POSITION OVERVIEW
The counselor will provide assessment, counseling, advocacy, and referral services to child witnesses of gender-based violence and survivors of teen dating violence. They will work with the clinical team to provide holistic, trauma-informed, and survivor-centered clinical services. They will utilize a trauma-informed approach to promote clients' rights and provide services in a manner that emphasizes safety, respect, and dignity. They will provide strength-based guidance and work to resolve the impact of gender-based violence in their young lives. They will also provide information and outreach to the community. RESPONSIBILITIES
Complete clinical assessments and treatment plans for children and teens referred for counseling;
Provide mental health services for children and teens (from 0-21 years old) including short term individual counseling and crisis intervention with individuals and their families;
Collaborate with staff at other Sanctuary for Families sites and Bronx Family Justice Center Partner agencies to provide holistic services;
Advocate for and assist families/individuals in the negotiation of service delivery system to ensure receipt of appropriate clinical and related services;
Demonstrate competence in evidence-based modalities such as CPP (Child Parent Psychotherapy) and TF-CBT (Trauma Focused Cognitive Behavioral Therapy);
Develop effective relationships with children and families;
Liaise effectively with other professionals in and out of the Bronx Family Justice Center and DA's office in order to meet the needs of the clients;
Facilitate appropriate clinical groups for clients, including, but not limited to, parenting skills groups;
Participate in team and agency meetings, individual supervisory meetings, case conferences and other meetings as requested;
Complete all appropriate paperwork, including case notes and reports, in a timely manner;
Assist with children's room coverage to support team in providing services to families seeking services;

What You'll Do:
Deliver family-centered early intervention services in alignment with each child's Individualized Family Service Plan (IFSP).

Provide counseling, trauma-informed care, and therapeutic support to children and their families.
Actively participate in IFSP development, reviews, and annual updates.

Care Coordinators use psychosocial and/or clinical knowledge to provide non-clinical services for Medicaid recipients to improve the medical compliance and health outcomes of the populations served. This position identifies barriers to medical compliance such as lack of transportation, illiteracy, or other social determinants that impact a member's health, and ensures services are delivered and continuity of care is maintained. The position analyzes the home and community environment and makes autonomous decisions regarding appropriate care plans and goals using a thorough knowledge of available community resources. These services are provided primarily in community and home settings via phone and/or in person. Local daytime travel is required via a reliable means of transportation insured following Company policy. This position will have work-from-home opportunities.

Experience in case management, human services, public health, or experience with the underinsured population
Also requires a valid driver's license in good standing, willingness to submit to vaccine testing and screening, and may require significant face-to-face member contact with duties performed away from the principal place of business. All positions require excellent interview and telephone skills as well as the ability to deal with recipients in a caring and helpful manner. The Care Coordinators should have a working knowledge of health-related service delivery systems and excellent communication and relationship skills. This position requires the ability to analyze varied environmental factors to members' well-being and work independently in an autonomous setting and the ability to locate, augment, and develop resources, including information on services offered by other agencies.

Plans for care to meet these needs and assesses entitlement programs and basic need services. Implements long-term care services plans.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Conducts functional assessment of client health needs.
Using a person-centered approach, develop a plan of care in conjunction with the participant, all allies of the participant who wishes to be involved in the planning process, and the participant's physician. Assist participant with setting both frequency and duration of services, and implement the plan as approved by the participant.
Provides advocacy and arrangements for care not available in the formal network (i.e., socialization, service needs) as specified in participant plans of care.
Monitors participant condition and provision of service pertaining to plans of care. Adjust the plan as determined through reassessment or normal monitoring. Maintain communication with the participant's family and physician.
Maintains participant files and a tracking system for participants applying for and receiving entitlement services, and other pertinent program records.
Seeks clinical consultation from supervisory staff as needed to assure high-quality plans of care.
Manages participant's eligibility process for financial assistance, including communications with DHHS Eligibility Specialist and preparing and tracking necessary applications.
Keeps current on gerontological social work trends and skills needed to be successful.
Adjust to changing guidelines regarding the MI Choice Waiver contract, etc.
