Job Description
Johnson County Developmental Supports (JCDS) is currently seeking a Service Coordinator. JCDS serves people with intellectual and developmental disabilities. We focus on individuals' abilities, provide choice driven supports, and advocate alongside people to live and work in our community. Conducts assessments of individuals' background and applicable circumstances to identify needs and/or determine service eligibility; interprets and communicates relevant information to individuals, families, and other stakeholders.
Develops plans to address the needs of individuals in single or multiple areas of focus; networks with other community agencies to research and coordinate a variety of resources and services to address short term intensive or long term individual needs; encourages the establishment and maintenance of support systems; consults and collaborates with other providers in the development, implementation and review of plans.
Provides a variety of services for a designated case load of individuals with multi-faceted and/or ongoing needs; ensures services provided are consistent with individual plans; initiates and attends individual and/or group meetings and provides services in a community based setting; provides transportation of individuals according to the needs identified in plans; organizes and facilitates skill development and support groups.
Completes and submits documentation regarding services provided; evaluates and documents individuals' progress according to measurable goals; reviews quality and effectiveness of services provided.

Minimum Job Requirements:
Bachelor's degree in Human Services or related field and one year of related work experience are required. May require KS/MO driver's license, depending on department or agency.

The Kansas Soldiers Home, a Medicare/Medicaid State veterans' home located in Southwest Kansas, is currently seeking a full time SocialWorker Specialist for our long-term care unit. The ideal candidate will be passionate about serving a veteran population, have excellent communication skills, and be highly motivated with a "can do" attitude.
Job Responsibilities may include but are not limited to the following:
* Assists residents and their families in adjusting to problems in the resident's lives such as serious illness, handicaps, substance abuse, anti-social behavior, and declining physical/mental health. Individual and group counseling sessions will be offered and the socialworker will provide an atmosphere of understanding and compassion, which will exist in all sessions. Coordinates community/collateral contacts when necessary.
* Interviews residents and their families/representatives before admission to establish the initial communication needed to create a list of needs and wants for the resident. Listens and then documents the requests on the appropriate forms. This may require travel to other agencies, hospitals, and private homes. Reviews documentation from other hospitals/facilities if applicable. Makes recommendations regarding the services needed. Uses well developed, advanced written and verbal communication skills.
* Assists with the establishment of the Community Governance Program. According to state and federal regulations, the facility must establish a Community Governance Program. The socialworker assigned to this caseload will be responsible for establishing this program, making sure the program is in compliance, and for assisting residents with the processes when needed.
* Establishes a baseline assessment of each resident in order to identify the services needed by each resident to attain or maintain the highest practicable physical, mental, and psychosocial well-being possible. Prepares required periodic assessments. Follows established guidelines, policies, and procedures regarding the assessment of each resident. Assessments are recorded on the appropriate forms required by state and federal regulatory entities.
* Establishes goals for each resident. Coordinates the facilitation of the goals and monitors the effectiveness of each service. Adapts or modifies goals as needed in order to provide the best service possible to each resident. Maintains confidential files containing all pertinent information on each resident.
* Acts as a liaison between administrative services and residential services when needed. Advocates for the resident when resident needs outweigh the services available. Works with Administration to assist in developing or obtaining needed services. Prepares presentations or other types of information describing the needed services and offers data indicating the need for such services.
* Serves on committees as required. Assists in the development of policies and procedures when requested. Develops Quality Assurance and Assessment tools to monitor the effectiveness of social work programs. Submits data as requested.

Position Summary & Responsibilities
Position Summary: This position is an integral and critical part of the treatment and discharge plan for the patients on the Psychiatric Services Program of Larned State Hospital. The mission is to provide inpatient mental health care and treatment to adult clients in the LSH catchment area; to provide inpatient evaluation, care and mental health treatment, and substance abuse services in a controlled and secure environment; to improve mental health services to the public in collaboration with community based mental health providers by providing services that include mental health awareness, education and coordinated services and; and to provide quality support services to state agencies in the surrounding area. The SocialWorker Specialist is responsible for gathering and documenting information regarding the patients' treatment, progress, and discharge planning. SocialWorkers are the primary liaisons to families as well as the community resources, relaying information to the team in written form as well as verbally. Socialworkers coordinate placement for patients to ensure that patients are released to the least restrictive environment possible, without endangering safety.
Job Responsibilities may include but are not limited to the following:
Social Work Duties:
* Make collateral contacts (i.e. courts, CMHC's, KDADs, CDDO's, and legitimately involved others) to gather and share information pertinent to the patient's treatment, maintaining professional standards of confidentiality.
