Summary of Essential Job Functions:

Providing caregiver training sessions through both telehealth and clinic-based sessions

Position Overview:
Horizons is seeking a dedicated and effective Family Engagement Specialist with a dynamic talent for fostering and nurturing deep connections and relationships with families in the out of school time setting. We are looking for a talented, passionate, caring professional who will work as a team player to fulfill the Horizons mission, and create a joyous, inclusive, inspiring and educational culture. Assist on an on-going basis to identify students during the summer program with excessive absences and work with families to improve student attendance.
Serve as a liaison between Horizons administration and families.
Work directly with the Horizons Parent Voice Committee to establish group guidelines for parent involvement in the program, develop a group structure, attend meetings and work with parents to establish goals for the group.
Plan and execute at least (4) special family engagement activities throughout the summer program including but not limited to Family Swim Night, Potluck Night, or new creative opportunities to share parenting resources with our families.
Participate in daily meals with the students, attend Family Night and Faculty Meetings, and other duties that may be required.
Attend paid pre-planning sessions prior to the start of the program.
Develop and nurture a safe, respectful, challenging and fun learning environment.

JOB SUMMARY

Participates in patient care planning responsive to age-specific needs.
Participates in planning the socialwork component required in selected hospital programs.
Maintains verbal and written communication with the multidisciplinary team on patient's psychosocial adjustment, interventions, and goals through medical record documentation and consultation.
Participates in socialwork and hospital-wide quality assurance activities.

Requirements

JOB SUMMARY
Under the general direction of the Transitional Care Director the Licensed Clinical SocialWorker will assist in the planning, developing, organizing, implementing, evaluating, and directing our social services and Discharge Planning Programs, and act as a coordinator between patient, family, physician, community agencies and resources, and this facility to assure that the medically related emotional and social needs of the patient are met/maintained in accordance with JCAHO, federal, state, and local guidelines, organizational and departmental policies and procedures. STANDARDS OF PERFORMANCE
Provides a psychosocial assessment of the patient, family, or appropriate support systems in order to implement crisis or support interventions and to maximize coordination of services.

Participates in planning the socialwork component required in selected hospital programs.

Requirements:
Minimum Level of Education: Bachelor's degree from an accredited college or university with a concentration in SocialWork or related field required. Master's Degree in SocialWork (MSW) preferred.
