HOW YOU'LL WORK:You'll provide patients with access to community resources and ensure that the patients' medical, emotional and safety needs are met. ESSENTIAL FUNCTIONS LISTED BUT NOT LIMITED TO:
I. WORKS A SCHEDULE THAT MEETS THE NEEDS OF THE DEPARTMENT AND THE
AGENCY
Works scheduled hours as designated by supervisor. Submits requests for the use of benefit time and schedule changes to supervisor. Uses timekeeping system per agency procedures. Accepts caseload and team assignments per supervisor. Actively participate on the floor with students a minimum of 3-4 hours a day. Establishes daily work schedule in coordination with others' schedules and informs supervisor. Meets with the supervisor regularly to review clinical treatment progress, discuss job responsibilities, and address any concerns or areas requiring support.
II. MEETS THE OVERALL AGENCY & DEPARTMENTAL EXPECTATIONS:
A: Treatment Coordination
Collaborates with the internal and external team to develop, implement, and monitor Individualized Treatment Plans and Behavioral Support Plans. Ensures treatment plans are person-centered, clinically appropriate, and aligned with agency philosophy and Indiana DCS standards for residential placement. Ensures all placements and treatment decisions reflect DCS principles of maintaining the least-restrictive environment, placement stability, and the child's best interests. Monitors progress toward treatment goals, documents outcomes, and revises plans as needed in consultation with all appropriate parties to the student's treatment. Coordinates discharge planning consistent with DCS transition expectations, ensuring continuity of care and appropriate community-based supports. Promotes positive, proactive discharge planning throughout the student's stay. Identifies needs and long-term plans including daily living skills, reunification, and discharge resources for students and their family. Consults with supervisor to keep them informed and to seek guidance on issues such as family/funder relationships or dynamics, internal team issues, funding issues, significant issues, and general student needs. Aftercare Services are offered to DCS students when discharged from T.C Harris. This is an extra support to the placement and the student when they leave from a residential placement. Will comply with the Aftercare Standard provided by DCS when applicable.
B: Case Management and Communication
Serves as the primary point of contact for assigned student caseloads, maintaining consistent and professional communication with guardians, case managers, and other referring agencies. Will need to conduct supervision of zoom calls or phone calls with the external team and their student. Follow all proper documentation once complete. Will sometimes be asked to be present in court (virtually/physically) with student or with the student to discuss progress. Facilitates/Attends Treatment Reviews (CST and TTR), Team Meetings, IEP Meetings and Discharge Planning, ensuring that all communication and documentation meet agency standards. Provides timely notifications and progress updates to external stakeholders as required by DCS policy (e.g., incidents, treatment changes, or critical updates). Ensures that all case coordinator activities prioritize student safety, family engagement, and DCS/Placement Agency permanency planning goals.
C: Clinical Collaboration and Oversight
Partners with program and direct care staff to ensure consistent implementation of the student Behavioral Support Plan (BSP). Conducts or assists with assessments, such as Functional Life Skills and Ansell-Casey, to inform treatment goals and identify strengths and needs. Collaborates with other members of clinical and program leaders to maintain compliance with DCS service delivery standards and agency treatment philosophies. Meets 1:1 once a month at a minimum with each client on assigned unit. Supports positive, proactive discharge planning and ensures continuity between residential treatment and community-based services since the arrival of the student.
Ensures that programming is developed that is of a therapeutic nature and takes into consideration age, level of functioning, likes/dislikes, cultural/ethnic differences, resident risks, appropriateness of activity, and recreation budget

Document treatment plans, psychotherapy sessions, and treatment plan updates according to Eventus WholeHealth's procedures and policies and provide the facility with written documentation within 48 hours of service for inclusion in the patient's facility medical record.
Meet with Resident Care Coordinator or other staff as designated by the facility upon arrival to and departure from facility to review current facility care needs and communicate findings or concerns.
Establish and maintain open and positive communications with facility staff and administration. Provide verbal and/or written instruction or feedback regarding treatment plans and other pertinent caregiver information.
Be knowledgeable of and adhere to Eventus WholeHealth standards, policies, and procedures. Be aware of and adhere to all legal and regulatory agencies' rules and guidelines and professional ethical standards. Comply with all regulatory agencies governing health care delivery.
Conduct self in a professional manner at all times.
Maintain patient confidentiality at all times including appropriate use of cell phone, email, text messaging, patient records, and EMR. Agree to abide by and be knowledgable of HIPAA rules and regulations.
Consult with the clinical supervisor when facing an unfamiliar resident, family, or facility request or recommendation regarding the care of a resident.
Qualifications/Education: Psychologists-Doctoral degree from an accredited educational institution and a current license as a doctoral-level Psychologist and Health Services Provider issued by the state in which you will practice, required. Licensed Clinical SocialWorkers-Master's degree insocial work, and a current license to practice in the state in which you will practice, required. Knowledge:
Knowledge of assessment techniques, psychotherapy techniques, population-specific interventions, and delivering care following treatment plan.
Knowledge of how to use a variety of diagnostic tools and techniques, including the current edition of the DSM.
Knowledge of rules and regulations of bodies governing psychotherapy practice.
Knowledge of organization's policies and procedures.

The Behavioral Health Counselor MS facilitates patient involvement in treatment and education within a multidisciplinary environment. Key Responsibilities:
Facilitate face-to-face (or telehealth) individual, group, and family counseling sessions.
Perform utilization review and discharge planning to support safe patient transitions.

Qualifications & Requirements:
Education: Master's Degree inSocial Work, Counseling, Psychology, or a related mental health field from an accredited college or university.
Experience: 0-3 years relevant healthcare, mental health, or community-based experience preferred.
Licensure: License-eligible inIndiana as an LCSW, LMHC, LMFT, or LCAC. Must have a minimum of a permanent LSW, LMHC-A, or LMFT-A upon hire.
Certifications:
Crisis intervention certification required (may be obtained after hire).
BLS certification may be required.
Knowledge & Skills:
