If hospice care is appropriate actively provides emotional support, grief work, education and funeral/financial planning referral. Facilitates hospice or nursing home placement as needed. Initiate referrals to external resources with community agencies such as Adult Protective Services, Housing Authority, or public utility companies. Advocates with these entities for purposes of maintaining community stability.

Develop a family-centered plan and a termination report

Job Qualifications:
Prior experience working with adoptive parents and the adoption process preferred.
Experience providing in-home services and/or working directly with children and their families in various settings, including child welfare, schools, juvenile justice, and mental health preferred.
Understanding of basic needs, social, cultural and religious differences in families' required.
Provide services in Genesee, Huron, Lapeer, Sanilac, St. Clair, Tuscola county

Qualifications
Master's Degree from a school of social work accredited by the Council on Social Work education.
Licensed as a social worker in the state in which practicing
Minimum of 1 year experience in social work, preferably in a home care agency, hospital, outpatient clinic, rehabilitation center or mental health program.
Demonstrates knowledge and skills necessary to provide care primarily to the adult and geriatric populations.
Demonstrates knowledge of the principles of growth and development over the life span.
Able to assess data reflecting the client's status and interpret the appropriate information needed to identify each client's requirements relative to their age-specific needs
Demonstrates computer skills sufficient to use the electronic medical record effectively.
