Position Description:

The candidate must have strong interpersonal, problem solving, decision-making and time management skills and the preferred candidate possesses knowledge of pediatric diagnoses and diseases that impact children's development and activities of daily living and has experience with pediatric home health care and related medical supports/supplies and has knowledge of Medicaid and Medicaid services.
The ideal candidate understands Children Services including medical and behavioral health conditions regarding the various children's populations of Serious Emotional Disturbance, Foster Care, Medically Fragile, Developmental Disabilities, and Substance Use and other chronic conditions.
·Become knowledgeable and develop an understanding of the Children's Health Home program, Home and Community Based Services (HCBS) Children's Waiver, Children and Family Treatment and Support Services, and Foster Care/29-I services to determine changes and or enhancements to the services and programs.
·Assist the supervisor and staff in the Office of Health Insurance Programs (OHIP), Division of Program Development and Management (DPDM) in a broad range of planning, implementation, evaluation, and reporting activities.
·Conduct HCBS eligibility determination assessments for Waiver applicants and enroll eligible participants. This includes managing referrals, providing assistance and program education, conducting initial screenings, gathering assessment information from the applicant, and providers, determining eligibility, enrolling participants and transferring participants to their choice of Children's Health Home.
·Monitor and analyze reports and feedback to provide updates on HCBS eligibility and enrollment status to leadership

SUMMARY
The Social Worker provides critical support to individuals and families navigating illness and disabilities. This role helps resolve personal, financial, and environmental challenges while working in accordance with ADAPT Community Network (ADAPT) policies, regulatory requirements, and standards of the profession. The Social Worker empowers families to understand and negotiate educational and clinical systems, advocating for their needs. Additionally, this position fosters collaboration with other disciplines to ensure a comprehensive continuum of care and support.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The duties and responsibilities of the Social Worker will include, but are not limited to the following:
• Participates in the Adapt Intake Processes
• Develops case notes, case summaries / social histories, etc. for each student on his/her caseload including appropriate recommendations.
• Assists families in obtaining services such as housing and residential placement, food stamps, Medicaid, Assistive Technology services; respite services; family reimbursement services; camp, vacation referrals, etc.
• Integrates therapeutic intervention techniques into school programs or home of students served through consultation with parent/guardian and staff at meetings, case conferences or through other informal contacts.
• Participates in interdisciplinary team meetings and annual reviews for purposes of planning, implementation, and monitoring of IEP's.
• Provide emotional and behavioral support to students, particularly those with disabilities.
• Lead the school's efforts to ensure students receive necessary social-emotional resources or
• Lead the school's efforts to provide students with the emotional support needed for academic, social, and emotional success.
• Facilitates communication with family and/or caregivers concerning issues such as attendance, transportation, medical status, and clearance, etc.

• Responding promptly to family concerns and providing guidance on available resources.
• Documents status of all active cases.

OVERVIEW
The Social Worker will be supporting the Home Study (HS) and Post Release Services (PRS) department. This position is remote, located in New York/New Jersey and requires up to 50% travel.
Starting pay is $72,134.40 and may be subject to change on experience, education, and cost of living.
DUTIES AND RESPONSIBILITIES
Provide culturally and linguistically appropriate clinical case management services to UC with significant needs.
Carry a caseload of 5 to 10 EPRS cases.
Conduct brief clinical interventions to help families address crises and manage conflict, as needed.
Complete individualized needs assessments with unaccompanied children (UC) and sponsors, and develop case plans with particular attention to culture, language, and special circumstances.
Maintain a high standard of record-keeping according to USCRI and ORR policies and procedures.
Provide information, education, referrals, outreach, advocacy, and support to ensure that each child receives the services they require.
Assess the safety and appropriateness of the placement on an ongoing basis using child welfare principles and social work best practices through phone/video calls, interviews, and home visits.
Build a network of local pro bono legal resources and service providers to facilitate access for unaccompanied children (UC) and sponsors, and track individual progress.
Coordinate and maintain communication with key stakeholders.
Participate in regular training and communication sessions with supervisor and headquarters staff.
Other duties as assigned by supervisor.

