Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient's medical provider. Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan. Case reviews may be conducted by telephone, video, or in person.
Track patient follow up and clinical outcomes using a registry. Document in-person and telephone encounters in the registry and use the system to identify and re-engage patients. Registry functions can be accomplished through an EHR build, on a spreadsheet used in conjunction with an EHR, or can be built into a stand-alone clinical management tracking system that may or may not be linked to an EHR.
Document patient progress and treatment recommendations in EHR and other required systems so as to be shared with medical providers, psychiatric consultant, and other treating providers.
Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care.
Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).
Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.
Be comfortable utilizing Excel and Word and completing assessments and productivity trackers daily.
Time-management skills are a must and the BHCM needs to be able to manage their day effectively through calendars and the EMR system.
This role is telephonic, however in person visits are done and the BHCM has to be comfortable completing them both ways.
Qualifications:

Provides patients and family education regarding diseases, treatments, social or financial situations, emotional stress and/or other problems
Monitors care to evaluate progress towards goals and institutes appropriate interventions as needed
Conducts high risk screening and provides crisis counseling, education and resources to patient and families regarding abuse or neglect, physical abuse, etc. and documents these sessions.
Acts as a liaison between patients/families and the clinical staff and other agencies
Advocate for patient concerns and escalate ethical dilemmas through the appropriate channels.
Completes applicable reports bases on mandated reporting requirements.
Precepts newly hired staff member. Provides field instructor services to schools of socialwork for the purpose of training bachelor and master's program intern to the hospital or health care setting.
Drives performance improvement initiatives to include data collection and research, development, and participation in continuing education programs
Maintains an open, approachable manner treating others fairly and respectfully preserving self-confidence and dignity as well as show regard for their opinion
Lead Care Management team meetings and interdisciplinary rounds.
Assists with projects as assigned.
Complete Leadership academy as assigned.
Commitment to continuing education and professional development.

Minimum Qualifications
Education Requirements: Master of SocialWork from an accredited institution required.
Experience Requirements: At least 3 years of socialwork experience.
License/Certification/Registration Requirements: Current Licensed Master SocialWorker (LMSW) or under supervision for Licensed Clinical SocialWork (LCSW) or LCSW from the Oklahoma State Board of Licensed SocialWorkers. Current Basic Life Support certification from the American Heart Association required
Knowledge/Skills/Abilities Required

Provide crisis support and stabilization to youth and adults in the Red Rock coverage area to help prevent need for higher level of care. Collaborate with URC staff, CSU, Transport, Licensed Mental Health Professionals, individuals/families and community providers to help the individual in crisis maintain safety in the least restrictive environment possible.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Respond to calls from the communities, family, and Red Rock Out-Patient offices for crisis support and stabilization
Develop crisis/safety plans to help the individual be able to safely maintain in the community
Help facilitate a high quality level place of care when necessary
Complete paperwork and billing as required
Travel to the community to meet face-to-face in a safe location when necessary
Support and assist with the facilitation of Care Plans for Red Rock consumers.
Maintains core competencies in relation to working with co-occurring disorders through continuing education and implementing skills into all aspects of treatment
Performs other duties as required
QUALIFICATIONS
Must meet one of the following parameters:
Current BHCM II Certification
Bachelor's or Master's degree in a Behavioral Health field
RN with Behavioral Health Experience
Bachelor's or Master's degree in any field and 6 months of Behavioral Health experience
60 College credit hours in any field wand 12 months of Behavioral Health experience
HS Diploma or equivalent and 36 months of Behavioral Health experience
Have demonstrated recovery from a mental health diagnosis, substance abuse disorder or both
Commitment to the mission of Red Rock BHS
Must have a valid Driver's License and clean driving record
Must be willing to work a flexible schedule
PHYSICAL REQUIREMENTS

As a Hospice Licensed Medical (LMSW) or Licensed Clinical SocialWork (LCSW), you will:
Provide wide range of psychosocial services to terminally ill patients and their families.
Responsible for making contact with patient, family, and caregiver within 48 hours of admission.
Psychosocial must become part of patient's chart within 24 hours of completion.
Regularly assess the patient and family psychosocial needs.
Regularly assess the pre-bereavement/anticipatory grief needs.
Implement the Plan of Care provisions to meet their needs, which include: social service goals for alleviating problems, supportive counseling, problem solving, community referrals pre-bereavement and bereavement care.
Regularly re-evaluate the patient's and family's psychosocial needs.
Participate in weekly interdisciplinary team meetings.
Assist with making funeral arrangements if requested.
Provide counseling during the process of the terminal illness and at the time of death if requested.
Required Skills
Qualifications:
Master's Degree from a school of socialwork accredited by the Council on SocialWork Education
Minimum of two year socialwork experience in a healthcare setting or social services agency
Current SocialWorker license
Hospice experience a plus
Knowledge of and ability to obtain community resources (information and referral, state/federal financial programs and eligibility requirements, etc.).
Understand Hospice philosophy, principles of death/dying
Strong organizational and communication skills
