b. Provides appropriate interventions to optimize health and well-being. Interventions may include education, the coordination of community-based support services, and other resources.

Requirements
Must have a Master's degree in Social Work
Must have successfully completed CMS's Long-Term Care Basic Training and passed the Surveyor Minimum Qualifications Test (SMQT).

Must maintain current licensure to practice as an LCSW

GENERAL PURPOSE: The Licensed Clinical SocialWorker (LCSW) provides comprehensive psychosocial assessments, counseling, and case management services to patients enrolled in the Partial Hospitalization Program (PHP). Working under the supervision of the PHP Program Manager, the LCSW plays a key role in developing individualized treatment plans, facilitating group and individual therapy sessions, and coordinating community resources to support patient recovery and well-being.
ESSENTIAL FUNCTIONS:
Conduct thorough psychosocial assessments and develop individualized treatment plans tailored to patient needs
Provide individual and group therapy to patients within the PHP framework
Collaborate with multidisciplinary team members including psychiatrists, nurses, and therapists to ensure holistic care
Assist patients and families with crisis intervention and provide education on coping skills and resources
Facilitate discharge planning and connect patients to community support services and outpatient resources
Maintain accurate and timely documentation of patient progress, treatment plans, and clinical notes in compliance with regulatory standards
Participate actively in team meetings, quality improvement initiatives, and professional development opportunities
Ensure compliance with all ethical, legal, and regulatory requirements governing clinical social work practice
REQUIREMENTS:
Master's degree in Social Work from an accredited program
Current licensure as a Licensed Clinical SocialWorker (LCSW) in the state of Nevada
Minimum of two (2) years clinical experience in mental health or behavioral health settings, preferably in Partial Hospitalization or intensive outpatient programs
Strong clinical assessment and therapeutic skills
Excellent communication, organizational, and interpersonal skills
Ability to work collaboratively within a multidisciplinary team
Commitment to patient-centered care and cultural competency
