The Medical SocialWorker functions to evaluate and develop a plan of care personalized to fit the patient's emotional and social needs. The Medical SocialWorker provides direction and supervision of the SocialWorker Assistant as required and when involved in the patient's plan of care. The Medical SocialWorkerworks within CenterWell Home Health's company-specific policy and procedures, applicable healthcare standards, governmental laws, and regulations.
Assesses the patient's social and emotional state as it relates to his or her illness or injury, needs for care and his or her response to such treatment, and adjustments to care.
Assesses any relationships of the patient's medical and nursing needs in the home setting, financial resources, and available community resources.
Provides any appropriate action to obtain available community resources to assist in resolving issues that may be impeding the patient's recovery.
Instructs patients and families in treating and coping with social and emotional response connected with Provides ongoing assessment of patient and family needs and responses to teaching
Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient's health Participates in the development and periodic re-evaluation of the physician's Plan of Care for the patient.
Observes, records, and reports changes in patients' condition and response to treatment to the Clinical Manager and the Participates in the discharge planning process
Participates as a member of the interdisciplinary care team in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs
Supervises instructs and evaluates the performance of the SocialWork Assistant (BSW) to assure that all medical social services are provided to patients in compliance with Company, government, and professional standards
Maintains and submits documentation as required by the company and/ or facility including any case conferences, patient/physician community contacts, visit reports progress notes, and confers with other health care disciplines in providing optimum patient care.

Required Skills/Experience
Masters or doctoral degree from a school of socialwork accredited by the Council on SocialWork Education.
SocialWorker licensure in the state of practice; if required by state law or regulation.
A valid driver's license, auto insurance, and reliable transportation are required.
Proof of current CPR certification
Minimum of one year of experience as a socialworker in a health care setting, home health, and/or hospice.
Knowledge of and the ability to assist with discharge planning needs, and to obtain community resources (housing, shelter, funeral/memorial service arrangements, legal, information and referral, state/federal financial and medication programs, and eligibility.
Excellent oral and written communication and interpersonal skills.
Knowledge of medications and their correct administration.
Ability to organize tasks, develop action plans, set priorities, and function under stressful situations.
Ability to be flexible in work hours and travel locally.
Ability to communicate effectively with patients and their family members and at all levels of the organization.
Maintains current licensure certifications and meets mandatory continuing education requirements.
Must read, write and speak fluent English.
Must have good and regular attendance.
Performs other related duties as assigned.

**Education & Licensing**
Master's degree from an accredited college or university in socialwork, psychology, or counseling required. Independent clinical license at the master's level- Licensed Clinical SocialWorker (LCSW), Licensed Independent Clinical SocialWorker (LICSW), Licensed Master SocialWorker (LMSW), Certified Independent Practice SocialWorker (CIPSW), Independent SocialWorker (ISW), Licensed Clinical Professional Counselor (LCPC), or Licensed Professional Clinical Counselor (LPCC), or equivalent in colleague state required. Six (6) years. of related experience or equivalent combination of education and experience required to include four (4) years of clinical experience. **The opportunity:**
As Counsel for Technology and Managed Services, you will be responsible for providing advice, counsel, and legal support in respect of provision of managed services and other technology services and products to EY clients, with a particular focus on advising, drafting, updating and negotiating the legal terms and conditions of client facing complex, multi-year managed services, outsourcing, systems implementation, application development & maintenance, software licenses and software-as-a-service agreements, artificial intelligence, big data, analytics, cloud computing, as well as providing legal advice and support to various other EY technology and product teams ("Technology & Managed Services Agreements").

The Kids' Harbor Counseling Program uses a team of professionals to provide trauma-informed, evidence-based treatment to child victims of physical, sexual, and/or emotional abuse, children who have witnessed violence, children who are drug endangered, and/or providing services to youth demonstrating problematic sexual behavior.
Summary: This position is responsible for community collaboration, counseling services, and group counseling connected to trauma-informed, evidence-based treatment interventions.
SPECIFIC RESPONSIBLITIES
Client Services:
Provide direct and indirect services to children who have been victims of sexual, physical, and/or emotional abuse, children who have witnessed violence, and children who are drug endangered. Provide caregiver support by providing informed care connected to child services. These supports include but not limited to:
Targeted based treatment for child victims of sexual, physical, and/or emotional abuse, crisis counseling, utilizing best-practice, trauma-focused treatment modalities
Treatment for youth with problematic sexual behavior, crisis counseling, utilizing best-practice, trauma-focused treatment modalities
Assessment and evaluation of counseling needs, including pre and post assessment tools
Development of treatment plan(s) with individualized goals and objectives
Provision of individual and “family unit” therapy modalities
Provide support groups and psycho-education groups for children that have victims of sexual, physical, and/or emotional abuse, have witnessed violence or are drug endangered, and/or youth with problematic sexual behavior, and their safe caregivers

