MINIMUM QUALIFICATIONS:
Masters degree in social work or counselling and licensure as clinical socialworker (LCSW) or clinical counselor (LPCC).
Must have knowledge of current theoretical approaches to the delivery of counseling services.
Must have knowledge of strategies for examining, applying and evaluating cognitive/behavioral therapy concepts.
Additional skills required include thorough knowledge of the clinical aspects of the mental health field

PRIMARY FUNCTION Are you a Behavioral Health Provider who can support the behavioral health needs of patients served in the primary care offices through provision of clinical social work services, including screening and brief intervention, assessment, diagnosis, treatment planning and coordination, and monitoring of mental health needs? Participate as an active member in the patient's outpatient primary care team.
Assist colleagues in managing behavioral health crises in the clinic.
Utilize clinical interventions from a variety of theoretical bases to treat patient mental health or substance use conditions, including enhancement of motivation to address mental health, physical health or substance use conditions, development of diagnosis, treatment plan, and plan for termination of treatment episode.
Maintain problem-oriented, timely, accurate, detailed clinical records. Assign appropriate diagnosis(es) for a range of behavioral health conditions, including substance use disorders.
Engage patient in identification of specific treatment goals and interventions/supports needed to achieve these goals. Utilizes standardized screenings and assessments in accordance with organizational clinical practice guidelines and quality improvement initiatives.
When applicable and as indicated by patient need, provide individual, family, couples and/or group therapy to patients.
Provide care management services to patients with behavioral health and substance use needs, including follow up on referrals to appropriate outside or specialty behavioral health resources and collaboration with Enabling Services to ensure that patient psychosocial needs are addressed.
As requested, participate with MSW programs in student practicum rotations.
Assist with other duties or in other departments as instructed by supervisor.
JOB REQUIREMENTS Minimum Education: Master of Social Work degree from an accredited institution of higher learning; passed the ASWB Master's Level exam. Minimum Work Experience: Two years experience in a patient care setting. Licensure Required: Maintain unrestricted Kentucky CSW license and meets ongoing licensure requirements. Impact Recruiting Solutions is currently seeking a Clinical SocialWorker Health Care Facility Surveyor to fill an opening with a Quality Improvement Consulting Company and will work in a technically exciting environment supporting internal and external customers nationwide.
Requirements of the Clinical SocialWorker Health Care Facility Surveyor:
Must have a Master's degree in Social Work
Must have successfully completed CMS's Long-Term Care Basic Training and passed the Surveyor Minimum Qualifications Test (SMQT).
Must have at least two (2) years of recent experience working as a surveyor accrediting or certifying facilities that serve the residents of long-term care facilities.
Must maintain current licensure to practice as an LCSW
Demonstrated history of independent decision-making skills to direct and effectively manage the survey process.
Ability to set priorities independently and collectively in performing survey tasks.
Ability to openly discuss conflicts/controversy, and to seek assistance when appropriate to make decisions and resolve conflicts.
U.S. Citizenship is required for this position
Ability to travel up to 75% of the time on a regular basis is required
