Responsibilities
Position Overview:
Works to maximize social functioning of patients and families impacted by illness. Provides appropriate social service intervention as part of a multidisciplinary team in an effort to meet the total needs of the patient. The Medical SocialWorker applies strengths-based and person centered philosophy of care. This position demonstrates practical application of socialwork core values.
Qualifications
Required:
* Master's Degree in SocialWork or equivalent
* One (1) year of clinical and/or medical socialwork experience
Preferred:
* Current CA Licensed Clinical SocialWorker (LCSW) license
* Previous experience as a socialworker in an acute medical setting
* Knowledge of contemporary behavioral and systems theories relevant to healthcare, end-of-life dynamics and interventions, grief and bereavement counseling, substance abuse identification and interventions, support for victims of abuse, neglect or violence
* Knowledge of community or financial resources for underserved or vulnerable populations
* Bilingual (English/Spanish)
Essential Functions and Responsibilities:
Provides direct socialwork service and counseling of patients and their families. Floats to other areas when needed. Gathers appropriate data and formulates relevant assessment of patient and family's medical, social, and emotional situations, using effective interviewing techniques, consultation with healthcare team and chart review. Assesses and identifies the social determinants of health of patients. Provides socialwork intervention and support for the complex social and emotional need of patients and their families. Direct clinical services may include individual and family counseling, crisis intervention, facilitating education and linkage with resources to patients, families, and staff as indicated by the assessment and treatment plan. Identifies and understands the behavioral and emotional factors underlying a patient's illness. Assesses and supports care coordination that meets individualized patient need. Provides crisis intervention and assists families in understanding the implications and complexities of the medical situation and its impact on one's lifestyle. Assists with discharge planning, and post-acute placement on complex discharges. Accurately documents all assessments, plans, interventions, patient/family involvement, outside agency communications, interdisciplinary contacts and case closure according to socialwork and patient care area standards. Flexible when assigned on other hospital floors/teams as needed by management. Actively participates in department activities including regular staff meetings, committees and task forces. Provides effective orientation, work direction, and guidance to less experienced socialworkers. Performs other duties as assigned.

Key Responsibilities:
Provides orientation to new patients to the programs' rules, policies, procedures, and patients' rights.
Conduct Intakes, Discharge Planning, Initial Assessments, and Initial Treatment Plans professionally and within time guidelines as required by applicable federal and state regulation, CARF guidelines and Pinnacle Policies.
Involve patients in the assessment, treatment planning, and intervention aspects of treatment, which provides the patient with a sense of entitlement to their own recovery.
Administer the biopsychosocial assessment, which evaluates all areas of patients' lives, including their addictions, medical and mental health history, socioeconomics, legal, etc. at intake and on an ongoing basis.
Recognize and utilize patients' Strengths, Needs, Ability, and Preferences (SNAP), to develop and implement individualized Treatment Plans that effectively help patients to meet their short & long-term goals. • Determine the clinical necessity of counseling services based on the clinical assessment and evaluation of the patients.
Monitor closely, and document any change of circumstances with the patients, especially as it relates to matters that may affect their recovery, identifying and addressing relapse indicators to promote relapse prevention interventions in an early stage.
Comply with and implement the Medical Q.A (Quality Assurance) and Clinical Risk Management Policies, including:
Evaluate the patients for any high-risk conditions (e.g., liver failure, pregnancy, overdose, bipolar, schizophrenia, etc.), which may endanger the patient (suicidal) or other parties (homicidal).
Administer the BAM-R, C-SSRS, GAD-7, and PHQ-9 screening tools to determine the severity of the patient's mental/emotional status.
Evaluate the risk factors concerning each such case, considering the appropriateness of, and professionalism in treating such individuals in an OTP in comparison to treatment in a residential facility or other treatment modalities.
Review and ask about other practitioners' involvement in the treatment of these high-risk conditions. Recommend immediate referral to such a specialist in case of need. Request the patient sign a waiver for release of information which will allow communication with the other practitioner in case of need (e.g., deterioration of the patient's condition).
Flag high-risk patients to the Medical Directors, Physician Extenders, and Regional Clinical Leadership (RCL) and highlight the conditions concerned.
Refer such patients to the Medical Directors, Physician Extenders or RCLs (Regional Clinical Leadership) based on the urgency, utilizing the informal Referral Policy or the scheduled Treatment Team/Case Conference session.
Review physician notes, following the Medical Directors or Physician Extenders' session with the patient, verifying that they noted and addressed the high-risk factor, or otherwise bring it immediately to the attention of the supervising counselor, as well as the clinic manager, until the matter's proper and professional handling by the physician or physician extender.
Advise, in writing, other staff members (including the clinic manager, supervising counselor, dispensing nurse and front office) of high-risk patients and their condition. Maintain and circulate minutes from treatment team/case conference sessions to the other staff members.
Provide professional counseling and referral services by:
Addressing patients' clinical and special needs by maintaining referrals to specialized (“second tier”) services rendered by physicians, physician extenders and counselors within Aegis, as well as referrals to Keys to Recovery (or other twelve-step) support groups, and other community resources (e.g., agencies and practitioners outside of Aegis), acting as a liaison to these agencies, as necessary.
Providing individual and group counseling as clinically necessary, to address the problem issues identified in the Initial Assessment and Treatment Plans. The counseling service will include protocols for the treatment of Relapse Prevention, Peer Pressure, Anger Management, Domestic Violence, Parenting, Family Preservation, Vocational Rehabilitation, etc.
Utilizing advanced counseling techniques such as Cognitive Behavioral Therapy, Motivational Interviewing, etc.
Scheduling regular counseling sessions with patients; issue patient appointment cards and record scheduled or rescheduled sessions in EHR scheduler.
Documenting counseling entries, including signature and date, as required by applicable federal and state regulations, CARF guidelines and Pinnacle policies.
Engaging in Community Relations and Outreach efforts.
Attending all recommended training including Physician Training of Counselors, RCL training, and case conferences with the program physicians and physician assistants.
Collecting research data when requested.
Observing the collection of patients' urine samples when required
Attend team meetings and complete all training courses timely as required.
Other duties as assigned.

