Assessing the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicating findings to the RN Case Manager and other members of staff including the interdisciplinary group.
Assessing the relationship of the patient's medical and nursing requirements to the patient's home situation, financial resources, and availability of community resources, to include funeral arrangements if needed.
Supervising the implementation of an individualized social work plan of care and notified Bereavement Coordinator of bereavement assessment needs.
Participating in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of the psychosocial stresses and symptoms of the terminal illness.
Providing consultation, education, and support to the interdisciplinary team on social work needs.
Following policies and procedures consistent with recognized standards of professional practice
Providing high levels of communication both internally with the Kindful team and externally with patients and families to assure top CAHPS scores
Collaborating with your Clinical Director and entire team to provide a "concierge" level hospice experience
