St. Vincent's Medical Center in Bridgeport has more than 3,200 employees. It includes a 473-bed community teaching hospital, a 76-bed inpatient psychiatric facility in Westport, a large multispecialty provider group, and special needs services for adults and children. St. Vincent's is the first hospital in Fairfield County to be integrated into the Hartford HealthCare network and is the system's second-largest hospital.
Job Summary:
As a member of the Palliative Care Department, the Palliative Care socialworkers will provide comprehensive clinical and psychosocial assessment and intervention for patients with serious illness. The socialworker must be comfortable working in a fast-paced inpatient and outpatient setting where the structure of the day may change depending on patient, family and staff needs. The Palliative Care socialworker provides support around adjustment to illness, decision making, eliciting goals of care and family coping along with the illness trajectory. In keeping with the NASW Standards for SocialWork Practice in Palliative and End of Life Care (***************************** the socialworker will play a multidimensional role as clinician, educator, advocate and team consultant.
Responsibilities:
1. Performs comprehensive psychosocial assessments of hospitalized patients with serious illness. Assists the health care team to integrate psychosocial factors into treatment plans: Ability to analyze significant psychological, physical, emotional, social and economic issues of patients/families and to effectively communicate the relevance of such to other health care providers. Ability to identify, analyze and suggest interventions for difficult patient/family management situations.
2. Provides socialwork intervention including counseling and support to assist patients/families. Demonstrates ability to form empathic relationships with patients/ families who have significant emotional adjustment and coping issues.
3. Effectively addresses quality of life and end of life issues within a culturally competent framework to support the equitable delivery of excellence in health care to all patients and families.
4. Assess patient/ family's level of understanding, barriers to learning, and to provide age-specific and culturally competent education as needed. Able to effectively assess and discuss the psychosocial issues of palliative care to patients/families.
5. Arranges, attends and actively participates in patient/family conferences, in person, as well as via available telehealth resources.
6. Demonstrates commitment to continuous improvement by seeking ways of improving care, effectiveness, and efficiency within the Palliative Care Service through active participation in Huddles and meetings.
7. Maintains patient records and other documentation in a manner consistent with standards of clinical care, hospital, department, third party and regulating agencies' policies and procedures and standards of risk management.
8. Attends Palliative Care Team rounds, and all required departmental meetings.
9. Demonstrates awareness of, and adherence to St Vincent's Medical Center policies and procedures. Maintains confidentiality and comply with professional ethics according to professional departmental and organizational standards and maintains current Connecticut license to practice socialwork.
10. Participates in continuing education opportunities to enhance and broaden skills relevant to job performance; maintains documentation of continuing education and in-service activities provided.
11. Participates in educational programs for St Vincent's Medical Center staff as an expert on psychosocial issues in palliative care.
12. Works with members of the team to provide bereavement follow up for our palliative care patients.
13. Helps to ensure that we have appropriately documented Serious Illness Conversations, as well as appropriately scanned- in copies of other advance care planning documentation (such as completed health care representative forms, MOLST forms, Living Wills, Guardianship paperwork) into the Electronic medical record for all patients seen by our service for palliative care consultation.
Qualifications
Education:
· Hold a master's degree from a socialwork program accredited by the Council on SocialWork Education. Completion of Post-MSW Fellowship or Certification Program in Palliative Care preferred.
License(s) and Certifications:
· Licensed Clinical SocialWorker (LCSW) with valid licensure in the State of Connecticut.
· Seek Advanced Palliative and Hospice SocialWorker - Certified (APHSW-C) certification within 2 years.
Experience:
· Two years of post MSW work experience in a healthcare setting.
· Work experience in the field of palliative care, hospice care or relevant disease-specific specialty care.

We require:
MSW from an accredited School of SocialWork; NYS licensure or permit as applicable.

Summary of position:
This position reports to the Deputy Director of SocialWork Reentry within the Division of Correctional Health Services and is responsible for working with the Deputy Director on the management, supervision and innovative development of staff (socialworkers, case managers, administrative) who work to address the social service needs of patients on the mental health service upon re-entry into the community. These needs include but are not limited to linkages to benefits, housing, and treatment for mental illness, employment, and family/community supports. This position will require strong collaboration with all relevant clinical services and with the courts.
Responsibilities include: (Detailed Task)
Develop and evaluate performance standards and guidelines for socialworkers and case workers in the forensic mental health treatment setting.
Provide direct socialwork services to individuals, families and communities; including the development and implementation of comprehensive clinically driven discharge plans.
Assist socialworkers and case managers in identifying and utilizing community resources for mental health, substance use, entitlement benefits, housing, and medical care.
Assist with referrals/appointments to appropriate community-based aftercare services.
Promote and support SocialWork Department collaboration with an inter-disciplinary treatment team approach to patient care through excellent verbal and written communication skills.
Conduct quality assurance reviews of patient care to ensure clinically appropriate, tailored comprehensive care, and in compliance with Brad H stipulation requirements.
Generate and reconcile reports and analyze data related to patient care and Brad H stipulation compliance.
Conduct appropriate follow-up to ensure connection to care; and participate in the orientation and in-service training of staff and other agency personnel.
When required, participate in program planning and policy making with regard to community health programs and facilities and discharge planning.
Supervise and evaluate all aspects of socialworker and case managers' work performance; take part in counseling and disciplinary processes when necessary.
Minimum Qualifications:
1. Licensure (LMSW or LCSW) issued by The New York State Education Department (NYSED); and,
2. Six (6) years of experience in Clinical SocialWork, of which three (3) years are in a health care or medical care setting including psychiatric and medical socialwork, and three (3) years in a supervisory, consultative and administrative capacity in a organization adhering to acceptable standards with knowledge of administration, with administrative and managerial skills and abilities; or,
3. A satisfactory completion of training and experience. However, all applicants/incumbents must possess licensure as described in (1) above at the time of appointment; and,
4.Demonstrated ability to function productively and cooperatively with Director of SocialWork, Medical Staff, and Administrative Staff and to supervise effectively.

What You Will Do
Delivers outreach services to mentally ill individuals in the community, providing in-home mental health intervention to those experiencing or at risk of psychosocial difficulties
Collaborates with another team member to perform psychosocial evaluations and assess mental health service needs of clients and their families through observation and interviewing
Creates and execute short-term service plans for clients in collaboration with an interdisciplinary team
Responds promptly to dispatch calls, traveling from the office to the clients' homes within two hours
Ensures seamless program operations as a vital liaison to community agencies and stepping in for the Program Coordinator as needed
Ensures uninterrupted comprehensive care from counseling and care management to long-term supplemental treatment referrals and follow-up
Prepares case histories and prepares and maintains case records, in accordance with the Program record-keeping mechanism
Encourages resistant clients to accept mental health services through interventions with clients and/or family members and friends concerned with the client's welfare
May provide clinical supervision for Mental Health Technician, graduate SocialWorker students or junior staff SocialWorkers
Qualifications
Current registration to practice as a Licensed Master SocialWorker in New York State Required as determined by operational/regional needs
Valid driver's license may be required
Minimum of one year experience as a SocialWorker in a health care setting required as determined by operational needs, bilingual skills may be required Must be able to walk up and down steps to access patient residences
