Responsibilities:
As a Hospice Clinical SocialWorker, you will:
Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and dignity of each patient.
Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
Assess the social, spiritual, and emotional needs/factors in order to estimate the patient's and involved caregiver's capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.
Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning
Utilize all available resources, such as chosen family, hospice, and community agencies, to assist the patient and chosen family to live better within the limitations of the illness
Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care
Required Certifications and Licensures:
Licensed to practice as a clinical socialworker in the state of agency operation if required
Must be a licensed driver who can travel to all business locations

Family Time Coaching, Intervention, and Skills Training - 70%
* Provide intensive in the room supervision and hands on skills training intervention for in office and in-home family time between parents, other relatives, and children. This could include coaching and modeling designed to increase a caregiver's ability to read the child's cues and appropriately meet their needs.
* Provide in-the-moment interventions that uphold the child's physical and emotional safety as well as the Family Time Center policies and guidelines.
* Provide parent education either formally through structured curriculum, or informally through interventions and modeling, for each case.
* Create goals with clients using strength-based engagement techniques.
* Provide feedback to parents to further educate, evaluate and review progress toward permanency for children.

Compensation:

Shifts:
Days: Sunday-Wednesday or Wednesday-Saturday
Overnights: Saturday-Tuesday or Tuesday-Saturday
Part time float positions are also available (will require travel to all locations)
Responsibilities:
Work with MA level Clinicians and Special Education Teachers on a daily basis
Collaborate within the framework of the client's treatment team in the completion of instructional activities and behavior management
Implement treatment plans and individualized education plans in an appropriate manner as directed by the client's clinician and teacher
Implement and maintain structure within the program and create/follow a routine
Provide intensive supervision, documentation, and organization
Utilize strong verbal de-escalation and positive strong boundaries
Basic meal preparation and light housekeeping
Conduct daily room inspections and15-minute bed checks with a focus on safety and security
Transport clients to and from school, activities, appointments, etc.
Attend treatment team and required trainings
Responsible for working with youth who have emotional and behavioral issues resulting from trauma and neglect, and who display verbal and physical aggression

This screening includes state criminal background checks, fingerprinting, sex offender registry searches, and abuse and neglect clearances. If the applicant has lived in any other state within the past five years, equivalent background and abuse/neglect checks from those states will also be conducted. In addition, all direct care applicants are required to complete a health evaluation or physical prior to beginning service to confirm they are fit for duty.
Minimum Qualifications: Applicants must be at least 23 years of age (will consider 22 years of age) and meet one of the following educational and experience requirements:
Two (2) years of college education (equivalent to 60 semester credits); OR
An Associate's degree; OR
A high school diploma or equivalent plus two (2) years of experience in the human services field; OR
A high school diploma or equivalent, plus 30 college semester credits, and at least one (1) year of experience in the human services field
Preferred Qualification:
Bachelor's Degree in related field

Clinicians will work with students and staff to identify and treat social, emotional, and behavioral issues in a positive and fast-paced setting. Responsibilities for this role include providing direct support to students to help them understand and manage their emotions, build resilience, and develop healthy coping strategies. The SocialWorker will also create individualized support plans and conduct interventions to address issues such as bullying, crisis situations, and behavioral challenges. Throughout the school year, they will provide counseling services to students in individual and group settings. The SocialWorker will also assist in providing training and resources to teachers and staff on effective strategies to drive social and emotional development both in school and at home.
