This is a limited term position anticipated to last approximately 2 years. Hiring Range: $27.36-$29.77 per hour Work Schedule: Monday-Thursday, 7:30 a.m.-5:00 p.m. and Friday, 7:30 a.m.-11:30 a.m. with some evenings and weekends as needed. No residency requirement. Examples of Duties Conducts regular outreach to establish rapport with individuals experiencing homelessness in various locations such as streets, shelters, parks, and encampments. Builds trusting relationships with homeless individuals through compassionate communication and active listening. Conducts comprehensive assessments to understand the unique needs and challenges faced by each homeless individual or family. Identifies factors contributing to homelessness, including mental health issues, substance abuse, medical conditions, and lack of resources. Collaborates with social service agencies, shelters, healthcare providers, and other relevant organizations to connect homeless individuals with essential services such as shelter, food, clothing, medical care, mental health support, and substance abuse treatment. Provides information and referrals for job training, employment services, and housing programs. Assists individuals with the completion of necessary paperwork/applications to receive services. Navigates the resource landscape with homeless individuals to better connect them to needed services and support within the community. Maintains accurate and up-to-date records of client interactions and service referrals. Develops individualized service plans in collaboration with homeless clients, setting goals and milestones for transitioning out of homelessness. Monitors and follows up on clients' progress toward achieving their goals. Responds to crises and emergencies as they arise, ensuring the safety and well-being of homeless individuals. Connects individuals in emergency danger to emergency shelters and services. Works closely with the Green Bay Police Department and other homeless outreach specialists in assisting homeless individuals in crisis. Compiles and submits accurate data and reports on outreach activities, client demographics, and outcomes. Assists in evaluating the effectiveness of outreach programs and identifying areas for improvement. Performs other duties as assigned. Minimum Qualifications Required High school diploma or equivalent. Bachelor's degree in social work, psychology, sociology, or a related field preferred. 1-3 years related experience in homeless outreach, case management, or social services. Valid driver's license and good driving record. A combination of equivalent experience and/or education may be considered. Knowledge, Skills & Abilities Knowledge of local social service agencies, resources, and housing programs. Strong interpersonal communication skills, including empathy and active listening. Skill in working with people of diverse backgrounds and experiences. Skill in analyzing problems and developing appropriate recommendations and solutions. Skill in multitasking and handling multiple projects concurrently. Ability to work independently and as part of a team. Ability to handle confidential material with discretion. Ability to work utilize a computer and the required software. Ability to communicate effectively, both orally and in writing. Ability to establish effective relationships with staff and the public. Under the general direction of the Health Division Manager, responsible for the psychosocial services and support to hospice patients and families. Coordinates hospice bereavement at the start of admission to hospice through 12 months after the hospice patient is deceased. Provides psychosocial support and counseling in accordance with the wishes and needs of the patient/family. Participates in the development, update, and completion of the Interdisciplinary Group (IDG) plan of care, including a bereavement plan of care for the patient and family and attends and participates in the IDG meetings. Regularly assesses the hospice patient's social service needs and implements the plan of care provisions to meet those needs. Provides ongoing psychosocial assessment of the family's coping capacity relative to the patient's terminal condition. Links patient and family with needed community resources to meet ongoing social, emotional and economic needs. Our Crisis Center is seeking a full-time Overnight Crisis Counselor to work at our Green Bay location. The Crisis Center provides responsive and professional crisis counseling services on a 24-hour basis to all residents of Brown County. The intent of the Crisis Center's services is to provide short-term and responsive crisis intervention services to individuals in crisis situations. The Crisis Counselor works to resolve the immediate emotional impact of the client's crisis situation. The Crisis Counselor may assist the client in activating their problem solving skills and developing some additional coping strategies to resolve his/her conflicts. The Crisis Center offers short-term intervention; it does not provide on-going therapy or case management. We work temporarily with clients of other systems to bridge the gap between immediate or emergency needs with long-term treatment or therapy programs. The Crisis Center attempts to form appropriate links between the immediate service needs of an individual in crisis and those community agencies that can serve their needs. A Bachelor's degree in the Human Services field is required. Extensive training is provided to new hires so new graduates are encouraged to apply. Full-time staff with Masters Degrees are provided the opportunity to earn their 3,000 hours. Successful candidates are committed to our mission and have a working knowledge of human growth and development, mental health, adolescent psychology, marital and family structure, and the impact of the environment on individual behavior. Candidates also need to be able to establish and maintain relationships within and outside the agency in a professional manner.
