POSITION OVERVIEW:
The Social Worker is responsible for supplying necessary clinical intervention within the realm of the program. Provide cultural competent and sensitive services. As part of a therapeutic team within the Agency, work closely with the divisions' medical, psychiatric and psychology staff, to ensure the appropriate treatment interventions for the client.
KEY ESSENTIAL FUNCTIONS:
Client Engagement
• Carries a caseload of residents, and provides services to those residents within the scope of program regulations and agency mission.
• Completes client assessments to include, intake assessments, overall client assessments, risk assessments
• Evaluates and assesses clients' functioning, and supports clients in meeting individual goals.
• Practices person-centered and collaborative service planning with clients, also ensuring service plans are completed and signed by residents according to regulatory guidelines and standards.
• Interprets and explains agency policies and procedures to residents.
• Develop safety plans
• Provide individual and family counseling.
• Makes referrals and coordinates services for clients, including but not limited to, medical, mental health, substance abuse prevention and treatment
• Advocates on behalf of residents with other services providers.
• Links residents to community-based programs and collaborates with other services providers
• May run groups under supervision
• Participates in audit preparation.
• Other duties as assigned.

Maintain caseload of estimated 35-45 participants
Record progress notes in comprehensive, timely manner and complete all reporting as required by the Monroe County Office of Mental Health and Mental Health Treatment Court
Participate in case review meetings with direct Court staff, as scheduled
Receive assigned cases, review and/or formulate recommendations, submit referrals based on client needs

RESPONSIBILITIES

In collaboration with the members of the health care teams and community agencies, coordinates discharge planning for patients who are determined to be at psychosocial risk, assistance navigating the health care system and psychosocial support for patients in the Emergency Department.
Handles risk management issues, in close consultation with their supervisor and Associate Director.
Documents social work activity in the electronic medical record
Provides social work coverage, as requested during absence of co-workers, including shift modification in order to have adequate shift coverage, including days, evenings and overnights.
Completes the following Administrative Functions:
Completes Hospital and SW Division required in-service training programs and ongoing professional development activities including those that are designed to enhance knowledge and skills in protective work
Completes required Employee Health appointments within required timeframe

QUALIFICATIONS
The social worker must have an MSW from an accredited social work graduate program and must obtain New York State license within eighteen months of employment required.

The Lafayette Program Supervisor shall be responsible for the daily case management of individuals or families who are clients in this 27-unit Permanent Supportive Housing Program. The Program Supervisor will collaborate, couch and advocate for formally homeless and disabled clients with the goal of long-term housing stability.
Essential Duties and Responsibilities:
Support intake and assessments for Clients
Completes assessment of individuals and family needs and care to include but not limited to financial, medical, food, domestic violence, abuse, etc.
Determine clients' rent contributions and ensure their timely payment to assist in their skill building for eventual independence. Work with clients to resolve owed rent or charges to the unit.
Develop, review, and revise comprehensive care plans that is inclusive of all family members. Ensuring client(s) plans are holistic and include asset building through life skills.
Assists Clients in securing necessary resources.
Makes appropriate referrals, advocacy, benefit and entitlement counseling.
Conduct at a minimum monthly home visits.
Coordinate Life Skills classes at a minimum of twice a month.

Clinical Primary Therapist
The Clinical Primary Therapist works with clients, either individually or in groups, to address mental health concerns. The Clinical Primary Therapist applies knowledge and skill to assist clients in recovery and illness management. The Primary Therapist is responsible to establish the boundaries of therapy including a comfortable safe environment, caring therapeutic culture and foundation for client / family engagement in recovery and behavioral management. The Primary Therapist completes a comprehensive assessment gathering data and information resulting in clinical formulation as the framework for development of the treatment plan. In collaboration with the client / family the Therapist establishes goals for treatment and identifies the interventions to promote goal achievement. The Clinical Primary Therapist is responsible for timely accurate documentation in the client's medical record, coordination of care with colleagues in the clinic and colleagues throughout the continuum to secure the appropriate treatment resources to promote client health. The Clinical Primary Therapist participates in the on-call rotation to the Emergency Departments of Finger Lakes Health to provide mental health crisis evaluations.

