Key Responsibilities

Qualifications
Possess a Master's degree in Social Work, Counseling, Marriage and Family Counseling, or a related field.
Experience in working with children and families
If required, provisional license per state regulation.

Position Overview:

Evaluate the health, safety, and service needs of seniors, develop individualized service plans, coordinate with families and community agencies, and follow up on high-risk or chronic concerns. Participate in Outreach staff meetings.
Respond to high-risk situations and crises affecting seniors, conduct office and home visits, involve community partners as needed, and file protective services reports as a mandated reporter.
Assist senior center staff in managing challenging behaviors, maintain accurate records of interactions, and develop follow-up plans for high-risk clients.

Qualifications:

Develops, implements, and maintains an ongoing quality assurance program for the social services department. Assists in the development, administration, and coordination of department policies and procedures. Reviews, documents and assists in resolving department complaints and grievances.
Keeps abreast of current federal/state regulations and professional standards, making recommendations on changes in policies and procedures to the Director. Maintains a reference library of written material and laws in order to comply with current standards and regulations. Develops quality social service programs.
Participates in facility surveys (inspections) made by authorized government agencies.
Serves on, participates in and attends various committee or department meetings as required and as appointed by the Administrator. Evaluates and implements recommendations from established committees as they may pertain to social services.
Schedules department work hours, personnel, work assignments etc. to expedite work. Conducts departmental performance evaluations in accordance with the facilities policies and procedures. Education
Educate staff regarding the role of the socialworker in the facility and the psychosocial needs of the residents and their families/significant others including the problems of aging and disability
Educate staff regarding cultural diversity and each staff member's importance when caring for residents. Educate staff regarding residents' rights and how to recognize and prevent abuse, neglect and mistreatment
Educate residents and families/significant others regarding their rights and responsibilities, effective problem solving and the extent of community, health and social services that are available to them, including those necessary for effective discharge planning
May supervise students assigned to social services in accordance with the respective school guidelines and monitor and document the progress of their work
Consultation
Social service consultation shall be provided by a MSW, LICSW on a planned basis with sufficient frequency to enable the socialworker(s), administration and facility staff to improve the quality of life of residents by meeting their psychosocial needs. The social work consultant shall have had prior experience with elders especially regarding placement and adjustment issues in nursing facilities. In addition, the social work consultant should be familiar both with regulations about social work in nursing homes and issues that might impact resident care when working on an interdisciplinary team. The consultative services should focus on each of the following three distinct groups:

Develop and maximize the socialworker's skills in psychosocial assessment, interviewing, documentation and treatment methods
