Basic Requirement for SocialWorker (Clinical):
Degree: Master's degree (or higher degree) in social work from a school accredited by the Council on Social Work Education.
In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:
Specialized Experience:One year of specialized experience which includes conducting bio-psychosocial evaluations of behavioral health patients, providing individual or group clinical counseling, and developing behavioral health treatment plans or recommendations. This definition of specialized experience is typical of work performed at the next lower grade/level position in the federal service (GS-11).

Your Mission in Action
The Family Services Worker (FSW) provides trauma informed, strengths based case management and direct services to foster care or adoptive children, parents, and their families, including but not limited to individual counseling, prevention, administration, advocacy on behalf of the child, parents, and/or families with social services, the court, police, school, and other medical and mental health providers and professionals in accordance to Agency and licensing standards and federal, state, and local regulatory requirements. These services are provided with a trauma-informed care approach and evidenced based or promising practices that support child safety, permanency and well-being.
Responsibilities
Conduct regular home visits and on-going assessment to assess child safety, wellbeing and placement stability to ensure foster care children and homes remain viable for placement and meeting their specific needs
Develop and implement a case plan to establish individualized goals focusing on healing and permanency using appropriate and necessary resources to maintain placement, minimize risk and provide for the safety of the children.
Ensure children's voices and needs are prioritized in case planning while advocating for necessary services in collaboration with their service planning team to provide comprehensive care.
Provide emergency and on-call services when needed
Provide supportive services and/or other resources for foster care children with emotional, social, behavioral, or mental health needs
Assess the safety of the child; take the necessary steps (actions) to protect the child if deemed appropriate in accordance with Agency standards and federal, state, and local laws and regulations
Provide stabilization services (crisis intervention) for foster children, parents, and their families in a timely manner
Determine action to be taken to remove or to reduce an immediate threat to the safety of the child; arrange for temporary placement in another foster care or group home or treatment facility
Follow a formal process for case reviews and informal transfer between agencies and referral sources that appropriately meet the needs of the child, parent, and/or family
Obtain needed information and complete regular reports regarding foster care children progress
Work closely with management, staff, service providers and professionals, and various local community systems (education, medical, behavioral health, etc.,) resources to ensure foster care children, parents, and families are receiving ongoing and services
Accurately document all relevant and appropriate information related to the child, parents, and/or families in accordance with Agency standards, state licensing requirements, and other federal, state, and local regulatory requirements
Build strong relationships with community partners to expand resources for children and families to include participation in team meetings, case staffings and court proceedings as a part of child well-being
Provide foster families with education on relevant child wellbeing topics to include agency generated trainings as well as PRN training needs.
Assist with child placement and post-placement services, which may include identifying and contacting the appropriate foster parent and coordinating arrangements between the placement agency and the foster home
Provide an aftercare plan to youth exiting placement
Provide time-specific follow-up meetings that may include referrals for new or additional services
Develop and maintain positive and strong working relationships with the Foster Care and Adoption teams and foster care parents and families, while maintaining professional and ethical boundaries always
Offer emotional support and crisis intervention as well as assist in conflict resolution and mediation between foster children, their caregivers, members of the service planning team and outside stakeholders.
Work as team player and exhibit professional behavior and a positive attitude with both internal and external clients that reflects positively on the Agency and is consistent with Agency policies and practices
Actively participate in exhibiting cultural awareness and sensitivity when interacting with children, parent(s)/guardian(s), staff, and the community

Summary: The Street Outreach Workers conduct trauma-informed, face-to-face outreach to youth ages 16-21 experiencing homelessness or at risk. They provide gateway services, basic needs support, and facilitate connections to emergency shelter, transitional housing, and other resources. Outreach is conducted in collaboration with law enforcement, schools, and community partners. Workers also support referrals to the agency's licensed Youth Emergency Shelter (BCP) and contribute to the agency's performance and quality improvement efforts. The scope of work is subject to change as needed to ensure the project's success and to meet funders' requirements. Likewise, complementary changes may be made to projects or individual goals and expectations for employees based on agency/management strategic goals. All Center for Children team members are expected to be collegial and collaborative, mutually supportive, and focused on excellent services to clients, fidelity to program models, and ethical provision of services.
Primary Duties and Responsibilities
Conduct outreach in high-risk areas (e.g., parks, schools, bus terminals, colonias).
Distribute comfort packs with hygiene items, food, and program information.
Provide immediate crisis intervention and mediation on the streets, utilizing distraction and diversion techniques to de-escalate issues.
Respond to chat line inquiries and engage youth via social media platforms.
Complete initial screenings (Roberts Rapid Risk Assessment, CST, ASQ, QYIT).
Refer minors under 18 to the Youth Emergency Shelter (BCP) and coordinate intakes.
Assess referrals from the Youth Emergency Shelter for TLP eligibility.
Develop individualized safety plans and ensure youth safety.
Provide or coordinate safe transportation to shelter or Drop-In Center.
Monitor shelter bed availability via HMIS and collaborate with shelter staff.
Ensure continuity of care from initial contact through housing placement.
Partner with law enforcement, CPS, schools, and CoC agencies to strengthen referrals.
Respond to HMIS and Coordinated Entry referrals, ensuring warm handoffs.
Build partnerships with schools, healthcare providers, and community organizations.
Maintain timely documentation in RHYHMIS and ExtendedReach.
Becomes an expert on TLP program standards via resources on the Health and Human Services website and the Runaway and Homeless Youth Training and Technical Assistance (RHYTTAC).

