Starting Pay Range: $32 - 34/hr.
Responsibilities
Provide and ensure compliance within the Early Head Start Home Base services.
Develop, maintain and track all Individual Family Service Plans (IFSP) and Individual Education Plans (IEP), children's progress reports and all record-keeping documentation in cooperation with Head Start Performance Standards.
Work collaboratively with staff and parents about issues related to atypical behavior or development and the individual needs of children.
Conduct on site behavioral screenings on children within 45 days of enrollment.
Identify resources in the community and secure Memorandums of Understanding (MOU) with each agency.
Assist in the development and presentation of staff in-service trainings regarding early intervention, and other related topics as directed.
Attend IFSP/IEP meetings as directed.
Perform other assignments as needed based on associate's expertise, knowledge, ability or needs of the program.
Qualifications
Knowledge of policies and procedures of the California Department of Education, Child Development Division and the Department of Social Services, Community Care Licensing program regulations, funding terms and conditions (Title 5 and Title 22).
Bachelor's degree or higher in related field of Psychology, Child Development, or Special Education.
Must have successfully completed an Infant toddler class from an accredited college with a B grade or better.
Ability to communicate effectively, through oral and written skills, and work cooperatively with a variety of individuals and groups. Must relate well to children and their families.
Exercise sound judgment, establish and meet timelines, plan. Coordinate and implement school site programs.
Possession of pediatric (infant/toddler) CPR and first aid certificate.
Knowledge of Microsoft Office and other related computer software.
Ability to stand for extended periods. Ability to reach, bend, stoop squat. Also possess fine manual dexterity, can push, pull, talk/hear, with near and far visual acuity/depth perception/color vision/field of vision.
Pass FBI, DOJ and Criminal Background Check

ABOUT OUR PROGRAMS
Camino A Casa: this private insurance-based residential treatment program serving youth ages 12-17 overcome a wide range of mental health concerns through individual and group therapy, family therapy, and medication management. Camino a Casa also offers both a partial hospitalization program (PHP) and Intensive Outpatient Program (IOP)
Short Term Residential Therapeutic Program (STRTP): this publicly funded residential program aims to provide a trauma-informed mental health treatment to address mental health symptoms that interfere with youth's functioning, as well as develop social, emotional, and self-regulatory skills that will increase safe behaviors and enhance emotional stability and resilience.
Transitional Youth Services: this program supports emancipated young adults ages 18-25 by providing them with housing and intensive case management services that assist them in becoming self-sufficient, independent, and well-adjusted adults in their communities
Community-Based Services: this program offers intensive home-based therapy and case management services with the goal of helping both privately funded and system involved youth, and their families maintain a healthy family and/or placement environment.
Non-Public School (NPS): our onsite WASC-accredited school serves students in grades K-12 who have needs that fall outside what a traditional classroom setting can provide. Our NPS specifically addresses students' IEP goals and provides a comprehensive and supportive learning environment where students in both our residential and community programs can thrive.
KEY RESPONSIBILITIES
Communicates critical information about each youth assigned to the treatment team. Communicates daily with members of the treatment team about the youth's progress.
Assists in developing daily and weekly goals for the assigned youth.
Provide behavioral coaching to the youth; individually and in group sessions, to assist in identifying/dealing with difficult feelings, and thoughts that result in maladaptive behaviors as well as teaching appropriate replacement behaviors.
Ensure effective crisis prevention and intervention through input and promoting awareness of safety plans and de-escalation strategies.
Communicate changes in or achievement of behavioral goals. Integrate the youth's treatment plan and clinician's recommendations when working with the youth.
Assures that safety plans are in place and that they are understood by cottage staff.
Provides required trauma-informed care, supervision, and treatment in accordance with milieu program and individual treatment plans. Facilitates youth in development of social skills, personal competence, individual responsibility, and self-esteem; provides positive reimbursement experiences.
Participates in individual youth therapy, family meetings and planning conferences.
Provides transportation as needed
Engages and participates in group recreation activities and events. Develops and assists in leading group activities, experiential outings, and activities of daily living.
Attends and participates in cottage/program and general staff training programs; attends out side training events and conferences as requested.
Cares for clothing, shelter, and other basic needs of assigned youth in a manner that contributes to achievement of treatment goals.
Provide crisis management and intervention and participates in Therapeutic Crises Intervention (TCI)-approved de-escalation of youth, as necessary.
Maintains confidentiality in all matters within the organization with respect to personnel, operations and clients.
Plans, organizes and facilitates MHW ran groups and on a variety of topics.
May perform other duties as assigned from time to time.
Regularly conducts 15-minute checks to ensure proper safety of youth when working overnight shift and as needed for youth on High Risk “HR” Status.
Participates in weekly group and/or individual supervision meetings with supervisor.
ABOUT THE ROLE

The staff provides screening, assessment, brief intervention, care coordination, and referral for patients with behavioral, developmental, or psychosocial needs. The BHI staff is a core member of the collaborative care team. As a member of this team, you will work closely with primary care providers (PCPs), medical staff, psychiatric Interventionists, and other behavioral health professionals at Community Memorial Health to deliver comprehensive care that integrates mental and physical health. This position is based in Community Memorial Healthcare's outpatient primary care clinics and does not include hospital or inpatient responsibilities.

Discharge Planning: On admission, participates in the coordination and development of a discharge plan. In conjunction with the healthcare team, patient and family stakeholders identifies likely post-discharge needs. Provides appropriate information, education and choice, making referrals as appropriate to implement the discharge plan (i.e. charity applications, Family Assistance Fund, community resources, home healthcare, institutional placements, financial assistance, DME equipment needs and/or alternative/specialized care settings). Resource linkage for uninsured and/or underinsured patients, displaced families, immigrants and refugees who need medication and aftercare placements in shelters/transitional housing, sober living, SNF, substance abuse residential programs, etc. Use multidisciplinary collaboration, hospital programs and resources, community resources, home care services, institutional placements, financial assistance and mental health counseling to plan and coordinate patient apos;s discharge as needed. Facilitates the transfer of patients to other care facilities as appropriate. Intervene with behaviorally challenged patients and/or patients unwilling to leave hospital to facilitate a safe/timely discharge plan. Maintain timely and accurate records of all activities relating to discharge planning, documenting activities as appropriate in the medical record. (30%)
Resource Coordination: Ascertain healthcare benefits utilizing both internal and external resources as appropriate to coordinate necessary and appropriate care, while appropriately preserving the healthcare benefits of the beneficiary. Link uninsured/underinsured patients to available medical insurance coverage or charity resources. Identifies and coordinates appropriate resources to address basic needs of patient and family i.e (meal coupons, breast pumps, car seats, transportation vouchers, clothing, housing, medical supplies, social services, humanitarian visas and charity services, etc). Identify and refer situations requiring immediate intervention to the attending physician, risk management, department director and/or physician advisor as appropriate. Quality Assurance: Participates in the improvement of organizational performance through quality assurance and performance improvement activities. Performs biopsychosocial and functional assessments for assigned clients

Reassesses clients throughout their stay to determine the clients' response to care and when there are significant changes in the clients' condition

Requirements:
CADC, SUDCC, CATC certification or equivalent required
