Position Summary:
The Social Worker is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources, balanced with the patient's resources and right to self-determination. The individual in this position has overall responsibility for to assess the patient for transition needs including identifying and assessing patients at risk for readmission. Conducts complex psycho-social assessment and intervention to promote timely throughput, safe discharge, and prevent avoidable readmissions. This position integrates national standards for case management scope of services including: Transition Management promoting appropriate length of stay, readmission prevention, and patient satisfaction; Care Coordination by demonstrating throughput efficiency while assuring care is the right sequence and at appropriate level of care; Compliance with state and federal regulatory requirements, TJC accreditation standards, and Tenet policy; Education provided to physicians, patients, families, and caregivers; and Leads a population of patients by service line and/or leads the team by being a resource to Tenet performance standards.

Completes comprehensive in-home psychosocial evaluations. Evaluations include:
Consideration of developmental, social and/or cultural, substance abuse, psychological, environmental, and medical issues
Physical, social, emotional, and familial characteristics
Use of evidence-based assessment tools to determine cognitive status and behavioral health needs including Dementia, Depression, Anxiety, etc.
Home safety, family dynamics, caregiver strain, and other risk factors including suicide
Patient/family understanding of their disease treatment options
Strengths, available supports, and barriers to care
Determine appropriate level of care including needs for long/short-term skilled-nursing facilities, assisted living facilities, adult day care, etc.
Based on a comprehensive, culturally competent assessment collaborates with the patient and family to create a care plan with measureable goals. Implements short-term interventions to address identified barriers and promote health and well-being.
Provides clinical treatment services in form of triage, crisis intervention, individual or family therapy, screening and planning
Utilize Motivational Interviewing and Coaching skills to assist patients/families in identifying and addressing goals. Using identified strengths, encourage self-determination and independence
Act as educators for patients, families, the community, and other professionals regarding disease prevention, disease progression, impact of illness, health maintenance, and adherence to treatment regimens
Advocate for the needs and interests of patients to improve access to care and improved delivery of services
Provide ongoing assessment of patient and family needs. Monitor and alter care plan goals and interventions as necessary
Maintains timely documentation of social work services, which reflect the patient and client systems' pertinent information for assessment and treatment; social work involvement and outcomes.
Facilitates team based care and collaboration
Work collaboratively with primary care and specialty physicians and other members of the health care team to improve quality of life

The Medical Social Worker participates in the interdisciplinary care provided to home health patients. The Medical Social Worker functions to evaluate and develop a plan of care personalized to fit the patient's emotional and social needs. The Medical Social Worker provides direction and supervision of the Social Worker Assistant as required and when involved in the patient's plan of care. The Medical Social Worker works within CenterWell Home Health's company-specific policy and procedures, applicable healthcare standards, governmental laws, and regulations.
Assesses the patient's social and emotional state as it relates to his or her illness or injury, needs for care and his or her response to such treatment, and adjustments to care.
Assesses any relationships of the patient's medical and nursing needs in the home setting, financial resources, and available community resources.
Provides any appropriate action to obtain available community resources to assist in resolving issues that may be impeding the patient's recovery.
Instructs patients and families in treating and coping with social and emotional response connected with Provides ongoing assessment of patient and family needs and responses to teaching
Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient's health Participates in the development and periodic re-evaluation of the physician's Plan of Care for the patient.
Observes, records, and reports changes in patients' condition and response to treatment to the Clinical Manager and the Participates in the discharge planning process
Participates as a member of the interdisciplinary care team in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs
Supervises instructs and evaluates the performance of the Social Work Assistant (BSW) to assure that all medical social services are provided to patients in compliance with Company, government, and professional standards
Maintains and submits documentation as required by the company and/ or facility including any case conferences, patient/physician community contacts, visit reports progress notes, and confers with other health care disciplines in providing optimum patient.

Required Skills/Experience
Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education.
Social Worker licensure in the state of practice; if required by state law or regulation.
A valid driver's license, auto insurance, and reliable transportation are required.
Proof of current CPR certification
Minimum of one year of experience as a social worker in a health care setting, home health, and/or hospice.
Knowledge of and the ability to assist with discharge planning needs, and to obtain community resources (housing, shelter, funeral/memorial service arrangements, legal, information and referral, state/federal financial and medication programs, and eligibility.
Excellent oral and written communication and interpersonal skills.
Must read, write and speak fluent English.
Knowledge of medications and their correct administration.
Ability to organize tasks, develop action plans, set priorities, and function under stressful situations.
Ability to be flexible in work hours and travel locally.
Ability to communicate effectively with patients and their family members and at all levels of the organization.
Maintains current licensure certifications and meets mandatory continuing education requirements.
Must read, write and speak fluent English.
Must have good and regular attendance.

Social Worker - Pioneer Valley Hospice & Palliative Care Weekly Pay | Mileage Reimbursement
Pay Range: $32 - 42/hr (based on experience)
This is a full-time 40-hour per week position. The purpose of this position is to provide initial psychosocial evaluations, ongoing psychosocial counseling, direct casework services, bereavement services, and community education, outreach, and referrals. Responsible for the delivery of varied social work services to a caseload of hospice patients and families, and actively participate as a member of the hospice interdisciplinary team. The hospice social worker may also provide visits or assistance to other patients/families being served by programs of the hospice as needed.
Essential Job Functions
Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group. Provides an assessment in the patient's identified residence and assistance when this is not a safe environment, or an alternative caregiving plan is indicated.
Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect, or abuse, and plans intervention based on evaluation findings. Counsels patient and family/caregivers as needed in relation to stress and other identified coping difficulties. Provides crisis intervention when necessary.
Assesses for and educates the interdisciplinary group on any special needs related to the culture of the patient and family. Includes communication, role of family, space, and any special traditions or taboos.
Maintains clinical records on all patients referred to social work.
Point of Service documentation is an expectation of all clinical personnel for quality care, accuracy, and communication standards. The clinician is expected to appropriately document, have timely synchronization of patient data & effectively communicate with the Patient Care Team.
Educates patients and families on, and assists in, preparation of advanced directives, durable power of attorney, and funeral arrangements as needed.
Provides information and referral services for the organization's patients and families/caregivers regarding practical and environmental needs.
Develops and maintains contacts with public and private agencies as resources for patients/caregivers. Serves as liaison between patients and families/caregivers and these agencies.
Maintains collaborative relationships with organization personnel to support patient care.

Qualifications

Job Description
The role of the PC is to assist clients in strengthening and establishing lifelong connections that will facilitate success throughout their lives. Working as part of a team that is led by the Permanency Specialists (PS), the PC meets with clients to provide support and counseling to them, their parents and family members. PCs are expected to work independently and be guided by the treatment plan. PC's typically provide some or all of the following services: strength-based counseling and support that includes working with the school or employer (if applicable), to monitor functioning and activities and provide coaching based on interests and or needs. This could include job searching, monitoring school attendance, finding pro-social community-based activities and encouraging the youth to engage in the activities that may assist in building their self-esteem and the positive connections that they have.
Representative Duties
· Meet individually with clients, or with family if identified by PS directives, and meet hours required by contract with DCF or DMH
· Provide mentoring and identify client and family strengths
· Communicate with local schools and/or job sites if appropriate
· Support clients in completing community service if appropriate
· Support clients and their families at night & on weekends as needed or when on call
· Set up pro-social activities for clients in their home community