International Health and Medical Services is looking for an experienced licensed independent clinical socialworker who serves as a subject matter expert (SME) in mental health and substance use disorder(s). The Behavioral Health Provider (BHP) possesses a high level of skill in the assessment, diagnoses, and treatment of patients with mental health concerns, and/or serious mental illness or serious mental disorders (SMI/SMD). The BHP conducts mental health evaluations to enhance the behavioral health treatment plan development and services for detainees with SMI/SMD to include referral to acute and/or inpatient mental health care as clinically determined.
The BHP provides direct mental services, including on-site counseling, individual and group psychotherapy within diverse therapeutic modalities, substance abuse education, and behavioral analysis for detainees identified with mental health and substance use problems. This role conducts specialized programming within a Modified Therapeutic Treatment Community model. The BHP also consults with medical, contract staff, and other healthcare professionals to conduct and provide education and training on continuity of care, treatment, monitoring and management of patients with co-occurring diagnoses.
Provides direction and oversight to multi-disciplinary team in the implementation and management of an integrated mental health treatment program.

Required Education and Experience:
Master's degree in social work from a program accredited by the Council on Social Work Education (CSWE).

Job Duties:
Duties shall consist of, but are not limited to, the following:
The LCSW may be involved with seeing service members who score low, moderate, or high on the Behavioral Health Screening tools in use. The purpose will be to determine if the service member is able to return from active duty to his/her home of record with a referral for continued services at home or need to stay at Ft. Bliss for continued counseling and/or treatment.
Provide initial screening of service members who screened positive to the Behavioral Health. Screen deploying SM using COCOM requirements to determine eligibility for deployment. Review waivers to determine if they meet COCOM requirements.
The LCSW will make clinical decisions on the need for further counseling and/or treatment.
Assess a high volume of clients to determine extent of their issues and if the issues require further assessment and potential treatment.
Consult with supervisor or CP to determine best course of action for the service member and his/her unit of assignment and their mission.
Discuss pertinent issues with the Commanders and First Sergeants as applicable within the HIPAA guidelines.
Use of telecommunications and information technologies to provide health assessment, diagnosis, treatment, consultation, education, and health-related information across distances.
Document services provided. Maintain documentation of all service provided in accordance with applicable directives and prepare such records and reports as may be required by the policy.
Ensure all documentation and reports are legible and complete. The CP shall prepare all documentation to meet or exceed established standards to include but not limited to timeliness, legibility, accuracy, content and signature.
Crisis intervention (contact hospital for transport or call MP's. Referral to VHA as appropriate.
Complete each encounter at the end of the day and appropriately close the record no later than 24 hours.
Shall see the patient per hour average of the other collective CPs, unless affected by circumstances beyond their control, which shall be reported to the Section OIC/NCOIC or designee.
Shall comply with all MTF productivity standards and maintain a level of productivity comparable with that of other individuals performing similar services.
Shall abide by all clinical standards, rules, and procedures including requirements for licensure, credentialing, and quality assurance programs.
Shall comply with all health requirements, immunizations, and vaccinations.
Shall complete all DoD, DHA, and MTF-specified training requirements.
Shall personally record hours worked in the Defense Medical Human Resources System - internet (DMHRSi) system, or other system, as required by the MTF.

We are seeking a Licensed Clinical SocialWorker (LCSW) to join our integrated clinical team. In this role, you will work onsite in one or more long-term care facilities within your local community, providing therapy, assessment, care planning, and behavioral health support. What You'll Do
As a key member of the interdisciplinary team, you will:
Clinical Care
Provide individual, family, and group therapy to LTC residents.
Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
Support residents with depression, anxiety, adjustment to illness, grief, trauma, substance use, behavioral disturbances, and other conditions common in LTC settings.
Deliver behavioral management and brief, goal-oriented interventions aligned with best practices in geriatric care.
Interdisciplinary Collaboration
Work closely with nursing, social services, rehabilitation, and medical providers to coordinate treatment and improve resident outcomes.
Participate in care plan discussions, facility meetings, and case collaborations as needed.
Documentation & Compliance
Maintain complete and timely documentation using our Geriatric Notes Tool (GNT).
Follow ethical, professional, and regulatory guidelines for clinical documentation, confidentiality, and care delivery.
Facility Partnership
Build strong, professional relationships with facility staff and leadership.
Serve as a behavioral health resource and trusted partner in supporting residents' mental and emotional needs.

Posterity Group LLC is a veteran owned, service-disabled small business, headquartered in Rockville, MD, that specializes in federal government contracting and staffing. We are tasked with staffing The William Beaumont Army Medical Center and associated Branch Clinics with Licensed Clinical SocialWorkers. The LCSWs are expected to perform the following:
Coordinate appropriate referrals to community and other Government agencies for services.
Independently assess the psychosocial functioning and needs of patients and their family members and formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team.
Independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diverse backgrounds.
Use medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.
Implement treatment modalities in working with individuals, families and groups who are experiencing a variety of psychiatric, medical and social problems to achieve treatment goals.
Independently provide counseling and/or psychotherapy services to individuals, groups, couples, and families. Must practice within the bounds of the license or certification and the privileges granted by the MTF.
Provide consultation services to other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.
Participate in professional peer review case conferences, research studies, or other organizations required at the MTF.
Other duties as assigned.
Requirements:
Degree/Education: Master's degree in social work (MSW). Graduate from an Accredited School of Social Work fully accredited by the Council on Social Work Education (CSWE).
Experience: Experience in clinical social work, either through a master's-level practicum or 2 years post-MSW experience, within the last 3 years, in a clinical/treatment setting.
Current, full, active, and unrestricted license to practice as a Licensed Clinical SocialWorker.
