* Provides non-professional counseling and emotional support, participates in treatment team meetings, and assists with implementing individualized treatment plans.

The following experience and/or education is required to qualify for this position.

One full year of graduate education directly related to the work of the position meets the requirement.
Equivalent combinations of education and experience are qualifying for all grade levels and positions for which both education and experience are acceptable.

Seek out clinical information and advice as needed, accepts constructive feedback and reasonable recommendations from manager
Provides support to leadership and programs with interns, new hires, and additional program needs.
Provides support to leadership and programs in times of program expansion (i.e. Therapeutic Foster Care, Home Based Shelter Care, Complex Care, etc.)
Perform other duties as necessary for the program/agency, including working in other programs
Assists and/or shadows the Social Worker I/II in initial onboarding or in a training capacity

Role Scope:
We are looking for a dynamic, service-oriented social worker who is passionate about supporting elderly persons in their journey. The social worker will work within a multidisciplinary care team led by a primary care provider to ensure whole-person, integrated care. Work with primary care providers and other members of the care team to guide smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home, etc.)
Initiate, coordinate and facilitate care conference meetings to ensure the highest level of care coordination among other care team members, participants, and other people within the participants' support network (family, informal caregivers, etc.)
Provide discharge planning when participants disenroll from the program

Required Qualifications:
Master's degree in Social Work (MSW).
A minimum of two years experience working with the frail or elderly.
Previous experience coordinating and facilitating care conference meetings.
Proof of medical clearance for communicable diseases, including a TB test.
Proof of all immunizations are up to date.
Proof of current CPR/BLS certification required or requirement to obtain within 30 days of employment..
Proof of valid CA driver's license, personal transportation, good driving record and auto insurance as required by State law.
Preferred Qualifications:
Bilingual: Spanish/Mandarin/Cantonese preferred.
Previous experience assisting patients/participants with behavioral health and substance abuse issues.

Compensation:
We take into account an individual's qualifications, skillset, and experience in determining final salary. The expected salary range for this position is $38.00 - $42.00 hourly. Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group

This position requires working both during regular office hours and after hours. We are responsive to clients during and after hours. This position also requires attendance for off-hours activities for youth engagement. This may require altering your hours of operation while accommodating mandated in-office workdays.

Job Title: Social Worker - Adult Learning Disabilities Team Rate: £198 per day Liquid Personnel is currently recruiting for a Social Worker to join an Adults LD team based in Stockton on Tees.
What will your responsibilities be?
In this role, you will support adults with learning disabilities, including young people transitioning into Adult Social Care. You will conduct Care Act (2014) assessments, develop person-centred support plans, and review care packages. Applying the Mental Capacity Act (2005), you will conduct capacity assessments and Court of Protection work. You will collaborate with health colleagues to support those detained under the Mental Health Act (1983) and assess eligibility for Continuing Health Care (CHC) funding. Additionally, you will identify and address safeguarding concerns, including abuse, neglect, and self-neglect. Ideal candidates will have experience in Learning Disability teams, Care Act assessments, Mental Capacity Act work, and strong written and verbal skills.
