As a Hospice Clinical Social Worker, you will:
Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and dignity of each patient.
Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
Assess the social, spiritual, and emotional needs/factors in order to estimate the patient's and involved caregiver's capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.
Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning
Utilize all available resources, such as chosen family, hospice, and community agencies, to assist the patient and chosen family to live better within the limitations of the illness
Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care
Required Certifications and Licensures:
Licensed to practice as a clinical social worker in the state of agency operation if required

The Youth Crisis Worker is a critical member of the Youth Services team, providing after-hours crisis intervention services to runaway and homeless youth and their families on an on-call basis. As a Youth Crisis Worker, you will rely on your ability to quickly build rapport, listen nonjudgmentally, facilitate effective communication through conflict, and partner with youth and families to help them negotiate solutions that work for their individual circumstances. Youth Crisis Workers sign up for at least eight flexible shifts per month based on their personal availability; shifts may include weekends, evening/overnight, or holidays. During on-call shifts, Youth Crisis Workers respond in-person to police departments to provide after-hours crisis intervention services to youth who have run away, been locked out, or are without a safe place to stay. During crisis intervention, Youth Crisis Workers meet with youth and parents to understand both perspectives, facilitate effective communication, and help the family negotiate a safe and appropriate solution, complete initial assessments of safety and protective factors to help inform the crisis intervention process, and may transport youth to temporary placements, using a personal vehicle (mileage expenses reimbursed by agency).
Candidate qualifications:
At least two years of college coursework in Social Work or a related field, OR two years of experience working with youth and families, OR an equivalent combination of education and experience. (All candidates must have at least high school diploma/GED.)
Availablility to respond in-person to after-hours crisis calls (within 60-90 minutes depending on location).

The Youth Crisis Worker is a critical member of the Youth Services team, providing crisis intervention services to runaway and homeless youth and their families on an on-call basis. As a Youth Crisis Worker, you will rely on your ability to quickly build rapport, listen nonjudgmentally, facilitate effective communication through conflict, and partner with youth and families to help them negotiate solutions that work for their individual circumstances. Youth Crisis Workers sign up for at least eight flexible shifts per month based on their personal availability; shifts may include holidays. During on-call shifts, Youth Crisis Workers respond in-person to police departments to provide crisis intervention services to youth who have run away, been locked out, or are without a safe place to stay. During crisis intervention, Youth Crisis Workers meet with youth and parents to understand both perspectives, facilitate effective communication, and help the family negotiate a safe and appropriate solution, complete initial assessments of safety and protective factors to help inform the crisis intervention process, and may transport youth to temporary placements, using a personal vehicle (mileage expenses reimbursed by agency).
Core Responsibilities and Essential Job Functions:
Responds to all police calls and appropriate referrals within the program timeframes.
Provides information and referrals to non-mandated youth and families.
Acts as a mediator/negotiator for families requiring crisis intervention.
Assesses for safety risks, creates safety plans and refers to mental health and/or child welfare professionals when necessary.
Develops a temporary alternate placement arrangement for youth who cannot return home, within program policy and guidelines and when authorized by the Coordinator or Supervisor.
Delivers supportive services to clients including transportation, as required by the program.
Writes case notes and completes intake paperwork, safety screens, crisis stabilization plans and other documentation.
Participates in an on-call schedule, including evenings, weekends, and holidays.
Works under the direct guidance and supervision of an assigned staff member.
Completes all documentation requirements
Candidate qualifications:
At least two years of college coursework in Social Work or a related field, OR two years of experience working with youth and families in a trauma-informed setting, OR an equivalent combination of education and experience. (All candidates must have at least high school diploma/GED.)
Availability to respond in-person to crisis calls (within 60-90 minutes depending on location).
Valid driver's license, insurance, and a reliable vehicle required.
Excellent skills at communicating effectively and empathically, even in a high-conflict situation.

Qualifications
Education:
Master's degree in Psychology, Social Work, Counseling, or a related subject area
Bachelor's degree with substantial social service experience working with families and youth
Job Responsibilities
Conduct MST assessment including review of referral information, identifying and engaging key participants, identifying systemic strengths and weaknesses, and developing interventions to address the referral behaviors
Engage primary caregiver and other key participants in active change-oriented solutions
Conduct sessions/meetings in the youth's natural environment (homes, schools, attends court dates, community). Must have a car to get to all these meetings.
Maintain clear and concise documentation of treatment efforts that promote peer and supervisory review and feedback, and that demonstrate compliance with the 9 MST Principles and the MST Analytic Process.
Ability to engage and collaborate with all relevant systems and key participants within each system, to maintain alignment throughout MST treatment.
Participate in all MST training, supervision and consultation activities.
Maintain caseloads of 5 families at a time.
Staff are available to families via phone 24 hours a day, 7 days a week, with exceptions to vacations and personal time off
Must be able to work independently and be flexible

This staff member will receive MST certification through a five-day training on the MST model and will receive additional training as required
