Salary Range/Hourly Rate: $40/hr
Dates: Prep/PD week of June 29th. Programming will run from July 6-30th, 2026.
Work Days/Times: Student contact days will be Mon-Thurs from 7:15am-1:30pm. 30 minutes of flexible prep time each day will be scheduled. Can be taken before or after programming times listed previously.

Position Summary:
The White Bear Lake Area Schools is currently seeking a SocialWorker/Psychologist to work in our Camp Summer and Summer U programs. Performs professional work assessing and delivering instructional services to individual and small groups of students with social-emotional or behavioral needs. Work is performed under the limited supervision of Student Support Services Administrators. 2 positions are available and there will be the ability to collaborate as a team to support all 4 sites.
**Positions will be responsible for
pulling and reviewing all IEP/504 documents for their assigned sites in May or early June 2026 to ensure proper preparation and staffing in collaboration with the Camp Summer/Summer U Coordinator at the assigned site. This will include creating and delivering any staff training on accommodations, purchasing or collecting of accommodation materials, assigning break times, etc.
Education and /or Experience
Licensed as a School Psychologist or SocialWorker
Special Requirements:
Minnesota Psychologist or Social Work License.
Experience working with White Bear Lake Area Schools preferred.

Position Objective
To provide social work and case management services. This position is responsible for identifying needs and developing case plans for children and families involving complex social problems.
Essential Duties and Responsibilities

Minimum Qualifications

Bachelor's degree or higher in social work, psychology, sociology or closely related field

About this position
The Licensed Graduate SocialWorker (LGSW) provides services to customers and their families to help them adjust to physical or mental illness, injury or hospitalization. Assists the interdisciplinary treatment team in understanding the significance of social, economic, emotional and family factors in relation to the customer's illness, treatment, recovery or dying processes. Responsible for the biopsychosocial assessments and discharge planning for customers with complex psychosocial and medical and/or mental health problems in collaboration with the responsible family and case management team. Identifies needed community services and coordinates appropriate services for customers in need.

They are responsible for the discharge planning and case management for the customers they are assigned.
Requirements
Education
• Masters Degree from an accredited university or college in social work working towards license as an Independent Clinical SocialWorker and fulfills the requirements as set forth by the Minnesota Board of Social Work.
Experience
• Socialworkers who are required by clinical assignment to psychiatrically diagnosis or engage in the practice of psychotherapy shall have a minimum of 4000 hours of post-graduate clinical social mental health professional as defined by Minnesota statute

This position is accountable for identifying the social, psychological, spiritual and emotional strengths and needs of residents and the provision of services to meet such needs for all residents on assigned units, thus alleviating the trauma associated with nursing home placement. This position has the responsibility for assuring compliance with the Resident's Bill of Rights, the Patient's Self-Determination Act as well as meeting the requirements of the State and Federal regulations and facility policies and procedures on a continuing basis. NATURE AND SCOPE
This position reports to the Healthcare Administrator. In performance of duties this position is entrusted with, but not limited to, the following responsibilities:
· Assess the psycho-social strengths and needs of residents on the unit to which the socialworker is assigned. Develop and maintain a social history, which becomes a part of the resident's permanent medical record.
· Provide intensive and/or continuing counseling services to residents and families on assigned nursing unit during the adjustment period of placement and thereafter as needed. Assist in orientation of families to the facility.
· Attend and facilitate family conferences, discharge conferences and patient care conferences.
· Maintain an awareness of community resources. Act as a liaison with social, health and community agencies. Coordinate discharge planning, transfers and room changes of residents for whom it is appropriate.
· Serve as an advocate for residents, including those instances in which appointment of a legal conservator, guardian, durable power of attorney or trustee is appropriate.
· Complete required documentation, including MDS and quarterly summaries.
· Serve as a member of the admissions committee. Participate in other committees and attend meetings, which have a direct impact on quality care for residents.
· Assist as needed in marketing the facility to prospective residents, their families and the community at large. Maintain communication and positive working relationships with referral sources. Demonstrate an attitude of customer service in all situations.
· Develop, and update as needed but a least annually, department policies and procedures.
· Participate in the Keynotes Speakers Group as requested.
· Develop and facilitate education/support groups as requested.

SocialWorker Responsibilities:
Provide valuable social services to residents and their families in medical, emotional, physical, or psychological concerns.
Assist the team in coordinating social services for the facility, individualized plans for the residents, medical equipment, counseling or other follow-up treatment plans.
Commitment to compassion, excellence and quality care.
Act as liaison between families, staff, and various agencies.
Act as an advocate ensuring basic human and resident rights.
Conduct and coordinate Admission, provide appropriate referrals and discharge planning of residents.
Provides resources and referrals to staff, residents, and family members.
Provide additional tasks as required.
SocialWorker Qualifications:
Must be a licensed SocialWorker (LSW) or be licensed within 60 days of hire.
Bachelor Degree in Social Work required.
Experience working in LTC and TCU preferred.
Must have knowledge of state and federal guidelines related to long-term care and resident rights.
Successful candidates will be mission-minded with excellent communication skills.

