Assess, plan, implement, coordinate, monitor and evaluate options and services needed to meet the patient's health needs as they move through the Rehabilitation phase in the continuum of care 2. Provide educational information and emotional support to the patient and family/significant other throughout rehabilitation and subsequent community settings 3. Identify patient care needs with input from members of the rehabilitation health care team and communicate/coordinate alternative settings with third party payers and other health care providers 4. Complete all necessary arrangements for transfer of care from one health care setting to the next after the rehabilitation care phase. Coordinate transfer and communicate to appropriate members of the health care team 5. Function as a case manager ensuring that a coordinated team approach to patient care is utilized throughout the continuum and into the community setting for patients 6.