Responsibilities:
The social worker assigned to URMC Mobile Crisis Team functions as a crisis therapist and provides mental health assessment, and crisis intervention and treatment to complex mentally ill patients who are at risk in a variety of outreach settings, including group homes, shelters, patients' homes, and other sites across Monroe County. The Social worker on the Mobile Crisis Team works closely with a Crisis Specialist. The social worker is also an integral member of the Mobile Crisis Team, providing psychosocial assessment, crisis intervention, resource consultation, and brief treatment, minimizing psychosocial risk and decreasing Emergency Department utilization.
RESPONSIBILITIES
Provide psychosocial assessment to patients in outreach settings (i.e., group homes, shelters, patients' homes) who are at exceptional psychosocial risk as needed. Provides psychiatric crisis intervention and treatment in the community to help stabilize patients. Provides clinical leadership to Mobile Crisis Team in the field. Exercises considerable independent judgment with respect to assessment and treatment of psychiatric patients in outreach settings.
Responsible for ensuring assessment of safety of scene prior to deployment of Mobile Crisis Team and accessing need for law enforcement services to ensure safety of Mobile Crisis Team in the field. Involves family and other mental health treatment providers as necessary in interventions.
Provides clinical leadership to Mobile Crisis Team in the field. Exercises considerable independent judgment with respect to assessment and treatment of psychiatric patients in outreach settings.
Documents treatment in patient's electronic medical record in compliance with Division, Department, and Hospital Policy.
Attends Social Work Division, Social Work in Psychiatry and Program staff meetings. Participates in activities of the Social Work and Psychiatry Division Psychiatry Division including, but not limited to, ensuring policies and procedures are implemented with regard to professional practice, discharge planning and patient care, timely completion of time reporting and other Hospital or Division forms; Hospital and Division mandatory in-service training programs and UHS reviews. Participates in routine committee process of the Social Work Division, and facilitates Division and Hospital quality assurance and staff development activities. Subscribes to the departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences. Participates in available cultural competency events and training appropriate to job duties
Ensures that proper treatment and referral protocols are followed with respect to child abuse, domestic violence, consultation with specialty clinics, and referral both within and outside of SMH.
Participates in on-call activities (rotational basis) of the Mobile Crisis Team, including after hours. Other duties as assigned.
Teaching: Based on experience and as determined by the social work supervisor, the social worker may supervise social work or other master's level graduate students. Upon invitation from the Program Director and in consultation with the social work supervisor, he/she may offer learning experiences for psychiatric residence, medical students, and family medicine residence. Supervision relates to psychosocial assessment and intervention with patients and their families.
Research: Based on experience/interest, may participate in interdisciplinary research as determined by the Social Work Supervisor, in consultation with the Social Work Clinical Manager in Psychiatry.
QUALIFICATIONS
Required:
Master's degree from an accredited school of social work (CSWE)
Master's degree from an accredited school of social work. Licensure as a social worker, as defined by the State Board for Social Work, State Education Department, Division of Professional Licensing Services is required. If licensure has not been obtained upon employment, the individual has 18 months within which to obtain it, unless duties will be performed under OMH then minimum of a limited permit (not to exceed 1 year) or NYS Licensure will be required upon employment start.
Must possess a valid New York State Driver's license and a satisfactory driving record.
Preferred:
2-3 years professional social work practice beyond the Master's degree, at least one of which must include psychiatric emergency, crisis or outreach work; or an equivalent combination of education and experience. and working with adult and/or pediatric and adolescent populations.
Knowledge about and ability to provide behavioral health assessments/evaluations and evidenced based interventions;
Commitment to and routine demonstration of biopsychosocial approach to care of individuals and families;
