Essential Duties & Responsibilities
Psychosocial Assessment & Intervention
Conducts psychosocial assessments of patients and families to identify needs related to medical conditions, social determinants of health, and overall functioning.
Interviews patients, families, and significant others to determine the need for social work services and develop appropriate treatment goals.
Participates in multidisciplinary rounds, providing psychosocial context and input into patient care planning.

Maintains up-to-date knowledge of psychosocial programs and services, providing relevant information to patients, families, staff, and community organizations.
Develops and maintains strong working relationships with community agencies.
Discharge Planning & Care Coordination
Coordinates comprehensive discharge planning to ensure continuity of care and access to appropriate aftercare services.
Arranges placements or referrals for Long-Term Care, Acute Rehab, Sub-Acute Rehab/SNF, LTAC facilities, Dialysis, Hospice, and Homelessness resources.
Ensures timely communication with patients, families, and interdisciplinary team members regarding discharge needs.
Documentation & Compliance
Completes psychosocial assessments, progress notes, family intervention summaries, and discharge planning documentation in accordance with department policy, and State, Federal, and Joint Commission standards.
Assists patients in obtaining governmental and community benefits such as food stamps, financial support, and transitional housing.
Ensures adherence to confidentiality standards and hospital policies.
Program Evaluation & Improvement
Evaluates program objectives and participates in continuous improvement initiatives aligned with hospital and departmental missions.
May accompany patients to access benefits, entitlements, or essential community services.

Ennoble Care is seeking a Full-time SocialWorker Care Manager (Hybrid) to service our patients within and around the Clinton, NJ area. This role is well-suited for early-career socialworkers looking to build experience in integrated behavioral health within a collaborative, team-based environment.

Support the mental and physical health care of patients on an assigned patient caseload.

Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient's medical provider. Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan. Case reviews may be conducted by telephone, video, or in person.
Track patient follow up and clinical outcomes using a registry. Document in-person and telephone encounters in the registry and use the system to identify and re-engage patients. Registry functions can be accomplished through an EHR build, on a spreadsheet used in conjunction with an EHR, or can be built into a stand-alone clinical management tracking system that may or may not be linked to an EHR.
Document patient progress and treatment recommendations in EHR and other required systems so as to be shared with medical providers, psychiatric consultant, and other treating providers.
Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care.
Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).
Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.
Be comfortable utilizing Excel and Word and completing assessments and productivity trackers daily.
Time-management skills are a must and the BHCM needs to be able to manage their day effectively through calendars and the EMR system.
This role is PRIMARILY telephonic, however in person visits are done and the BHCM has to be comfortable completing them both ways.
Qualifications:
The SocialWorker Care Manager will have a minimum of MSW
Preferred Licensure or provisionally licensed
Behavioral Health experience and knowledge
Must be licensed driver with an insured automobile in good working order
Ability to communicate effectively, both verbally and in writing.

Defines and maintains the standards of social work practice within the Center based on all dictates of the National Association of SocialWorker's Code of Ethics and the SocialWorkers Licensing Act of 1991 (N.J.S.A. 45:15BB-1 et seq.) and amendments thereto and with all the rules of the New Jersey State Board of Social Work Examiner
REQUIREMENTS:
EDUCATION & EXPERIENCE:
Master's Degree in Social Work or PhD in Clinical Psychology required
LICENSURE AND/OR CERTIFICATIONS:
Licensed in the State of New Jersey as a Licensed SocialWorker (LSW) required
Licensed in the State of New Jersey as a Licensed Clinical SocialWorker (LCSW).
Licensed Professional Counselors (LPC) and Licensed Addiction Counselors (LAC) can also be considered for this position.
KNOWLEDGE, SKILLS, ABILITIES AND OTHER (KSAO's)
Computer Skills: To perform this job successfully, an individual should have knowledge of Word Processing software.
Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.
Ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions.
Should be comfortable with the pace of primary care, working with an interdisciplinary team, and have strong communication skills.
Good knowledge of psycho-pharmacology.
Ability to design and implement clinical pathways and protocols for treatment of selected chronic conditions.
