    PURPOSE STATEMENT: Plan, direct, define and coordinate individualized and group therapeutic support and services to patients with behavioral problems, psychiatric disorders, post-traumatic stress disorder, substance use and physical and emotional abuse. This position is 8:30a-5p wuth weekend and holiday rotations. Conduct psychosocial assessment of patient and develop an effective treatment plan that applies therapeutic principles appropriately while ensuring patient safety at all times. Through patient observation and patient/family interview obtaining information on the patient's needs, skills, interests and limitations for the development and implementation of the treatment plan. Provide crisis intervention and assist families in understanding the implications and complexities of a life situation and its impact on lifestyle. Participate in multidisciplinary treatment team meeting to discuss psychosocial assessment, treatment planning and ongoing discharge planning. Identify, assess and coordinate patient cases and anticipate their needs related to illness, hospitalization, finances, education and home/community services. Facilitate education, process groups, support groups and referrals as requested. Conduct intakes and assist in coordinating placements within facility programs as required. Develop and execute clinically relevant activities in coordination with activity professionals, as required. Promote recovery, symptom reduction and the achievement of the highest level of social functioning. Communicate or aid family members in understanding and supporting the patient. Manage a caseload and assume responsibility for patient chart documentation. Facilitate education, process groups, support groups and referrals as requested. Manage a caseload and maintain accurate and timely documentation to assure patient records comply with facility and accreditation standards. Supervise social work interns completing field placement, as required. Provide direction and guidance to social services staff or other staff members, as required. Lead the program/unit and coordinate the activities of other clinical or direct patient care staff, providing direction and clinical guidance, as required. Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority. Perform other functions and tasks as assigned. EDUCATION/EXPERIENCE/SKILL REQUIREMENTS: Master's degree in Social Work or related Human Services Degree (Counseling, Psychology, etc) from an accredited college or university required. Three or more years' related experience working with comparative patient population required. Supervisory experience preferred if position is responsible for supervising staff. LICENSES/DESIGNATIONS/CERTIFICATIONS: State licensure, as required by the state in which the facility operates; which could include: Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), Licensed Social Worker (LSW), LMHC, LMHCA, LMFT. CPR and de-escalation/restraint certification required (training available upon hire and offered by facility). First aid may be required based on state or facility. Job Specifics * Conducts psycho-educational groups for patients. * Develops and implements a plan to assist the patient and family in resolving identified problems. * Provides counseling and support to patients and their families in order to assist in the solution of psychosocial, behavioral health related problems such as suicidal ideation, depression, psychosis, and mania. * Assists in the utilization management of patients in the Behavioral Health Services Unit. * Communicates effectively and collaboratively with all members of the healthcare team: * Reviews patient/family care needs with nursing personnel and physicians. Provides advice concerning patient's social and emotional needs and solicits input regarding patient assessment. * Establishes and maintains a cooperative working relationship with representatives of various community social agencies, extended care facilities, and other resources essential to effective post-discharge care. * Provides program/information on social work services to outside organizations when requested. * Records information concerning social assessment, patient counseling, discharge planning, and other appropriate information on the patient's chart. Performs a formal psychosocial history when deemed necessary or when requested. * Maintains appropriated records for the reporting of individual and departmental statistics on a regular basis. * Demonstrates commitment/support of departmental goals, policies and procedures to ensure efficient and effective operations. * Performs related professional services such as program development and visitation of community patient care facilities. * Remains abreast of advances in the field of social work by reviewing pertinent literature, in-service training, seminars, and discussion with peers.
