The Social Services Coordinator position provides social services for the County inpatient nursing facility, working with patients and families on admission and discharge, as well as providing assistance during residency in the facility.

This position reports directly to the Assistant Nursing Home Administrator.

This position is not responsible for the supervision of other employees.
ESSENTIAL DUTIES OF THE POSITION
Conducts interviews of applicants and families for admission to facility and processes application documents and consent forms.
Collects information on family, social, and financial history/status. Establishes written plans to meet the needs of the resident.
Assists with problem resolution for the resident and/or their family. Facilitates arrangements for various support services for residents and/or their family.
Investigates and attempts to resolve complaints received by or on behalf of the client population served.
Assesses information to determine likelihood of acceptance under state-established financial criteria and medical eligibility qualification requirements.
Contacts external social service, hospital, and other agencies to ensure complete social history and evaluation.
Coordinates discharge planning.
Attends care conferences and contributes to care planning.
Completes Social Services section of MDS (Minimum Data Set) forms, admissions assessment, quarterly assessments, and care documentation.
Suggests alternative resources for applicants not eligible for admission.
Coordinates admissions with patient, prior social service, medical, or other agency and facility staff.
Prepares prospective, and orients new, patients and their families to facility, detailing rules and operating procedures, daily routine, activities, therapy, and living conditions.
Prepares appropriate reports which includes findings, conclusions, and recommendations.

REQUIRED MINIMUM QUALIFICATIONS
EDUCATION & EXPERIENCE - Graduation from an accredited college or university with a Bachelor's degree in Social Work, Sociology, Special Education, Rehabilitation, Counseling, or Psychology; AND
At least one (1) year of supervised full time social work experience in a geriatric setting; OR
A Master's degree in Social Work, Psychology, Sociology, or Counseling from an accredited college or university may be substituted for the experience requirement.
CERTIFICATION - Employee will be required to become certified as a Helping Hands Feeding Assistant. This is to provide assistance in feeding/tray preparation, delivery, and encouraging residents at meal times, if necessary.
KNOWLEDGE, SKILLS, AND ABILITIES
Thorough knowledge of medical casework records, documentation methods, and techniques.
Knowledge of social service goals, objectives, principles, and practices pertaining to the provision of services to patients in an inpatient nursing facility.

DUTIES & ESSENTIAL JOB FUNCTIONS
Works with program leadership in the design, implementation, and evaluation of the program's objectives to the underserved, uninsured, underinsured, and newly insured populations.
Assists with state required functions for Medicaid members including Age Out Transition Procedure and resources for members in substitute care, waiver programs or facing an emergency placement situation.
Assists with developing coordinated care plans for members with complex medical/social/behavioral health needs. Fosters a collaborative team approach by working with the member, family, primary care provider, behavioral health clinician, community agencies, and other members of the treatment team to ensure coordination of services.
Assist to identify outreach, wellness and education planning needs of the community member and communicate findings to the treatment team.
Coordinates referrals between and among physical, behavioral and dental health providers and other community resources to improve overall community member outcomes. Ensures appropriate clinical management information is shared with peers, providers and outside agencies in a timely fashion while securing system privacy standards.
Works closely with members to appropriately apply insurance benefits or obtain insurance benefits. Serve as a resource for the member and the healthcare team.
Maintains required documentation for all program related activities. Collect data and utilize data to adjust the care plan when indicated.
Acts as a patient advocate in order to coordinate required services or to resolve emergency problems in crisis situations. Provides individual and/or family education/counseling to assist in establishing members' overall wellbeing.
Utilizes evidenced based counseling techniques such as motivational interviewing and solution focused skills with focus on conflict resolution, assertiveness, problem solving, and decision making to assist members with effectively negotiating the healthcare continuum.

Educates patients on the proper use of the Emergency Department and provides information for alternatives. Coaches patients in effective management of their chronic health conditions and self-care. Assists patients in understanding care plans and instructions. Motivates patients/patients to be active and engaged participants in their health and overall wellbeing.
Continuously expands knowledge and understanding of community resources and services. Facilitates patient access to community resources, including locating housing, food, clothing, prenatal classes, parenting, and relevant mental health services. Assists patients in utilizing community services, including scheduling appointments with social services agencies and assisting with completion of applications for programs for which they may be eligible.
Facilitates communication and coordinate services between providers and the patients/patients. Coordinates and monitors services, including comprehensive tracking of patients' compliance in relation to care plan objectives.
Works collaboratively and effectively within a team. Establishes positive, supportive relationships with participants and provides feedback to other members of the team. Builds and maintains positive working relationships with the patients, providers, care managers, medical residents, and office staff. Works to reduce cultural and socio-economic barriers between patients and institutions.
Attends weekly huddles and morning/afternoon mini huddles.

