Must possess as a minimum , a Bachelor's Degree in social work, counseling, psychology, or any other specialized intellectual instruction directly related to social work.
Preferred minimum, 2 years supervised social work experience in a health care setting working directly with individuals.
Prior work with elders in the community or long -term care setting is preferred.

Working Conditions/Environment
While performing the duties of this job, the employee is regularly exposed to outside weather conditions; occasionally exposed to fumes or airborne particles, extreme cold and extreme heat; unsanitary conditions, e.g., spoiled foods, feces, urine, and disease. Collaborate effectively with coworkers and partner organizations. Build and maintain relationships with area service providers for the benefit of clients.
Use personal, insured vehicle and/or public transportation (where available) to travel and transport clients and materials as needed throughout the service delivery area.
Collaborate with colleagues across grants and programs, ensuring that clients access services for which they are eligible. Complete program quality assurance activities to ensure appropriate use of funds.

The Medical SocialWorker collaborates with the multidisciplinary team to provide input in the development of the plan of care for those patients requiring social work intervention. The Medical SocialWorker provides medically necessary intervention to patients who have social or emotional problems that are, or are expected to be, impediment(s) to the effective treatment of the medical condition and/or rate of recovery. Additionally, the Medical SocialWorker provides these services in accordance with physician's orders.
Essential Duties and Responsibilities:
Provides medically necessary social work intervention in accordance with physician's orders
Assesses patient's psychosocial needs; plans for, evaluates, and modifies interventions to meet the established patient goals
Collaborates with other disciplines to plan and evaluate team goals for each patient
Facilitates patient referrals to community agencies when appropriate
Provides financial counseling when necessary to achieve the treatment goals established by the multidisciplinary team
Assists patient/family/caregivers in planning for present and future psychosocial needs related to the patient's current status and prognosis, including terminal illness
Adheres to company policies and procedures while providing services and preparing/submitting required paperwork
Observes and reports patient progress and/or changes in patient status to physician and appropriate multidisciplinary team members
Functions as a resource person for other members of the health care team, providing inservice education and consultation in understanding the social and emotional factors related to health problems.
Maintains knowledge of the field including Medicare, Medicaid, and other third-party reimbursement sources.
Participates in planning and implementing quality improvement activities as necessary
Provides positive experiences in assisting patients, families, physicians, other departmental employees, referral resources, service agencies, and others.
Adheres to facility and Amergis policies at all times
Minimum Requirements:
Baccalaureate degree in Social Work (BSW) or Master's degree from a school of social work accredited by the Council on Social Work Education per state/client requirements required
Minimum of one (1) year of social work experience in a health care environment in the last two years preferred
Complies with all relevant professional standards of practice
Participation and completion of Amergis' Competency program when applicable
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or state regulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable to job site
Understand patient confidentiality and HIPAA requirements

Exciting opportunity to join multi-disciplinary team at the state-of-the-art Wellness Circle Crisis Stabilization Unit, a 16-bed, short-term residential crisis stabilization unit that serves individuals with mental illness and co-occurring disorders or developmental disabilities who are in psychiatric crisis. Services include crisis intervention, individual, group and family counseling, case management, medication management, social and medical detoxification and linking individuals to ongoing mental health, substance use or co-occurring services. Monitors symptoms and behavior of clients and provides crisis intervention/crisis stabilization services under clinical supervision. This position is primarily responsible for 10-hour, awake overnight shift coverage, including weekends and holidays. As needed, other shifts may be required in order to provide 24-hour shift coverage.

CERTIFICATES AND LICENSES REQUIRED:
First Aid and CPR certification, within 90 days of appointment and maintained throughout CSB employment;
MANDT Certifications within six (6) months of appointment and annually thereafter;
Valid Motor Vehicle Driver's license with fewer than six demerit points (or the equivalent in another State) maintained throughout employment with CSB;
Qualified Mental Health Professional (QMHP) within 6 months of appointment.
NECESSARY SPECIAL REQUIREMENTS:
The appointee to this position will be required to complete a criminal background check, a Child Protective Services Registry check, a driving record check, and sanction screening to the satisfaction of the employer. A TB screening upon hire and annually thereafter.
This position is emergency/essential services personnel. (Required to report for duty during inclement weather and/or other emergencies.)
PREFERRED QUALIFICATIONS:
Bachelor's degree in counseling, psychology, social work, or nursing.
2+ years of post-degree experience working with adults who suffer from serious mental illness and co-occurring disorders in an intensive treatment group home, hospital emergency department or psychiatric unit, community-based behavioral emergency services, and/or crisis stabilization setting.

