Operating in over 70 treatment centers in multiple states, our team members are engaged in medication-assisted treatment, counseling, support, and care management of individuals living with OUD. We treat the whole person and, in so doing, address the underlying causes of OUD in an effort to provide a continuum of care that not only addresses treatment needs but supports the patient on their journey to recovery.
Job Summary:
New Season operates a number of nationally recognized and accredited methadone treatment centers dedicated to helping individuals with prescription drug abuse and/or opiate addiction regain control of their lives.

Essential Functions:
Works with patients to complete all intakes, admissions, discharges, and transfer paperwork.
Documents patient progress through counseling and interaction through groups.
Completes patient psychosocial and an individualized treatment plan within the required time frame.
Identifies any clinical/case management needs and works to address those needs.
Performs individual, group, and family counseling as required.
Performs direct one-on-one patient couneling weekly through individual or group counseling.
Reports patient abuse, neglect and exploitation as required.
Reports patient grievances as required.
Educates patient in all aspects of treatment, corresponding health issues and steps to recovery.
Obtains Urine Drug Screens and initial patient photo identification.
Assists in monitoring all patient activities on center premises.
Actively participates in community relations activities as directed and authorized.
Ensures the reading and understanding of the Policy and Procedures Manual.
Ensures compliance with 42 CFR Part 2 (Federal Confidentiality Regulations) and 45 CFR, Parts 160 & 164 (HIPAA)
Is responsible for obtaining or maintaining proper licensure and/or certification according to specific state requirements.
Actively participates in preparation for surveys and inspections conducted by CARF, the State, DEA, Board of Pharmacy and any other agency site visits as dictated by the state laws or regulations.
Participates in all staff meetings
Ensures compliance with local, State, Federal and Colonial Management Group, LP rules, regulations and policies.
Always acts in the best interests of the program and company; honors, supports, and protects the proprietary data and rights of the company.
Performs other tasks as assigned by clinic, region or corporate leadership.
Minimum Qualifications:
Education/Licensure/Certification:
Experience in substance abuse field is not required, but preferred
Qualified candidates will hold one of the following licenses; LPC, LMFT, LCSW, LMHC
Experience Required:
Minimum of 500 hours of experience in substance abuse is preferred

ROLE-SPECIFIC TASKS
A. The HCHV Case Manager independently performs case finding functions and outreach, identifying and screening patients for psychosocial needs.

Social worker will make rapid assessments and develop crisis management plans. This includes the ability to facilitate written crisis plans, suicide risk assessments, collaborative safety action plans, and assessment of homicidal ideation.
C. The incumbent will provide wellness/prevention education and facilitate Patient/family support groups as appropriate.
D. The incumbent will, as appropriate, supervise the practice of social work students, or a less experienced social worker.
E. The incumbent will act as liaison between VA and community resources, to include marketing and public relation duties, as needed and approved per supervisor and policy.
F. Incumbent operates a government vehicle as an essential function of the job and could be required to transport patients in a government vehicle, even one time per year. The incumbent is required to have and maintain a valid state driver's license and to successfully complete any annual physical or education requirements to maintain the driving requirements of the position.
G. Incumbent will coordinate, implement, and report on community development activities on behalf of Veterans experiencing homelessness to the program coordinator, as well as facility, VISN and National Homeless Leadership at the direction of the MH Special Programs Manager. They will engage with Data collection, administrative record keeping and monitoring patient or program clinical indicators, thresholds and performance measurements

The Shelter Advocate is responsible for providing crisis intervention, direct service, and systems advocacy for individuals residing in a domestic violence shelter setting. It is expected that the Shelter Advocates' job performance will incorporate the practice and promotion of Family Pathways guiding principles: trust, collaboration, communication, integrity, respect, positivity and creativity.
Who You Are:
Capable of providing support, advocacy, crisis intervention, information, and referral to callers on a 24-hour crisis line, while also assessing calls for appropriateness.
Knowledgeable about providing a Primary Advocate approach, facilitating case planning and coordinating personal/legal advocacy for residents, while also collaborating with staff to review cases and make recommendations.
Able to complete intake forms with all new residents within 24 hours of entry into the shelter; orient residents to the shelter facility; explain expectations of residents in the shelter, and inform residents of services offered.
Excited to plan resident groups and activities: support, parenting, illness management, house meetings, etc., as needed and directed.
Experienced with helping residents hone skills such as problem-solving, crisis management, conflict resolution, and de-escalation, in addition to cultivating a welcoming, peaceful, healing environment for residents.
Able to provide transportation for residents when necessary.

Qualifications
High School Diploma or GED Required, Bachelors Degree Preferred No experience required. One year patient care or related experience preferred, preferably in a psychiatric setting. Skills: - Verbal and written communication skills and ability to work in collaboration with others. - Driver's license (MA, CT, NH, NY, RI, VT) preferred (varies by program), and ability to satisfactorily meet hospital driving standards. Certifications: - Medication Administration Program Certification (SELECT PROGRAMS ONLY). Applicants who are not currently MAP certified will have a specified time period from the date of hire, to successfully complete a McLean sponsored MAP certification program. Successful passing of the Medication Administration Certification is a requirement of the job. - All employees must possess basic computer skills to use a variety of electronic or online systems for communication, clinical and administrative purposes. - Required to complete electronic medical records training, and demonstrate 80% accuracy on competency test within 30 days of hire.
