Advise parents, by interviewing the child/adolescent and their family to determine if additional actions are required, such as referring the child or adolescent according to their needs or problems. Maintain case history records and prepare reports. Advise parents, families, and the community regarding topics including mental health, child care, social adjustment, and other topics. Coordinate different activities for the community, parents, and family (educational, cultural, and social activities). Advise participants whose behavior, school progress, or physical or mental impairment indicates a need for assistance and the organization of necessary services. Advocate for individual or community needs. Collaborate with other Club professionals to assess the needs of children and adolescents. Collaborate and participate in organizational events. The work requires being sensitive to others and having a positive role with the participants and the community. Perform any other task requested by the supervisor. EXPERIENCE, EDUCATION, SKILLS & KNOWLEDGE REQUIRED: § Bachelor's degree in social work, psychology, education, sociology or related field. Master's degree in related field preferable. § Required social work license and to belong to the association of social work professionals, preferred. § One (1) or more years of experience working with communities, students or adolescents. § Administrative - Knowledge of administrative and office procedures and systems such as word processing, managing files and records, stenography and transcription, designing forms, and workplace terminology. § Education and Training - Knowledge of principles and methods for curriculum and training design, teaching and instruction for individuals and groups, and the measurement of training effects. § English Language - Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar. § Customer and Personal Service - Knowledge of principles and processes for providing customer and personal services. Social Worker also directs the CLC Residents/Relatives Councils a means to identify feelings and concerns and improve their physical and emotional environment. Additional duties include but not limited to: * Will develop an assessment of the veteran in collaboration with the interdisciplinary treatment team, the veteran, family members and significant others. Demonstrates an understanding of the range of treatment and skills needed for all adult patients including geriatric patients related to their physical limitations, psychological needs and age associated illnesses and conditions. * Assessments are conducted according with the established time-frame in the CLC. A reassessment is required as required by changes in patient's condition. * Is responsible for formulating the treatment plan, and setting achievable goals with the veteran/family. * Coordinates Discharge Planning Process on assigned area and determines the most effective treatment modality utilizing a range of social work models and techniques. * Integrates these services with those of other professional personnel as member of the interdisciplinary team. * Assures Continuity of Care identifying the need of post-discharge follow- up. * Completes an electronic Coordination of Care Referral when social work follow-up is required in the patient's next level of care. * Performs individual supportive counseling with patients and families/significant others as necessary, requiring utilization of a variety of psychological and counseling theories and modalities. * Assistance is also provided to allied health personnel through consultation to the end that general patient care and therapeutic intervention is maximized. * Understands the intimidation of bureaucracy and will act as an advocate when it serves the best interest of the veteran/family. * When appropriate and feasible, social worker will educate and encourage the veteran/family to advocate on own behalf, thus fostering a sense of independence and empowerment. * Provides education related to VA and community resources, entitlements, Advance Directives/Living Will and will refer veterans/families to the appropriate interdisciplinary team member for identified health education needs. * Will review the progress notes from the other providers to accurately determine the strengths and limitations of each veteran being referred for social work intervention. * Educate the veteran, their families and the team of all the options available to them and will collaborate with the veteran and family on the preferred option. * Will enter all veteran/family contacts in the electronic record using appropriate formats and templates. * Establish and maintain positive working relationships with employees, volunteers, consumers and stakeholders within the VA and outside community agencies. * Will attend all appropriate staff meetings (interdisciplinary treatment team meetings, Social Work staff meetings, other committees as appropriate, etc.) and perform other duties as required. Work Schedule: Monday through Friday, 7:30 am to 4:00 pm, with rotating tours of duty as needed. The primary age of patients served in the BHIP is normally between the ages of 18-65 or older, much of whom are men.