Possess an associate degree in relevant field and has the registration/credentials necessary to provide counseling services to our patient population.
Kern County requires a bachelor's degree or High school diploma/GED with 75% certification education.

Community Memorial Healthcare is hiring full-time master level behavioral health staff with excellent communication skills, clinical acumen, and passion for working with youth and their families. These positions play a key role in addressing behavioral health needs in primary care settings, improving access to services for children and their caregivers, and reducing health disparities among pediatric and vulnerable populations.
The Pediatric Behavioral Health Interventionist (BHI) supports the implementation and coordination of the Collaborative Care for Youth Innovation grant. This position operates within a Collaborative Care Model (CoCM) and Primary Care Behavioral Model (PCBH), integrating behavioral health into pediatric and young adult primary care settings. The staff provides screening, assessment, brief intervention, care coordination, and referral for patients with behavioral, developmental, or psychosocial needs. The role emphasizes early identification, family-centered support, and seamless transitions to appropriate care across child, adolescent, and young adult populations (ages 0-26 years).
The BHI staff is a core member of the collaborative care team. As a member of this team, you will work closely with primary care providers (PCPs), medical staff, psychiatric Interventionists, and other behavioral health professionals at Community Memorial Health to deliver comprehensive care that integrates mental and physical health. This position is based in Community Memorial Healthcare's outpatient primary care clinics and does not include hospital or inpatient responsibilities.
We are looking for clinicians who thrive in dynamic environments and demonstrate flexibility in meeting the needs of both patients and medical providers. Job Overview
Our SUD Counselor Internship program is specifically for students enrolled in a Drug and Alcohol Studies Program through an accredited educational program. During their time in our programs, interns are able to observe the daily activities of a residential or outpatient SUD treatment program. Interns have an opportunity to shadow certified SUD Counselors and/or LPHAs during assessment, treatment planning, groups, classes, counseling sessions, and case management sessions. What would this role do?
The Behavior Interventionist plays a critical role in implementing behavioral health treatments for children with Autism Spectrum Disorder (ASD) and other developmental disabilities. Through strategic programs and initiatives, this position will assist families seeking support with Autism and other related developmental disabilities by providing behavioral health treatment in-home, at a support center or in a community. This position is responsible for making meaningful change in the Client and families' life.

Pay Rate: $20.00 - $23.00 per hour
An additional $1.00 will be offered for Spanish bilingual candidates. Fluency will be assessed through a test.
Additional Pay Incentives:
+ $2/hour for weekend shifts
+ $5/hour for company-observed holidays
Candidates with a completed BA or BS will be offered an additional $1.00 per hour, with a copy of the degree required during onboarding.
Monday-Friday between the hours of 3:30pm - 7:30pm

Role Responsibilities (Here's what it takes)
Provide 1:1 care directly with Clients during scheduled sessions under the supervision of a Board- Certified Behavior Analyst (BCBA).
Implement established behavioral treatment plans based on the science of applied behavior analysis (ABA) across skill levels, moderate to severe problem behaviors, and service delivery settings.
Ensure that 100% of the scheduled direct services are provided without any scheduling interruptions.
Maintain accurate and timely documentation of client progress, interventions, and outcomes.
Collect data related to Client's progress during treatment sessions.
Incorporate activities and games that are developmentally appropriate and engaging for the client in alignment with the treatment plan.
Promote Client engagement to build their strengths in a natural and enjoyable manner.
Consulting with case Supervisor on treatment plans, Client updates, and intervention strategies.
Receiving and utilizing ongoing training, guidance, and support.
Follow program procedures, administrative tasks, direct therapy skills, and other duties that are in line with our compliance requirements.
Maintaining reliable communication to record scheduled appointments and return phone calls within 24 hours.
Ensuring that the company's “zero tolerance policy” is understood, implemented, and reinforced in the region.
Completing all duties, tasks, and/or special projects assigned by leaders and/or the business.
Participating in team meetings, trainings, and professional development opportunities.
Providing make up hours as an alternate care team member.
Maintaining eligible transportation and current liability insurance (if applicable).

Work in both indoor and outdoor settings as they relate to the patient's natural environment, which may include being outdoors in a variety of weather conditions (e.g., community skills, recess in a school setting, etc.)
