Responsibilities include, but are not limited to:
* Identify economic, cultural, physical, and environmental factors which support or limit a child's functioning

* Assess observations and data, identifying trends and patterns, analyzing, drawing conclusions and making recommendations
* Evaluate developmental and behavioral screenings and make referrals where needed
* Participate in conferences with parents and teachers when disabilities and/or mental health issues are present
* Represent the child and their parents with community agencies in regard to mental health and disabilities
* Complete documentation, data entry, and reports in an accurate and timely manner
* Monitor in-kind documentation for children who are receiving special needs services with outside agencies
* Utilize rack data from completed of Ages & Stages Questionnaire (behavioral & developmental) on all enrolled children
* Track and review Individual Education Plans (IEP) and Individual Family Service Plans (IFSP) for children and ensure that staff are developing activities and goals around these plans
* Develop community resources around Mental Health Agencies in community meet with staff when approved to discuss Mental Health or Disability issues
* Perform other duties as assigned or delegated by the Director for the Center for Families.
REQUIRED QUALIFICATIONS:
* Bachelor s degree in mental health for children, special education OR related field.
* Two years of work experience in mental health or special education early childhood setting

PHYSICAL WORK ENVIRONMENT:
Work is typically performed in an office setting. The ability to sit at a desk/computer station, bend, stoop, walk and generally move around the office to consult with others, work within a busy environment and remain focused on tasks. Inventory management requires some lifting and moving of objects up to 30lbs. Excellent communication skills are essential. This includes the ability to speak clearly and fully comprehend written and spoken English and will include communication with people for whom English is not their first language. COMPENSATION:
Salary is $36.93/hour for 15 hour per week schedule and is based on the Edmonds CC administrative/exempt salary schedule. This temporary position is not eligible for benefits.
WORK SCHEDULE:
This is a 6-month non-permanent position. 15 hours per week. Hours would typically be between 9-4 M-Th and Fri 1-4pm. Some evenings may be required.

Your contribution to the team includes:
* Provide advanced biopsychosocial evaluation, assessment, triage, referral, and support to patients with complex medical, behavioral health and social needs by actively partnering with interdisciplinary care teams, focusing on overcoming social drivers of health and improving health equity. Develop, maintain, and advance individualized care plans with patients, focusing on individuals at high risk for poor health outcomes or avoidable high-cost care and actively facilitating achievement of health and wellness goals.
* Provide intake, longitudinal care management, and Social Work support to identified patients including behavioral health treatment planning, crisis intervention, transition of care support, resource navigation, community resource procurement, care coordination, emotional and short-term behavioral health support. Evaluate acuity of needs and assist patients in overcoming barriers to optimal health and wellness, promoting graduation from care coordination when appropriate and maintaining appropriate patient panel size.
* Provide advanced assessment and coordination of care for patients with complex behavioral health conditions or significant social challenges. Act as team and organizational resource providing Social Work expertise and perspective. Assist Supervisor with training and onboarding of peers, including mentoring and precepting.
* Actively maintain engaged patient panel utilizing proactive person-centered techniques and approaches such as critical thinking, motivational interviewing, case finding, SMART goal setting, health coaching, patient-empowerment, relationship-building and proactive independent collaboration. Work to improve health equity by identifying opportunities for system improvement, advocating for and implementing person-centered approaches to care.
* Be committed to a continuous learning environment where programmatic goals will shift based on the healthcare environment, requiring flexibility and prioritization. Provide advanced care coordination and Social Work support for patients in various programs including pilot projects, grant-funded initiatives or other populations as identified in collaboration with leadership. Travel to various locations including clinic, community, and home visits to provide care and support as needed.
* Actively partner with care team members to provide advanced psychosocial support and Social Work expertise especially for situations involving domestic violence, homelessness, trauma, substance use, crisis intervention, complex family dynamics, newly arrived refugees and other complex social or behavioral health situations. Promote patient self-management, self-determination, and person-centered care. Complete any payer contract requirements including verification of patient eligibility, coordination of appointments, attending required trainings, administering and documenting screenings within required timeframes.
* Utilize patient-engagement skills to positively impact quality metrics, program, and clinical outcomes with designated patients. Be accountable for improving health outcomes, utilization rates, patient satisfaction, and self-sufficiency for a defined population of patients in alignment with evolving organization and population health goals for people with complex health and social situations.
* Maintain professional relationships and boundaries while supporting patients, families or caregivers with empathy, compassion and cultural congruence and maintaining respect for confidentiality, privacy, and mandated reporting.
* Identify and take appropriate action on patient safety situations, including assessing and facilitating patient safety planning, referrals and connections utilizing HealthPoint safety protocols, state and local guidelines. * Identify opportunities and lead initiatives in population health approaches to patient care and support. Qualifications

BASIC REQUIREMENTS: Degree: Social Work. Must fulfill all of the requirements for the Master's Degree in social work in a school accredited by the Council on Social Work Education.
ADDITIONAL EXPERIENCE REQUIRED: In addition to meeting the basic entry level qualification requirements, applicants must have the following:
GS-12: 1 year of specialized experience equivalent to the GS-11 grade level in the Federal service (obtained in either the public or private sector) which includes work involving applying practical knowledge of a wide range of social casework, family therapy, systems theory and community organization; direct provision of child welfare services, maintaining active agency participation in the varied advocacy and intervention programs and the coordination of and required cooperation with allied social service, health and other agencies; providing counseling services; developing and maintaining a formalized case management system which documents the workflow of case services with individual case plans; provides technical assistance on all aspects of social services programs and activities; and develops and supports program budget matters. Responsibilities
What You'll Do:
The Behavioral Health Specialist (BHS) manages and maintains the safety of the individuals and staff. Takes vitals and assists individuals in meeting their needs while on the unit by providing exceptional customer service. Documents services provided and significant interactions in the Electronic Health Record (EHR).
* Conducts security screening and inventories property of individuals upon admission to the facility. Provides orientation to the Observation Unit and Short-term Inpatient Unit. Educates patients on their rights under ADHS regulations, grievance process, provides expectations, and explains rules and regulations.
* Intervenes with escalated individuals using therapeutic techniques for de-escalation and performs restraints/seclusions when needed to ensure the individual's safety and the safety of others. Completes 1-to-1 on a patient in seclusion or mechanical restraints Q15.
* Lifts, moves, and transports individuals using proper body mechanics or lifting devices for accident prevention.
* Assists individuals with activities of daily living to include eating, bathing, dressing, toileting, and transferring.
* Conducts and documents individual's vitals and reports all questionable results to the RN/Physician attending the individual. Conducts and documents all BAL, urine drug screens, and urine pregnancy tests.
* Attends court ordered hearings when subpoenaed. Must be willing to be a witness on a petition and available to provide testimony at court ordered hearings.
* Documents all services and significant individual interactions in the Electronic Health Record. Monitors and documents patients Q15.
* Responsible for maintaining and ensuring the cleanliness of the facility and designated assigned unit. Performs light cleaning of interior center, collects soiled linen, and launders individuals clothing when needed.

STATEMENT OF ESSENTIAL JOB DUTIES
* Conducts investigations and evaluations of individuals for involuntary commitment and detains individuals when legal criteria are met and commitment is appropriate.
* Assesses behavioral health emergencies and provides crisis intervention for individuals experiencing a behavioral health crisis in the community.

Minimum Qualifications