Ennoble Care is seeking an experienced Full-time Behavioral Health Specialist for our department supporting patients within and around the Clinton, NJ area. This position is primarily remote, with the possibility of occasional in-person visits in the future based on patient needs.
Responsibilities:
Support the mental and physical health care of patients on an assigned patient caseload.
Screen and assess patients for common mental health and substance abuse disorders. Facilitate patient engagement and follow-up care.
Provide patient education about common mental health and substance abuse disorders and the available treatment options.
Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
Support psychotropic medication management by consulting with the behavioral health consultant and then communicating with the primary care provider assigned.
Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.
Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments (e.g. problem-solving treatment or behavioral activation) as clinically indicated.
Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient's medical provider. Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan. Case reviews may be conducted by telephone, video, or in person.
Track patient follow up and clinical outcomes using a registry. Document in-person and telephone encounters in the registry and use the system to identify and re-engage patients. Registry functions can be accomplished through an EHR build, on a spreadsheet used in conjunction with an EHR, or can be built into a stand-alone clinical management tracking system that may or may not be linked to an EHR.
Document patient progress and treatment recommendations in EHR and other required systems so as to be shared with medical providers, psychiatric consultant, and other treating providers.
Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care.
Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).
Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.
Be comfortable utilizing Excel and Word and completing assessments and productivity trackers daily.
Time-management skills are a must and the BHCM needs to be able to manage their day effectively through calendars and the EMR system.
This role is PRIMARILY telephonic, however in person visits are done and the BHCM has to be comfortable completing them both ways.

Summary

Demonstrates expertise in various therapeutic modalities, the DSM 5 Classification system, multi-cultural issues, and employing counseling and education skills to empower their patients.
Completes medical record documentation in a timely manner in accordance with program, departmental, regulatory, third party payor, and network compliance standards.
Works collaboratively with both internal/external customers and teams. Uses knowledge of available resources to ensure patient care needs are met and transitions of care coordinated.
Minimum Qualifications
Master's Degree in counseling, social work, or other clinical discipline, such as psychology.
2 years experience in counseling or social work in a mental health related field.
Ability to use good judgement to respond to emergencies.
Ability to establish program goals and objectives that support the strategic plan.
Comprehensive understanding of behavioral health services and appropriate levels of care.
Familiarity with differential diagnosis of common mental health and/or substance use disorders.

Ennoble Care is seeking a Full-time SocialWorker Care Manager (Hybrid) to service our patients within and around the Clinton, NJ area. This role is well-suited for early-career socialworkers looking to build experience in integrated behavioral health within a collaborative, team-based environment.
This position will be primarily remote with occasional travel to patients homes/facilities.
Responsibilities:
Support the mental and physical health care of patients on an assigned patient caseload.
Screen and assess patients for common mental health and substance abuse disorders. Facilitate patient engagement and follow-up care.
Provide patient education about common mental health and substance abuse disorders and the available treatment options.
Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
Support psychotropic medication management by consulting with the behavioral health consultant and then communicating with the primary care provider assigned.
Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.
Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments (e.g. problem-solving treatment or behavioral activation) as clinically indicated.
Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient's medical provider. Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan. Case reviews may be conducted by telephone, video, or in person.
Track patient follow up and clinical outcomes using a registry. Document in-person and telephone encounters in the registry and use the system to identify and re-engage patients. Registry functions can be accomplished through an EHR build, on a spreadsheet used in conjunction with an EHR, or can be built into a stand-alone clinical management tracking system that may or may not be linked to an EHR.
Document patient progress and treatment recommendations in EHR and other required systems so as to be shared with medical providers, psychiatric consultant, and other treating providers.
Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care.
Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).
Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.
Be comfortable utilizing Excel and Word and completing assessments and productivity trackers daily.
Time-management skills are a must and the BHCM needs to be able to manage their day effectively through calendars and the EMR system.
This role is PRIMARILY telephonic, however in person visits are done and the BHCM has to be comfortable completing them both ways.
Qualifications:
The SocialWorker Care Manager will have a minimum of MSW
Preferred Licensure or provisionally licensed
Behavioral Health experience and knowledge
Must be licensed driver with an insured automobile in good working order
Ability to communicate effectively, both verbally and in writing.
Time Management skills
Prior behavioral health experience is helpful but not required. New MSW graduates are encouraged to apply. The salary for this position starts at $55,000.

Summary
