**$5,000 Sign on Bonus** The essential functions of this position include, but are not limited to, serving as a Behavioral Health Therapist for adolescent patients in the Intensive Outpatient Program (IOP). The BHT is responsible for planning and delivery of therapy and educational services to patients, and, if indicated, to their families and supports; monitoring and management of patient behavior in order to maintain the therapeutic environment and patient safety; completion of case management tasks; performs admission, discharge and utilization review functions; and effectively markets the program. Other functions may be assigned. The IOP BHT will also carry a supplemental caseload of individual psychotherapy patients as part of the MyMichigan Medical Group-Psychiatry department.
Responsibilities
(30%)* Responsible for IOP group therapy and psychoeducational groups and for individual therapy sessions. Also responsible for collateral contacts, as needed, and family sessions.
(20%)* Assists in screening IOP program applicants. Completes admission assessments. Creates and reviews treatment plans. Provides ongoing case management and discharge planning. Participates improving the patients' physical and mental health and maintaining patient safety.
(10%)* Participates in treatment team meetings to review IOP patient progress.
(30%)* Maintains a caseload of individual psychotherapy patients, including children/adolescents and/or adults. Completes all aspects of outpatient BHT role including assessment, treatment planning, coordination with team, and closures. Maintains outpatient productivity expectations.

Must have been involved in the delivery of mental health services for at least 1 year within the previous 5 years.

Therapy

* Ability to work independently, make accurate, and at times, quick judgments.
* Ability to respond appropriately to crisis outside of a hospital setting.
* Acceptance of and adaptability to different social, racial, cultural and religious modes.

Position Summary:
The Social Worker delivers varied social work services to Hospice patients and their families. Required Experience for Social Worker:
Required education and training for a social worker must include one or all of the following:
Must have MSW degree from a school of social work accredited by the Council on Social Work Education one year of experience in a health care setting.
Must have a baccalaureate degree in social work (BSW) from a school of social work accredited by the CSWE, and one year of experience in a health care setting and be supervised by a MSW from a school of social work accredited by the CSWE and who has one year of experience in a health care setting. If the BSW is employed by the Hospice before December 2, 2008, he/she is exempted from the MSW supervision requirement.
Have a baccalaureate degree in psychology, sociology or other field related to social work, and at least one year of social work experience in a health care setting and be supervised by a MSW from a school of social work accredited by the CSWE and who has one year of experience in a health care setting.
Ability to work independently, make accurate, and at times, quick judgments.
Ability to respond appropriately to crisis outside of a hospital setting.
Acceptance of and adaptability to different social, racial, cultural and religious modes.

Active patient contact within past three years, preferred.
Accountability for Social Worker:
Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in the development and implementation of goal-directed IDG care plan.

Assumes responsibility for own personal and professional development and maintenance of skills in social work.

Scope of Work

Social Worker Intake Coordinator
Policy:
Under the supervision of the Director of Intake and Enrollment, PACE Southeast Michigan (PACE SEMI) SW Intake Coordinator is responsible for the completion of the PACE SEMI intake process including initial home visits and assessments, assistance in obtaining Medicaid documents, and communication with the PACE SEMI Intake team. Communicates information about PACE SEMI program and other community services to potential participants through home visits and phone contact and assesses eligibility for enrollment into the PACE SEMI program.
Specific Duties & Responsibilities:
Meet with prospective enrollees and utilize Social Work assessment skills to determine eligibility of potential participants for the PACE program.
Responsible for all aspects of the Home Visit process: explaining PACE SEMI, completing all initial intake assessments, signing enrollment paperwork, etc.
Prepares written communication to Intake team introducing each intake candidate and updating daily on Home Visit results.
Responsible for inputting potential participant's information and assessments into TruChart.
Communicates regularly with Participant Advocate to ensure Interdisciplinary Team (IDT) has all needed information on the potential participant.
Works closely with eligibly specialist to ensure Medicaid and Medicare eligibility upon intake.
Coordinates timely follow-up with potential participants and their families thorough phone contact when needed.
Keeps confidential, all PACE SEMI or prospective PACE SEMI participant information, and/or all other information pertaining to PACE Semi or perspective PACE SEMI participants, issues, or business practices.
Other duties as assigned.
Knowledge, Skills, and Abilities:
Master's degree in Social Work required.
Limited or Full social work license required.
One or more year(s) of experience working with elderly in a community-based setting required. Must have knowledge of senior community and area resources.
Requires basic knowledge of Microsoft windows and Microsoft Outlook.
