Position Overview
The Medical SocialWorker will work directly within a hospital unit, managing a daily caseload of approximately 25 patients. Key Responsibilities
Conduct psychosocial assessments for hospitalized patients
Manage discharge planning for an assigned caseload of approximately 25 patients per day
Coordinate post-acute services, community resources, and follow-up care
Collaborate daily with Nurse Case Managers, physicians, nurses, and interdisciplinary team members
Address barriers to discharge including housing, insurance, transportation, and social support needs
Provide emotional support, counseling, and education to patients and families
Ensure documentation is completed accurately and timely in accordance with hospital policies
Advocate for patient needs while maintaining compliance with regulatory and ethical standards
