EXAMPLES OF DUTIES Receive, analyze and document reports of alleged maltreatment and unborn child abuse. Meet with families to identify and prioritize needs and goals and to consider appropriate available services. Establish service agreements with families that ensure child safety, address identified needs within the family, and are consistent with juvenile court orders. Make referrals to appropriate community services and resources. Maintain regular contacts with schools, treatment providers and other involved professionals to ensure service provision, effectiveness, and family's progress. Maintain regular contact with the families to monitor progress and to assist them in following the case plan. Prepare factual, accurate and comprehensive petitions and court reports according to agency policy and file them in court within legal time limits. Attend court hearings according to agency policy and effectively/ proactively participate in the process. Contact families promptly after case assignment to schedule meetings. Prepare for assessments by reviewing the case record and other available information prior to family meetings. Conduct assessment interviews with family members according to Department protocols and State mandates. Gather additional information from collateral sources to complete assessments. Within required time limits, complete comprehensive and accurate family assessments, taking into consideration family and individual strengths and needs. Conduct initial assessments by reviewing all relevant information, interviewing all persons according to state standards and within mandated response times. Analyze information regarding risk level, child safety, and maltreatment substantiation. Education, Training and Experience: Applicants with a Bachelor's or Master's Degree in Social Work, or who are currently in their final academic semester towards obtaining a Bachelor's or Master's Degree in Social Work will be screened through. Applicants with any other major must possess a training certificate, temporary or permanent Social Work Certification from the Wisconsin Department of Safety and Professional Services (DSPS) prior to applying. Social Worker II: All employees hired with a Master's degree and six (6) months of post-graduate t experience relevant to the position or a Bachelor's degree and two-and-a-half (2.5) years of post-graduate experience relevant to the position will be hired as a Social Worker II, range 20. Those who do not meet these criteria upon hire will be hired as Social Worker I and will advance to Social Worker II after obtaining one of these criteria. Certifications/Licenses: All individuals must possess a degree and a training certificate, temporary or permanent Social Work Certification from the Wisconsin DSPS prior to the first day of employment. If any applicant does not possess a training certificate, temporary or permanent certification on the first day of employment the job offer will be rescinded and they will be terminated. Candidates with a training or temporary certificate must obtain a permanent certification within one (1) year in order to pass probation. Works with the other social work staff to improve quality of services provided campus-wide. Develops social work policies and procedures for the Assisted Living program, and works with intercampus Social Workers to develop or review campus-wide social work policies and procedures. Maintains concise and accurate resident documentation including social histories, assessments, and assists with annual reviews for SNF residents. Assists Director of Nursing in ensuring all state and federal regulations related to abuse or misappropriation are adhered to. Assists residents with completing paperwork to activate their Long-Term Care Insurance. Arranges transportation for residents to/from doctor appointments. Coordinates specific resident needs with available community resources as appropriate. Investigates and completes resident incident reports. Offers grief and loss support individually and/or in groups. Welcomes new residents and coordinates their orientation to SNF. ESSENTIAL QUALIFICATIONS Knowledge of HFS-89 and standard social work practice. Provides informal leadership which fosters a team approach in meeting the mission of Oakwood. Relates well and demonstrates empathy and compassion towards our residents, families and staff members. Establishes and maintain a positive working relationship with the community. Maintains and protects the confidentiality of information. Exercises independent judgment and make sound decisions. Plans, organizes, and prioritizes work schedule to meet deadlines and business needs. Demonstrates strong communication skills, both written and verbal. Displays objectivity in working with needs and concerns of residents and families as well as ability to follow through to resolve issues. EXPERIENCE, EDUCATION, AND/OR TRAINING Bachelor's degree in Social Work, Psychology, Sociology, or related human service field. Wisconsin Social Work certification. At least one year of experience working in the health care industry required. Experience working in a Community Based Residential Facility with geriatric population desirable.
