QUALIFICATIONS:
Bachelor's degree in social work or MSW preferred. Completion of a undergraduate or graduate degree in a related. Agreeable to pursue further education in the field of social work or gerontology if necessary.
A minimum of three years experience working in direct service with the geriatric population preferred.
Basic computer skills are necessary including but not limited to the ability to work on a laptop or personal computer, the ability to create, open and save electronic documents and emails, and the ability to navigate between multiple computer programs within a Windows OS required.

In this role, you will
Have a routine presence as liaison in Monroe County Mental Health Treatment Court
Maintain caseload of estimated 35-45 participants
Record progress notes in comprehensive, timely manner and complete all reporting as required by the Monroe County Office of Mental Health and Mental Health Treatment Court
Participate in case review meetings with direct Court staff, as scheduled

RESPONSIBILITIES

In collaboration with the members of the health care teams and community agencies, coordinates discharge planning for patients who are determined to be at psychosocial risk, assistance navigating the health care system and psychosocial support for patients in the Emergency Department.

QUALIFICATIONS

KEY RESPONSIBILITIES:
Resident Assessment and Care Plan Development:
Conduct comprehensive evaluations in compliance with ALR/SNALR/EALR NYS DOH regulations of residents' cognitive, physical, emotional, and social needs upon admission and throughout their stay.
Collaborate with the Administrator, licensed nurses, medical professionals, care partners, and families to create personalized wellness plans that address the specific challenges of memory impairment, identified needs and goals.
Regularly review and update wellness plans in coordination with the RN Case Manager and the Administrator.
Family Support and Education:
Offer counseling and education to families, helping them understand the progression of memory-related diseases and strategies for caregiving.
Work closely with family members to ensure they understand the resident's wellness plan and assist them in coping with the challenges of caregiving and progression of memory loss. Provide education on aging-related issues, caregiving skills. Provide information about community resources, and long-term care options.
Facilitate family meetings to discuss wellness plan updates.
Behavioral and Emotional Support:
Provide emotional support and counseling for residents experiencing anxiety, depression, or confusion due to memory loss.
Develop strategies to manage difficult behaviors associated with dementia, including agitation, aggression, or withdrawal.
Offer one-on-one support and activities that stimulate cognitive function and promote positive emotional well-being.
Advocacy:
Advocate for residents' rights, ensuring they receive appropriate care and services.
Address any concerns from residents, families, or staff, ensuring that the residents' wishes and needs are respected in accordance with their wellness plan.
Act as a liaison between residents and staff to address concerns and resolve issues.
Resource Coordination:
Identify and connect residents and families with community resources, such as legal assistance, financial support, and healthcare services.
Assist with the transition of residents into the facility and provide guidance on community integration.
Crisis Intervention:
Respond to and manage crises involving behavioral issues or emotional distress for residents.
Provide crisis intervention and emotional support in situations of trauma, such as a sudden health decline or the death of a resident.
Provide support during significant changes, such as hospitalization or end-of-life decisions.
Coordination of Care:
Collaborate with nurses, physicians, and other healthcare professionals to provide holistic care that addresses medical, social, and emotional needs.
Assist with transitions to and from the memory care facility, ensuring residents and families receive guidance during these challenging times.
Keep detailed records of wellness plans, progress, and any changes in residents' conditions
Therapeutic Interventions and Socialization:
Assist with group activities designed to encourage socialization and cognitive stimulation (e.g., memory exercises, music therapy, arts and crafts).
Provide individualized support to residents who may not be able to participate in group activities, ensuring they still feel connected and engaged.
Training and Staff Support:
Provide staff training on dementia care techniques, communication strategies, and how to manage residents' challenging behaviors.
Offer guidance on how to maintain a supportive and therapeutic environment for residents with memory loss.
QUALIFICATIONS:
Education: Bachelor's or master's degree in Social Work (BSW or MSW) from an accredited program OR 10 + years of relevant leadership experience in assisted living, memory care.
Experience: Prior experience in healthcare, assisted living, memory care, community health, or social services setting is required.
Skills: Strong communication, problem-solving, and interpersonal skills. Ability to work in a team-oriented environment.
Other Requirements: Compassionate, patient, and empathetic toward the elderly. Knowledge of aging-related issues and elder care resources.
WORKING CONDITIONS:
Primarily office-based, with direct interaction with residents in the ALR/SNALR/EALR.

