAD‑2/7, brief caregiver‑burden check, health‑literacy and SDOH prompts).
Facilitate a concise “What Matters” conversation to elicit the caregiver's priority needs and preferences; co‑create an action plan focused on1-3 priority areas.
Provide tailored resource packets (education, benefits, respite, mental health, community‑based supports); use teach‑back to ensure understanding and feasibility.
Coordinate with the care team (e.g., primary team, case management, nursing, behavioral health) to align plans and communicate a concise summary note.
Track outputs and outcomes (encounters, screening completion, referrals provided, closed‑loop hand‑offs, caregiver‑reported stress/depressive symptoms when available).
We require:
MSW from an accredited School of SocialWork; NYS licensure or permit as applicable.
Minimum 1 year in medical, geriatric, primary care, or community health socialwork experience with brief screening, goal‑setting, and resource navigation preferred.
Motivational interviewing or equivalent brief counseling; familiarity with evidence-based screening tools (eg, PHQ/GAD screening); knowledge of accessible and relevant caregiver resources (respite, benefits, mental health); EMR proficiency; strong communication and teamwork.

Summary of position:
This position reports to the Deputy Director of SocialWork Reentry within the Division of Correctional Health Services and is responsible for working with the Deputy Director on the management, supervision and innovative development of staff (socialworkers, case managers, administrative) who work to address the social service needs of patients on the mental health service upon re-entry into the community. These needs include but are not limited to linkages to benefits, housing, and treatment for mental illness, employment, and family/community supports. This position will require strong collaboration with all relevant clinical services and with the courts.
Responsibilities include: (Detailed Task)
Develop and evaluate performance standards and guidelines for socialworkers and case workers in the forensic mental health treatment setting.
Provide direct socialwork services to individuals, families and communities; including the development and implementation of comprehensive clinically driven discharge plans.
Assist socialworkers and case managers in identifying and utilizing community resources for mental health, substance use, entitlement benefits, housing, and medical care.
Assist with referrals/appointments to appropriate community-based aftercare services.
Promote and support SocialWork Department collaboration with an inter-disciplinary treatment team approach to patient care through excellent verbal and written communication skills.
Conduct quality assurance reviews of patient care to ensure clinically appropriate, tailored comprehensive care, and in compliance with Brad H stipulation requirements.
Generate and reconcile reports and analyze data related to patient care and Brad H stipulation compliance.
Conduct appropriate follow-up to ensure connection to care; and participate in the orientation and in-service training of staff and other agency personnel.
When required, participate in program planning and policy making with regard to community health programs and facilities and discharge planning.
Supervise and evaluate all aspects of socialworker and case managers' work performance; take part in counseling and disciplinary processes when necessary.
Minimum Qualifications:
1. Licensure (LMSW or LCSW) issued by The New York State Education Department (NYSED); and,
2. Six (6) years of experience in Clinical SocialWork, of which three (3) years are in a health care or medical care setting including psychiatric and medical socialwork, and three (3) years in a supervisory, consultative and administrative capacity in a organization adhering to acceptable standards with knowledge of administration, with administrative and managerial skills and abilities; or,
3. A satisfactory completion of training and experience. However, all applicants/incumbents must possess licensure as described in (1) above at the time of appointment; and,
4.Demonstrated ability to function productively and cooperatively with Director of SocialWork, Medical Staff, and Administrative Staff and to supervise effectively.

The goal of the Family Enrichment Center (FEC) is to enhance child and family well‑being by providing access and support to enable the strengthening of family protective factors inherent in its residents and community by building mutual community connections, capacity, and resilience.
In partnership with FEC staff, families will drive the planning, development, and sustainable co‑ownership of the centers. Families who participate in the FEC programming will be encouraged to share their expertise by volunteering at the center and given meaningful opportunities to engage with neighbors and peers. The goal of the FEC is to strengthen families' protective factors and build community connections, capacity, and resilience.
Position Summary
The Family Advocate is responsible for contributing to the day‑to‑day operation of the FEC including building connections with other staff, residents, and community partners; co‑designing and delivering relevant programming and activities (offerings); providing advocacy and making referrals for participants; and creating a warm and inviting gathering place for families and individuals using the protective factors model to support community resilience.
Responsibilities

Participate and engage with the FEC team in learning collaboratives facilitated by ACS with other FEC teams from across the city.

Qualifications
Minimum qualifications include a bachelor's degree and three (3) years of professional experience; or a high school diploma and five (5) years of professional experience in community-driven programs working directly with children, youth, and families.
Reside in or near the FEC community, or reside close enough to arrive promptly, in the event of an emergency.
Bi‑Lingual either /Mandarin &/or Cantonese is required.
Strong interpersonal skills and the ability to warmly connect with and engage children, youth, and adults individually and in groups.
Strong verbal and written communication skills.
Familiar with the community and its assets and resources, such as other programs, services, supports, local civic groups, and community leaders.
Experience in developing and running a group, a club, or coaching a team preferred.
Experience caring for children (as a parent, guardian, relative, neighbor, or babysitter) preferred.
