Required to ensure that all paperwork is present, and/or the client record is well maintained.     Department: Case Management Shift: Weekend Shift (United States of America) Standard Weekly Hours: 24 The LMSW Social Worker provides direct social work services to patients and their families who utilize the Emergency Department of Cook Children's Health Care System. This position requires an assertive person who is able to communicate effectively, problem solve and think critically. This position requires dialogue with individuals regarding difficult topics. This Social Worker will be available to other floors during the scheduled shift to assist with any social work needs. Education: MSW/MSSW from an accredited school of Social Work. Previous experience in a hospital setting and some prior experience working with children and families is preferred. Certification/Licensure: Current Texas License for LMSW. Basis Life Support (BLS) Certification for Health Care Providers required within 30 days of employment and must be maintained per Medical Center Policy, MC 175. Essential Job Functions & Accountabilities: * Performs psychosocial assessments to identify individualized needs and develops a plan to address each need that takes into consideration the requests of the patient/family. Develops and sustains long-term working relationships with patients and families. * Aids with care coordination for patients with complex psychosocial needs including providing services in a variety of settings (i.e. PCMH, patient homes, hospitals and other health care locations). Local travel may be required to perform duties at multiple sites.
