What You'll Do

As part of the Care Response Team, provides screening, crisis intervention and crisis assessment services to individuals experiencing a mental health or substance use crisis following a call to 911 or 988. The Crisis Intervention Specialist will respond in the community with a Peer Support Specialist and be responsible for providing brief mental health and/or alcohol and other drug screenings, crisis intervention, and appropriate recommendations for care and linkage to services to a diverse population of clients. Services will be provided in a community-based environment.
Primary responsibilities:
Completes initial screenings/assessments for mental health and alcohol or other drug with individuals in a behavioral health crisis and have reached out for help by calling 911 or 988.
Along with a Peer Support Specialist, complete community-based assessments with individuals in crisis and facilitate linkage to appropriate services.
Provide follow-up services to those previously assessed to assure linkage to on-going providers.
Schedule:
40-hour work week, various days, third shift (including weekends)
This is a non-exempt, union position with a minimum-starting wage of $38.46 per hour.

Position Description:
The MyCare Ohio Case Manager is responsible for the assessment and care management of long- term clients ages 60 and older enrolled in the waiver programs. Social Worker (LSW) licenses is required. At least one year of experience working with individuals with physical or behavioral disabilities in a home health care, medical social work, or behavioral health environment is required.
