In this role, you will: Identify and engage caregivers of adults who are 65 and older through admission/rounds flags and referrals; obtain consent and explain program scope. Conduct brief structured screens using evidence-based tools (e.g., PHQ‑2/9, GAD‑2/7, brief caregiver‑burden check, health‑literacy and SDOH prompts). Facilitate a concise “What Matters” conversation to elicit the caregiver's priority needs and preferences; co‑create an action plan focused on1-3 priority areas. Provide tailored resource packets (education, benefits, respite, mental health, community‑based supports); use teach‑back to ensure understanding and feasibility.
