Assist with Medicaid and other payer source enrollments and help client understand their health insurance benefits. Maintain current directory of community resources in anticipation of the needs of the client to assist with referrals to healthcare resources and resources for assistance with social determinants of health. Track patient in appropriate electronic medical record or registry.
Conduct screening and risk assessment interventions as they relate to mental health. Identify needs of clients and connect them to resources in the community. Advise for the patient and coordinate behavioral health treatment with primary care and behavioral health providers and consult and participate in the care plan of the patient. Educate provider, patient and family on behavior health issues and substance use disorders including symptoms, relapse prevention, stress reduction and healthy lifestyle choices. Design appropriate health interventions using evidence-based treatment.
Participate in peer review and quality improvement efforts designed to optimize the efficacy of services provided. Attend and participate in COM professional and administrative committees as they relate to FMPP clinical practices. May conduct case presentations, special projects and other related assignments as assigned. Actively participate with other medical staff in implementing approved policies and procedures.
Work cooperatively in a multi-disciplinary setting, which may include Psychiatrists, nurses, ancillary staff, socialworkers, and health and wellness units and programs. Collaborates with a multi-disciplinary team to discuss cases as part of the team. May serve as a supervisor to students.
Qualifications
* Master's Degree or higher.
* One of the following valid FL licenses is required prior to hire: Licensed Clinical SocialWorker, Licensed Mental Health Counselor, or Licensed Marriage and Family Therapist.
* Current Basic Life Support (BLS) Healthcare Provider Certification issued by a certifying agency, or the ability to obtain prior to hire.
* Post-job offer, but prior to hire, the following will be required:
* Documentation of current PPD (Purified Protein Derivative) test as performed by a qualified physician, reporting negative results within the past six months.
* Documentation of physical examination completed by a qualified healthcare provider/physician within the past six months.

Six (6) years. of related experience or equivalent combination of education and experience required to include four (4) years of clinical experience. Provide daily supervision and support to residents in a structured and safe living environment. Serve as a co-teacher while youth attend the on-site school each day. Monitor youth behavior and ensure environment is conducive to learning.

As a live-in residential counselor, flexible schedule availability is required including some nights and weekends. Camp E-Nini-Hassee was founded in 1969 by Jack and Ruth Eckerd as the first outdoor therapeutic program for girls at-risk in the southeast. We provide a positive, strength-based environment for girls ages 12-18 to experience nurturing guidance. We present a reality-therapy approach in a residential, outdoor therapeutic environment, using alliances with staff and group therapy to provide safety, adventure, and education. Each girl receives individualized treatment and is supported with family services. We provide an accredited education on site with certified teachers and transferable credits. We help girls considered at-risk who are struggling with school, causing family conflicts, experimenting with drugs, experiencing depression, have low self-esteem, or are exhibiting emotional problems and behavioral issues.

Job Summary and Qualifications
The SocialWorker (MSW) is responsible for non-clinical, biopsychosocial assessments and interventions for the inpatient floors and the outpatient emergency department. The MSW performs crisis intervention, patient/family interventions, high-risk screening, focused emotional support, and referrals for financial assistance or other identified resource needs. The MSW assists in post-acute care placement on all discharges, primarily with those patients identified with post-discharge behavioral health and patient assistance/resource needs. Facilitate substance use and/or cognitive programming, based on the needs of each circuit. Non-traditional hours may be required to ensure program delivery. Travel may also be required for training, court, as well as to facilitate groups in other assigned locations.
If not certified upon hire, counselors must work towards obtaining certification (costs provided by DCS) by completing education hours and clinical supervision hours as set forth by the Alcohol and Drug Abuse Certification Board of Georgia (ADACBGA).
Minimum Qualifications:
Bachelor's degree in in counseling or a related field from an accredited college or university or certification as an addiction counselor from the Georgia Addiction Counselors Association (GACA) or equivalent certification.
Preferred Qualifications:
One year of experience providing individual and/or group counseling services to clients identified to have substance abuse needs. Experience in facilitating Cognitive Behavior therapy classes. Certification as an addiction counselor from the Georgia Addiction Counselors Association (GACA) or equivalent certification. A Group Meeting Facilitator interacts with groups that need coaching to work productively as a team to reach a common goal. The job of a "Group Meeting Facilitator" is more of the "conductor" than a participant throughout the Program Period. The ProSidian Group Meeting Facilitator shall serve as a consultant to clients across private companies, fortune 1,000 enterprises, and government agencies of all sizes. Follow-Up Deliverables - work to improve the contributions from meeting participants; coordinate meeting schedules; provide assistance with the creation of agendas; facilitate meetings; distribute meeting minutes, and maintain task and issues logs.
Post-Event Hot Wash / After-Action Analysis / After Action Reports - after the session is over, a complete report describing everything that went on in group meetings and/or project/taskforce initiatives shall be prepared. An organized debrief post-meeting and provide post-meeting feedback session held to provide suggestions and improvements for future meetings. All action items agreed to by the participants are documented as part of this report.
Public Involvement And Communications - foster collaboration and communication through open, ongoing, and effective maintenance of knowledge repositories to include correspondence, presentations, meeting summaries, work products, and responses to information requests by ensuring distribution of requested information.
Document Retention And Knowledge Repositories - live capture of meeting discussion/decisions in Microsoft word or excel is also expected. Maintain a library per established guidelines as a repository of related information co-located with the clients, program leadership, sub-group chairs, meeting participants, and other stakeholders as the official record required (i.e. Per Federal Advisory Committee Act - FACA) for an information repository.
Program/Recommendations Implementation - help participants reach decisions and accomplish goals.