2. Program Coordination
Provide treatment planning, case management and documentation, and follow-up of cases including support calls, networking with other agencies, and coordinating services
Establish a therapeutic setting in which the environment is conducive to meeting the physical, social, and emotional needs of the children we serve
Maintains client files according to federal law, State of Missouri, the National Children's Alliance, The American Counseling Association, and Kids' Harbor's documentation standards
Develop and integrate group therapy for child abuse victims, children with problematic sexual behavior, children who are drug endangered, or witnesses of violence and their safe caregivers
Integrate evidence-based treatment interventions into clinical processes
3. Community Collaboration
Participate in any legal/judicial and administrative proceedings when required and in accordance with professional standards of conduct
Participates in peer review, group supervision, and individual supervision of cases as needed
Participates in professional development and training specific to youth with problematic sexual behavior, evidence-based treatment, and continuing education through workshops, online training or in person training opportunities

Demonstrated ability to work collaboratively with community partners, such as law enforcement, medical professionals and social service providers. Preferred Qualifications

Substance Abuse Counselors are responsible for:
Completes comprehensive assessments of clients' substance abuse history and treatment requirements, and develops individualized treatment plan.
Provides individual and group counseling and may facilitate didactic groups.

Additional Qualifications:

The Hospice SocialWorker provides preventive, educational, and evaluative and treatment services to meet the psychosocial needs of patients and their families, to help them improve, restore, and maintain their maximum level of coping. S/he utilizes community resources and the interdisciplinary team (IDT) to aid in this process.
Position Specific Responsibilities
* • Explains hospice services and Medicare benefits to patients and families; obtains Informed Consent and Election of Benefits documents as requested.
* Submits appropriate documentation and paperwork to facilities at the completion of patient visits; documentation is completed and submitted timely according to company policy.
* Participates as a member of the IDT, including development and implementation of the plan of care.
* Provides psycho-social support and counseling services to the patient and family; supports the involvement of external counseling resources commensurate with patient/family needs and scope of practice.
* Assists in identifying the need for intervention of other IDT members.
* Effectively communicates patient and family needs to IDT.
* Collaborates with IDT to coordinate psycho-social care and support for the patient and family to ensure appropriateness, continuity, and quality of care.
* Maintains updated, comprehensive care plans which reflect current problems, goals and interventions for patients.
* Maintains respect for the family's environment and belief systems, and remains nondiscriminatory regarding age, race, religion, sex, sexual orientation, national origin, physical or mental disability, and other personal matters.
* Completes initial and ongoing psycho-social assessments for patients; identifies and addresses concerns/issues related to financial means, insurance, living arrangements/placement for long-term care, caregiver stressors, pre-bereavement and anticipatory grief, and community resource needs.
* Facilitates placement in long-term care as warranted and monitors adjustment.
* Participates in care planning meetings at long-term care facilities.
* Supports and facilitates advance planning including living will/POA documents, funeral arrangements, memorial services, and body/organ donation.
* Performs other duties as assigned.

Education and/or Experience
* Required - Minimum of Bachelor's degree in SocialWork, Psychology, Sociology, or other field related to socialwork with one (1) year of socialwork experience in a healthcare setting required; and licensure if required by state rules. Will require supervisory oversight by MSW for hospice patient care planning and counseling needs.
* Preferred - Master's degree in SocialWork with one (1) year of socialwork experience in a healthcare setting highly preferred; and licensure if required by state rules.
* One (1) year of socialwork experience in a healthcare setting with Bachelors Degree
Skills
* Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
* Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
* Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces hospice philosophy.
State Specific Requirements
Missouri
* BSW from an accredited school of socialwork is minimum education required but must be supervised by MSW.
Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
