DESCRIPTION
Provides culturally competent and appropriate behavioral health treatment to people served by Community Counseling Solutions (CCS). The position provides the treatment in accordance with the Oregon Administrative Rules (OAR's) and the mission and values of CCS. The position works closely with other involved community partners and is professional in all aspects of service delivery. Additionally, this position assists administration in meeting the behavioral health needs of the community.
SUPERVISION
Supervision Received
Depending upon location, this position is supervised by the Clinical Supervisor, Clinical Director or Site Manager.
Supervision Exercised
This position does not supervise other employees.
RESPONSIBILITIES
Provide clinical services to children, adults and families. This would include but is not limited to screening, assessment, individual and group therapy, consultation, and all other allowed clinical services.
Actively participate in joint case planning and coordination with agency employees and relevant agency contractors. Act as a liaison between clients and other agencies. Act as a client advocate when appropriate.
Actively participate in group and individual supervision sessions.
Work with other staff and community representatives to initiate new programs, reviews, evaluates and revises existing programs as necessary.
Perform joint case planning and coordination in areas of behavioral health with other community agencies including but not limited to: schools, group homes, police organizations, district attorney's offices, courts, community clinics and hospitals and other agencies in the Department of Human Services.
Provide behavioral health and addiction education, prevention and information to various interest groups.
Assist other clinicians in the facilitation of groups as requested.
Provide after hours emergency services on a 24 hour, rotating basis to adults and children. This includes telephone consultation and crisis intervention in the community.
Initiate holds, provide civil commitment investigations, and work with the hospitals, case workers and the state and other individuals/agencies for all individuals in the civil commitment process.
Complete all paperwork in a way that is professional, clinically sound and timely as prescribed by the OAR's and other pertinent guidelines. Required to ensure that all paperwork is present, and/or the client record is well maintained.
Attend trainings, meetings, conferences, etc. as directed by administration and use the information gathered to benefit the agency and the customers.
Provide intensive children's treatment services.
Utilize agency software/computer systems to prepare current client records of treatment and reports as necessary and required.
Transport clients as required
Administer forms for urinalysis testing (UA's), provide accurate information on the UA's to clients, in some cases observes the UA, and record all pertinent information.
Must report all abuse to appropriate legal office, complete all necessary paperwork and investigations, and develop safety plans as required.

QUALIFICATIONS
Education and/or Experience
MHS IV - Either a doctorate degree from an accredited college in social work, psychology or other human service related field is required, or a master's degree with accompanying certification as an Licensed Professional Counselor (LPC), a Licensed Clinical Social Worker (LCSW), or a Licensed Marriage and Family Therapist (LMFT).
MHS III - Requires a Master's degree from an accredited college in social work, psychology or other human service related field.

Certifications

PHYSICAL DEMANDS
While performing the essential duties of this job, the employee is regularly required to use office automation including computer and phone systems that require find manipulation, grasping, typing and reaching.

WORK ENVIRONMENT

Some individuals in this position may be required to drive long distances routinely as a part of their regular job duties. Occasional out of area travel and overnight stays will be required for attendance at meetings and/trainings.
However, the employee also will be required to work in the communities, homes and other living environments, of the clients we serve. Handicap access may not be available at all places where this position must go. There are some situations where this position may be required to respond to environments where a client is in crisis. The environments in these situations are difficult to predict and may be in uneven terrain.

Job Summary: The Social Worker - Outpatient MSW PRN assists patients and their family members by obtaining services or initiating referrals in an effort to eliminate personal or environmental barriers which may interfere with maximum benefits of health care. This job supports the provider and interdisciplinary team in facilitating and coordinating patient care through the continuum.
Essential Job Functions & Accountabilities:
* Performs psychosocial assessments to identify individualized needs and develops a plan to address each need that takes into consideration the requests of the patient/family. Makes referrals to the appropriate resources within the JPS Health Network and within the community.
* Collaborates one-on-one, in huddles or team meetings with Providers, Nurse Case Managers and Patient Centered Medical Home (PCMH) staff focusing on identifying the needs of complex high-risk patients. (Not applicable to Post-Acute program).
* Provides brief supportive counseling to patients and families exhibiting complex family dynamics directly impacting patient care.
* Assists patients and families to cope with and manage life-changing diseases, death and dying, advanced directive education, disabilities, etc.
* Supports patients and providers with reviewing and completing various eligibility forms for social programs, funding sources, durable medical equipment, etc.
* Follows-ups with patients and families ensuring that services provided are helpful, appropriate, and adequate. Develops and sustains long-term working relationships with patients and families.
* Aids with care coordination for patients with complex psychosocial needs including providing services in a variety of settings (i.e. PCMH, patient homes, hospitals and other health care locations). Local travel may be required to perform duties at multiple sites. (Travel is required while working under the Post Acute program).
* Educates patients and families regarding various care options (i.e. home care, nursing home, hospice, etc.) and ensures that the patient's choices are incorporated into the care coordination plan.
* Documents each component of the case management process and related activities in the electronic medical record system according to departmental guidelines.
* Assists with cost containment, short-term intervention, ambulatory and community-based care, and decentralization of services.
* Works as a member of the interdisciplinary team as the patient advocate.
* Complies with National Association of Social Workers (NASW) Code of Ethics and maintains legal and regulatory accountability.
* Local travel may be required.
* While working under the Post-Acute Program also performs the following:
* Attends multidisciplinary meetings held within SNF/IRF to determine care plan. Meetings will be held both in person and virtually as determined by the needs of the facility and patients in the program.
* Serves as a liaison between the Social Services Teams within the post-acute agencies to ensure smooth transitions for patients in the post-acute program.
* Provide coverage support to the JPS Care Management Outpatient team as needed.

Qualifications:
Required Qualifications:
* Master's Degree in Social Work from a program accredited by the Council on Social Work Education.
* 2 plus years social work experience (including internship work) with one year in a health care setting.
* Social Work licensure through the Texas State Board of Social Work Examiners.
* BCLS may be obtained subsequent to hire.
Preferred Qualifications:
* Case Management Certification (ACM or CCM).
* For Post-Acute Program Position: Experience working in the post-acute setting (hospice, home health, nursing home or inpatient rehabilitation hospital).
