The position is based at our Racine location - 5200 Washington Ave, Racine, WI 53406. *****Sign On Bonus $5,000.00***** Social Worker Job Responsibilities: Provides direct social work case management services in Community Care's Program of All Inclusive Care (PACE). Maintains documentation standards and member related paperwork. Facilitates team meetings. Occasional local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage. Job Requirements: Education: Master's degree in Social Work required Licensure: Current Wisconsin Social Work licensure required Experience: Minimum of 1 year experience providing care or services for a frail or elderly population required. 1 to 2 years case management experience with the elderly and/or developmentally or physically disabled population preferred. Social Worker Job Schedule: 8:00AM - 4:30PM, 40 hours per week No weekends/holidays No on-call responsibilities About Community Care Community Care is a local nonprofit with more than 40 years of experience helping older adults and adults with disabilities live as independently as possible within the community.
