The Hamilton County Health & Social Services Division has a responsibility to respond quickly and efficiently when public health emergencies occur. All Hamilton County Health & Social Services Division employees are responsible for ensuring their contact information is current and they are accessible should a public health emergency arise. The Hamilton County Health & Social Services Division maintains a call down system to store contact information and mobilize staff in the event of an emergency. During a public health emergency, all staff are required to respond should a call to mobilize be made. During a public health emergency, work hours for any and all Hamilton County Health & Social Services Division employees are subject to modification to meet the needs of the community.
Under general supervision, provides mobile medical services and/or social counseling, support, and clinical case management to the homeless population in an outpatient or outreach setting.
ESSENTIAL FUNCTIONS: (Essential functions, as defined under the Americans with Disabilities Act, may include the following tasks, physical requirements, knowledge, skills, abilities, and other characteristics. A Bachelor's Degree or an Associate's Degree in Nursing. Must possess an active Registered Nurse (RN) licensure or eligible for licensure as a Registered Professional Nurse. Current CPR Certification and a valid driver's license are required.
PREFERRED QUALIFICATIONS:
Prior work experience in working with homeless populations and/or serving in a clinical lead position.
ADDITIONAL REQUIREMENTS:
This position is deemed essential during inclement weather situations and must report to or remain at work, even when administrative closings are announced, as determined by the Department Head.
Prior to operating a County-owned vehicle, all employees are required to complete the HC Defensive Driving course and/or or an Emergency Vehicle Operations Course (EVOC) provided at the department level.
PHYSICAL REQUIREMENTS:
Work requires long periods of standing, walking, lifting medical supplies or patients weighing up to 50 pounds, assisting patients in transfers to examination tables, performing routine lab work such as blood draws and specimen collection, extensive contact with infectious diseases, mentally ill or combative patients, home visits, case management and attention to detail. Some positions require occasional after hours on-call duties.

Georgia Legal Services seeks a Domestic Violence staff attorney to work in its Dalton Regional office. The Dalton Office serves 6 counties in Northwest Georgia and is located about 20 minutes south of Chattanooga and 88 miles north of Atlanta.
This is a grant-funded position that will provide holistic legal representation in civil matters to victims of domestic violence in protective order, divorce, custody, benefits, and housing cases.
Primary Responsibilities:
· Litigate cases in administrative, state and federal forums.
· Draft briefs, motions and memoranda.
· Conduct community legal education sessions and engage in outreach activities.
· Participate on action teams that explore and solve systemic issues affecting clients.
· Collaborate with domestic violence shelters to provide legal services to victims of domestic violence.
· Participate in domestic violence task force meetings.
Requirements
· Graduate of accredited law school;
· Admission to the Georgia Bar, or willingness to sit for the next exam;

Provide Family Centered Treatment (FCT) Clinical Services as per the service definition and model fidelity requirements.

Convene the Treatment Team for Service Plan.
Consult with identified medical (e.g., primary care and psychiatric) and non-medical (e.g., Dept. of Child Services, school, Dept. of Juvenile Justice) providers; engage community and natural supports and include their input in the person-centered planning process.
Provide and coordinate behavioral health services and other interventions for the youth or other family members with other behavioral health professionals and Treatment Team members.
Qualifications
Minimum of a Master's Degree in human services or related field is required.

Responsibilities
As a Home Health SocialWorker, You will:
Guide and counsel individuals and families in crisis, ranging from the unemployed to children who need foster homes to elderly people who have no one to care for them
Assist clients in obtaining government funds, education, or treatment
Secure information, such as medical, psychological, and social factors contributing to the client's situation, and evaluate these and the client's capacities. Ask clients to explain problems, such as personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments, to determine the nature and degree of the problem
Help patients and families through individual or group conferences regarding plans for meeting needs and aids clients in mobilizing inner capacities and environmental resources to improve social functioning
Help the client to modify attitudes and patterns of behavior by increasing understanding of self, personal problems, and the client's part in creating them
Develop a service plan designed to restore the patient to optimum social and health adjustment within the patient's capacity
Qualifications
Master's Degree in Social Work
Five plus years of clinical social work experience
Valid and current Licensed Clinical SocialWorker or Licensed Medical SocialWorker license
Ability to make psycho-social assessments and develop and implement viable care plans

LICENSURE/CERTIFICATION/EDUCATION REQUIREMENTS
Minimum of a Bachelors Degree from an approved social work course study.
OTHER TRAINING, SKILLS, AND EXPERIENCE REQUIREMENTS
Minimum of two (2) years experience in a supervisory capacity in a hospital, skilled nursing center or other related medical center.
ESSENTIAL SKILLS/KNOWLEDGE FUNCTIONS
* Plan, develop, organize and direct the Social Services Program.
* Coordinate and participate in family and patient activities designed to promote social interaction, reality orientation, and intellectual stimulation.
* Counsel patients and families including dealing with feelings about death or dying and other emotional, mental, environmental or physical limitations.
* Complete the social service portion of the MDS within 14days of admission.
* Complete social documentation in line with procedural guidelines.
* Coordinate admissions and discharges.
* Act as liaison with social, health and community agencies.
* Coordinate family and community resources as needed to solve financial needs and to promote emotional security.
* Facilitate patient and family councils as appropriate.
* Assist with grievance resolution when requested.
* Understand appropriate documentation for Advance Directives (Living Will, DNR, DPAH).

* Participates in nursing center surveys (Licensure/JCAHO) and any subsequently required reports.
* Attends and participates in continuing educational programs to keep abreast of changes in your field as well as to maintain current licensure/certification, as required.
* Attends and participates in mandatory in-services.
* Follows established safety procedures when performing job tasks and/or working with equipment.
* Honors patients rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints.
* Complies with Corporate Compliance Program.
* Reports job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessary.
* Follows established safety regulations, to include fire protection/prevention, smoking regulations, infection control, etc.
* Performs other related duties as necessary and as directed by supervisors.
