    Overview Good Shepherd Hospice is seeking a compassionate Social Worker to serve our patients and their families in Columbia and surrounding communities. The Hospice Social Worker provides medical social services to the patient and family in accordance with the interdisciplinary Plan of Care. Responsibilities Assists the physician and other members of the care team in understanding significant social and emotional factors related to the patient's health problems. Assesses the social and emotional factors within 5 days of admission in order to estimate the patient's and family's capacity and potential to cope with the problems of daily living and with the terminal illness. Helps the patient and family to understand, accept and follow medical recommendations and provide services planned to assist the patient and family in achieving the optimum social and health adjustment within their capacity. Utilizes all available resources, such as family and community agencies, to assist the patient and family to live better within the limitations of the illness. Observes, records, and reports social and emotional changes. Documents all services in the patient's clinical record. Participates as a member of the Interdisciplinary team and actively participates in the development and review of the plan of care for assigned patients. Participates in ongoing educational programs. Participates on committees and performs other duties and special projects as assigned. Completes all required documentation in a timely manner according to Good Shepherd Hospice policies and procedures. Supports staff with loss and patient/family issues. Currently, we are assisting over 640 foster families, over 1400 foster children, over 400 adopted children in our service area. Our goal is to be a support to the families, children and youth in this area, and we work tirelessly to provide much needed services to them. Position Summary 30 Days to Family is a short-term intervention to diligently search for a child's relatives and kin to identify potential placement options and supports. The initial search is for parents, grandparents, and siblings, but the goal is that at least 80 additional relatives will be identified and explored. Because placements can be fragile, the goal is to identify one primary placement provider and one to two other relatives or kin as potential back-up placement providers. Maternal and paternal relatives are explored with equal interest. In all cases, every effort is made to search for relatives and kin who will keep siblings together, maintain children in their school of origin, and preserve the child's important relationships with friends and supportive adults. Essential Functions/Job Expectations Effectively work an average of two cases, serving approximately 30-35 youth per year Identify and document at least 80 family members for each case through diligent search activities, including: internet and database searches, child protection and court case-file reviews, and child and family engagement interviews. Collect information from relatives/kin to expand the child's support system and family connections, while identifying potential relative home providers. Assist in preparing the family for licensure by explaining the licensing process and assisting with home licensing requirements. Facilitate communication with the child's professional team and provide the case manager with timely and accurate documentation, including detailed family genograms, diligent efforts made by the Specialist, family contact information, and identified family supports. Facilitate and attend all meetings and court hearings, advocating for relative/kin placement. Complete assessments on the child and family functioning upon case referral, case closure, and 30 days following case closure to measure the success and outcomes of the program. What You'll Do As a key member of the interdisciplinary team, you will: Clinical Care Provide individual, family, and group therapy to LTC residents. Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning. Support residents with depression, anxiety, adjustment to illness, grief, trauma, substance use, behavioral disturbances, and other conditions common in LTC settings. Deliver behavioral management and brief, goal-oriented interventions aligned with best practices in geriatric care. Interdisciplinary Collaboration Work closely with nursing, social services, rehabilitation, and medical providers to coordinate treatment and improve resident outcomes. Participate in care plan discussions, facility meetings, and case collaborations as needed. Documentation & Compliance Maintain complete and timely documentation using our Geriatric Notes Tool (GNT). Follow ethical, professional, and regulatory guidelines for clinical documentation, confidentiality, and care delivery. Facility Partnership Build strong, professional relationships with facility staff and leadership.