Overview:
This placement is a school-year internship, under the individual supervision of a faculty socialworker, for the purpose of providing experience and guidance in individual counseling, group work, prevention methods and clinical diagnosis in a school setting. The social work intern serves students whose social-emotional difficulties interfere with his/her capacity to function adequately as a student. The social work intern professionally assesses the problem and, through consultation, develops a plan to assess and support the student. The plan may involve both the school and/or community resources. This professional relationship is dependent upon its confidential nature.
Supervision Provided
Weekly individual supervision with a socialworker with two or more years of experience.
Weekly group supervision focused on integrating university coursework with practical application and case consultation.
Qualifications:
Must be currently enrolled in, or have completed, a Master of Social Work (MSW) degree program and eligible for field placement as determined by the sending university.
Must be pursuing an Illinois Professional Educator License (PEL) with a Social Work endorsement and eligible to receive the license at the completion of the internship.

Position Expectations:
Participates with Social Work Department members in developing and implementing program policies and procedures and preventing crises.
Acts as a Mandated Reporter on all issues of child abuse and neglect.
Assists in assessing crisis situations, helping others on the team identify potential solutions, identifying resources that may be helpful in problem resolution, and developing plans for the resolution of the crisis.
Completes initial assessments on newly referred students.
Completes social developmental histories on students being assessed for Special Education eligibility.
Provides school based social work interventions, including but not limited to individual and group counseling, crisis intervention, case management, collaborations with school staff and administrative team.
Establishes a referral network and communication with outside agencies for both students and parents.
Participates in Individual Education Plan meetings, including reporting on present levels of performance, writing goals related to IEP eligibility, and determining level of service needed.
Occasionally presents at teacher meetings.
Serves as an advocate for students' rights within the school.
Implements the philosophy and objectives of the school's social work program.
Remains available for crisis intervention social work as the need arises.
Participates in relevant professional development training.
Performs other related duties as required.

This is a 0.6FTE (6 shifts per two week pay period) Acute Care Float Social Work position at Fairview- The University of Minnesota Medical Center! This position floats to cover various units throughout the hospital including the ICU, Cardiology, Neurology, Ortho, Med/Surg, Observation and Emergency Departments. The hours are 8a-4:30pm. There is an every 6th weekend rotation and 1 holiday per calendar year requirement.
The acute socialworker works on-site with the interdisciplinary teams including providers, physical therapists, nurses, dieticians, chaplains and pharmacists to assist patients and families in navigating through their hospitalization. The acute socialworker completes psychosocial assessments, care planning/ progression and creates comprehensive discharge plans from the hospital. The socialworker is patient facing, responds to crisis situations, manages patient and family expectations, creates solutions to identified discharge barriers and aims to reduce length of stay and prevent avoidable hospital readmissions.
Responsibilities
* Attends daily in person interdisciplinary care team rounds
* Conducts thorough psychosocial assessments with patients and/ or their surrogate decision makers
* Helps the care team navigate and mediate complex psychosocial dynamics
* Is well versed in the surrogate decision maker policy and assists care team members with identifying surrogate decision makers according to the policy
* Communicates frequently with the interdisciplinary team to ensure continuity and effective coordination of care
* Provides emotional support and counseling surrounding adjustment to sudden illness, grief/bereavement and end of life planning
* Addresses social determinants of health and assists with connecting patients to needed resources in the community such as chemical dependency treatment, domestic violence resources, housing stabilization services, mental health services, food shelves, transportation, financial assistance etc.
* Arranges and facilitates in person care conferences as needed to support patients and their families surrounding navigating complex medical decision-making processes/ goals of care
* Documents all interactions thoroughly and timely following established department standard work
* Serves as the expert in system adult and child protection reporting policies
* Collaborates with patients, families and interdisciplinary teams in developing an individualized discharge plan based on age, social and cultural considerations, clinical diagnoses, level of functioning and other relevant needs
* Participates in hospital initiatives and projects to reduce length of stay and related barriers which may include collaborating with providers for clinical adjustments in care
* Assists with ensuring the hospital is in regulatory compliance for care management related conditions of participation
* Assists with efforts to improve patient flow in a high-capacity hospital setting
* Works in a fast-paced hospital environment
* Effectively re-prioritizes competing time sensitive job demands
* Demonstrates the ability to advance evidence-based practice by utilizing standards, guidelines and pathways for care delivery
* Incorporates data and information into practice to continually improve care and practice to enhance patient outcomes
* Supports site and system quality initiatives
* Provide age appropriate short- and long-term therapeutic interventions
* If a licensed supervisor, provide clinical supervision as requested (site dependent)