The Family Preservation Program (FPP) serves families that have a child/children at risk of foster care placement or who are in need of intensive therapeutic services. The Family Preservation Program partners with Ontario County families in their homes for 4-6 weeks and provides therapy 3-5 times a week.

We are seeking a talented Mental Health Specialist, Court Liaison to join our team of nearly 500 employees. The Mental Health Specialist, Court Liaison, works with justice involved individuals, who are diagnosed with a mental illness, through their transition into treatment and services in the community. The Court Liaison will have a primary role in Monroe County Mental Health Treatment Court and attend all Court appearances to provide routine progress updates of assigned participants on active caseload. This role requires significant collaboration between the Court systems and local treatment/service systems; attention to deadlines, Court requirements and completion of documentation in a timely manner is essential.
CCSI is partnering with Monroe County Office of Mental Health to hire a Mental Health Specialist, Court Liaison This position is employed by CCSI and working on behalf of Monroe County Office of Mental Health.
In this role, you will
Have a routine presence as liaison in Monroe County Mental Health Treatment Court
Maintain caseload of estimated 35-45 participants
Record progress notes in comprehensive, timely manner and complete all reporting as required by the Monroe County Office of Mental Health and Mental Health Treatment Court
Participate in case review meetings with direct Court staff, as scheduled
Receive assigned cases, review and/or formulate recommendations, submit referrals based on client needs
Recommend adjustments to treatment plans, advocates for equity and system movement to support individual identified needs
Obtain status updates and feedback from direct providers and Court officials to assist with development, and implementation of performance improvement activities for participants
Assess discharge planning needs of individuals being released from local jails, hospitals and other treatment agencies and interact with other personnel as needed for release planning
Communicate in a professional manner, orally, written and/or electronically, as a representative of Monroe County to all Court and community providers and partners
Be culturally responsive to the population being served in the development, design, monitoring, evaluation and/or implementation of programs and services.
Have working knowledge of the local behavioral health and social services network to assist with matching individuals with appropriate resources to promote health, wellness, and recovery
Use team approach in working with Socio-Legal Center staff, Courts and community partners.
Incorporate a recognition of diverse clientele into daily activities.

What You Bring
BSW, BS, BA in Mental Health, Social Work, or Human Services related area with 2-3 years of experience in the behavioral health OR Human Services related field

Ability to satisfactorily complete required background screenings (criminal, SCR, driving, and Monroe County Jail)
Valid Class D New York State Driver License and access to reliable transportation to meet transportation requirements of position

Assist with needs, requests (on ICE Detainee Request Form or otherwise), and complaints from the detained noncitizen population regarding non-officer or immigration case related issues.
Assist ERO officers with addressing noncitizen requests related to parent/child communication and/or child welfare involvement.
Facilitate on-site support of disability accommodations, including coordinating follow-up, as requested by ERO, with detention facilities upon submission of a Disability Accommodation Notification to ERO Headquarters (HQ).
Assist ERO and detention facility staff as a resource to facilitate the compliance of applicable agency policies and detention standards. Facilitate discussions and communicate directly with detained noncitizens. Areas of facilitation include but are not limited to the following topic areas: allegations of sexual abuse and assault, identification and monitoring of pregnant noncitizens, resources for noncitizens in segregated housing or special management units, and admission and release.
Address ERO and detention facility staff questions or concerns about specific noncitizens, including available services.
Provide noncitizen referrals to appropriate ERO and detention facility staff for care and follow-up to prevent critical incidents and issues (e.g., medical, mental health, chaplain, etc.).
Support noncitizens with specialized needs or requiring heightened attention and conduct noncitizen wellness checks and follow-ups upon request by ERO Field Office and/or ERO HQ.
Meet with newly detained noncitizens during or after intake to answer questions, address concerns, provide brief orientation to the detention facility and CRC services.
Assist ERO Field Offices who run specialized housing units and programs or services that are designed to provide appropriate care to vulnerable populations, such as: a. Assist ERO Field Office in release planning for noncitizens with vulnerabilities and in general population by researching referrals and identifying needed resources in the U.S. Planning activities may include communicating with family members, securing resources such as medical/mental healthcare, and identifying housing options or placement in a shelter or hospital/institution.
Develop and maintain a national resource database of resources (in and outside of the United States) for CRC and ERO Officer use to facilitate the release and removal of noncitizens booked out of ICE custody.