OUR BEHAVIORAL HEALTH SOCIALWORKER POSITION
The primary focus of the SocialWorker role.

The identified target population includes residents residing in Hebrew Home (Smith Kogod and Wasserman buildings). Ensure residents referred as the result of behavioral health concerns receive appropriate treatment and services to correct the assessed problem or attain the highest practicable mental and psychosocial well-being.
Ensure residents referred due to behavioral health concerns who display or have been diagnosed with dementia, receive the appropriate behavioral health services to attain or maintain his or her highest practicable physical, mental, and psychosocial well-being.
Support Hebrew Home in its provision of medically-related social services to attain or maintain the highest practicable physical, mental and psychosocial well-being of each resident.
Create an environment of non-pharmacological behavioral strategies to address behavioral health issues related to residents/patients who have been referred.
Collaborate with the Care Team in identifying and implementing behavioral modification/intervention strategies with a focus on reducing use of psychotropic medications.
SHIFT & STATUS
This is a FULL TIME EXEMPT position with a Monday thru Friday schedule.

IRC's Anti-Trafficking Program is designed to find sustainable solutions that are appropriate for the unique situation of each client. Caseworkers serve clients with holistic and broad supports, including but not limited to connecting clients with legal service providers and housing resources, helping clients access public benefits, health providers, and psychosocial services, and offering regular checks-ins to discuss safety and service plans. Major Responsibilities:
Case Management and Referrals
• Determine client eligibility and provide orientation to services, outcomes, and confidentiality with each potential client.
• Complete individualized intake assessment and safety planning. Develop individualized SMART self-sufficiency plans for each client enrolled in the program.
• Provide trauma-informed comprehensive case management support, referral, and advocacy services for survivors of trafficking to help them feel safe, empowered, and stable.
• Meet clients' needs in a timely and culturally appropriate manner.
• Assist clients with referrals and access to appropriate resources including shelter, physical and mental health services, and legal services to work toward a stable immigration status.
Community Outreach and Partnerships
• Contribute to the development and implementation of a region-wide outreach plan to increase awareness regarding the needs of Victims of Trafficking services.
• Coordinate with program leadership regarding outreach and education plans.
• Distribute informational material and posters to hotels, businesses, bus stations, law enforcement, hospitals, school administrators, and other public places. Host public information meetings and trainings alongside the program coordinator.
• Build partnerships with community groups and providers; work to connect clients to available resources, including various local, state, and federal law enforcement agencies. With their assistance, develop safety plans/protocols for clients, staff and the office.
• Participate in all IRC program meetings, staff development activities, and fully engage as a member of the team.
Documentation and Reporting
• Ensure timely, quality case notes and documentation for contacts made and services provided on behalf of client and for case file management and quality assurance.
• Prepare, distribute and document direct assistance requests for clients. Maintain record of case management expenses in case file.
• Draft reports as required by funders and/or donors.
• Comply with all policies, procedures and protocols of the agency.
• Other related duties as assigned.
Job Requirements:
• Minimum of 1-2 years of relevant work experience in social service, case management, human service or related fields required.
• Undergraduate degree preferred, ideally with an emphasis in Social Work, Psychology, or a related field of study.
• Self-starter with excellent problem-solving skills combined with the proven ability to multi-task, prioritize duties, and manage time effectively.
• Compassion toward and understanding of unique needs of survivors of trafficking.
• Knowledge and understanding of the Trafficking Victims Protection Act (TVPA) of October 2000, accompanying regulations, reauthorizations and related policies concerning trafficking strongly preferred.

• Working knowledge of legal providers, social services, public benefits, community organizations, health care systems, and related human services in the Silver Spring area preferred.

• Fluency in English required. Fluency in Spanish is strongly preferred.
• Valid driver's license and access to a personal, insured vehicle and willingness to transport clients required.
Working Environment:
• A combination of standard office environment, remote work, and ‘field' time within the service delivery area to perform the above outlined responsibilities. Typically, this position is based at the office 4 days/week and remotely 1 day/week, with potential field work all days.
