Essential Duties and Responsibilities:
Provides necessary social work intervention
Counsels individuals and families in crisis and help with issues such as poverty, loss and illness
Assesses patient's psychosocial needs, plans for, evaluates and modifies interventions to meet the established patient/client goals
Collaborates with all disciplines to plan and evaluate team goals for each patient/client
Facilitates patient/client and family referrals to community agencies when appropriate
Provides financial counseling when necessary to achieve the treatment goals established by the multi- disciplinary team
May be required to respond to crisis situations, including domestic violence and child abuse
May be required to provide witness services and evidence in court cases

Observes and reports patient progress and/or changes in patient status to physician and appropriate multi- disciplinary team members

Minimum Requirements:
Master's degree in Social Work from a school of social work accredited by the Council on Social Work Education required
Active license as an LCSW in state of practice required
Minimum of one year of social work experience preferred
Complies with all relevant professional standards of practice
Participation and completion of Amergis' Competency program when applicable
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or state regulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable to job site

Job Overview:
IRC's Anti-Trafficking Program is designed to find sustainable solutions that are appropriate for the unique situations of each client. Caseworkers serve clients through a holistic and broad spectrum of support, including but not limited to connecting clients with legal service providers and housing resources, transportation, helping clients access public benefits, health providers, and psychosocial services and offering regular checks-ins to discuss safety and service plans. Case Management and Referrals
Provide trauma-informed comprehensive case management for minor survivors of trafficking to help them feel safe, empowered, and stable.

Complete individualized initial assessment and safety planning, orientation to services, support, referral, and advocacy services by assessing and evaluating clients' distinct needs.

Community Outreach and Education
Develop and implement a region-wide outreach plan to increase awareness regarding the needs of Minor Victims of Trafficking survivors
Coordinate with Human Trafficking Supervisor regarding outreach plan and travel to sites to meet with potential develop and implement a region-wide outreach plan
Creation and distribution of informational material and posters throughout the community to hotels, truck stops, bus stations, law enforcement, hospitals, businesses, school administrators, and in other public places.
Host public information meetings at faith-based organizations and locations as needed
Partnerships and Community Engagement
Build partnerships with coalition members and providers; work to connect clients to available resources, including various local, state, and federal law enforcement agencies. With their assistance, develop safety plans/protocols for clients, staff and the office.
Work alongside coalitions to improve regional services available to survivors of trafficking via community outreach, training, and engagement.
Participate in all IRC program meetings, staff development activities, and fully engage as a member of the team.
Documentation and Reporting
Ensure quality case notes and documentation for all contacts made and services provided to partners and on behalf of client and for case file management and quality assurance.

Minimum 2 years of trauma-informed, victim centered, relevant work experience in advocacy, social service, case management or related fields. Experience should include a minimum of one year providing direct service to underserved and marginalized communities. Excellent understanding of the criminal justice system as it relates to prosecution of traffickers, survivor protection laws, confidentiality and case management principles.

Requirements:
Must have a master's degree in social work from an accredited university.
If you hold an undergraduate B.A, Degree in Social Work, you are eligible to apply. Please know the hirable rate of pay will be based on degree type.
Must have minimum of two (2) years of documented supervised experience in health care, hospice experience preferred
Must understand hospice philosophy, and issues of death/dying.
Certified Hospice and Palliative SocialWorker is desirable
Must be flexible in work hours and have the ability to travel throughout the assigned Bristol Hospice service area
Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs

ESSENTIAL DUTIES & RESPONSIBILITIES
The essential functions include, but are not limited to the following:
Interview participants to determine eligibility to receive cash assistance and supportive services from the CTTP program.
Identify barriers to self sufficiency and provide family with tools to remove those barriers.
Meet with participant each month to receive Monthly Eligibility Report. Enter current information into the TAS system monthly.
Review participant work log each month to ensure participant is meeting work hours.
Refer participants to community resources and other organizations.
Compile records and prepare reports.
Enter participant case plan and information into TAS computer system.
Implement participant case plan.
Secure supplementary information, such as employment, medical records, and/or school reports.
Provide home visits when necessary.
Attend court appearances as needed to assist participants.
Make child abuse reports when necessary.
Work with participants and agencies relative to domestic violence cases.
Transport participants when necessary.
Perform janitorial duties to keep the office clean and safe.
Set up confidential filing system.
Perform other duties as needed.