OVERVIEW
The Social Worker will be supporting the Home Study (HS) and Post Release Services (PRS) department. This position is remote, located in New York/New Jersey and requires up to 50% travel.
Starting pay is $72,134.40 and may be subject to change on experience, education, and cost of living.
DUTIES AND RESPONSIBILITIES
Provide culturally and linguistically appropriate clinical case management services to UC with significant needs.
Carry a caseload of 5 to 10 EPRS cases.
Conduct brief clinical interventions to help families address crises and manage conflict, as needed.
Complete individualized needs assessments with unaccompanied children (UC) and sponsors, and develop case plans with particular attention to culture, language, and special circumstances.
Maintain a high standard of record-keeping according to USCRI and ORR policies and procedures.
Provide information, education, referrals, outreach, advocacy, and support to ensure that each child receives the services they require.
Assess the safety and appropriateness of the placement on an ongoing basis using child welfare principles and social work best practices through phone/video calls, interviews, and home visits.
Build a network of local pro bono legal resources and service providers to facilitate access for unaccompanied children (UC) and sponsors, and track individual progress.
Coordinate and maintain communication with key stakeholders.
Participate in regular training and communication sessions with supervisor and headquarters staff.

REQUIREMENTS

OVERVIEW
The Social Worker will be supporting the Home Study (HS) and Post Release Services (PRS) department. This position is remote, located in New York/New Jersey and requires up to 50% travel.
Starting pay is $72,134.40 and may be subject to change on experience, education, and cost of living.
DUTIES AND RESPONSIBILITIES
Provide culturally and linguistically appropriate clinical case management services to UC with significant needs.
Carry a caseload of 5 to 10 EPRS cases.
Conduct brief clinical interventions to help families address crises and manage conflict, as needed.
Complete individualized needs assessments with unaccompanied children (UC) and sponsors, and develop case plans with particular attention to culture, language, and special circumstances.
Maintain a high standard of record-keeping according to USCRI and ORR policies and procedures.
Provide information, education, referrals, outreach, advocacy, and support to ensure that each child receives the services they require.
Assess the safety and appropriateness of the placement on an ongoing basis using child welfare principles and social work best practices through phone/video calls, interviews, and home visits.
Build a network of local pro bono legal resources and service providers to facilitate access for unaccompanied children (UC) and sponsors, and track individual progress.
Coordinate and maintain communication with key stakeholders.
Participate in regular training and communication sessions with supervisor and headquarters staff.
Other duties as assigned by supervisor.

REQUIREMENTS
Master's degree in social work, psychology, sociology, or other relevant behavioral science; with at least three years' progressive experience, graduate level licensure (LMSW/LGSW) is preferred; or a Bachelor's degree with at least five years of clinical experience.
Candidate must have at least 1 year of direct clinical experience.
Experience in child welfare, child placements, family preservation, behavioral health counseling, and social service provision is preferred.
Knowledge of trauma-responsive service provision and crisis intervention skills.
Must be skilled at writing professional, thorough assessment reports.
Must possess an extensive knowledge of immigrant community local resources.
Excellent organizational, verbal, written, and interpersonal communication skills, and ability to prioritize duties in a fast-paced environment.
Must have a valid driver's license, insurance, a clean driving record, and personal transportation.
Position requires up to 50 percent long distance travel with limited notice.

Position Overview
The Social Worker is responsible for identifying and assisting students, families, and school systems in overcoming barriers that interfere with learning through assessment, counseling, consultation, and school and community resources coordination.

Mental Health Counselor
DEPARTMENT/DIVISION: MEDICINE
DIVISION OF HIV MEDICINE (DHIVM)
REPORTS TO: Manager of Client Services
QUALIFICATIONS:
Master's degree and licensure in Social Work, Counseling/Psychology, or related field required. One to three years documented experience in the provision of case management and/or counseling services to medically compromised patient population, familiarity with DSM 5, community mental health resources and ability to work in a fast-paced, interdisciplinary team setting required.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
* Mental Health Counselor will receive and manage all referrals of (MH & SA) from providers, case managers, nursing staff, self-referrals
* Caseload of 20 patients for counseling services
Provide screenings, intakes, and brief mental health (MH) needs assessments and (SB) substance abuse for all patients referred by members of the DHIVM healthcare team. Formal cognitive assessments may also be needed.
Based on needs assessment and availability of services, develop mental health and Substance Abuse referral plan and initiate referral services rather in house or in the community.
Complete referral paperwork, including consents for release of HIV, MH and SA service providers, both internal and external.
Contact MH &SB service providers, as needed, to facilitate referral processes.
Follow up referrals through EPIC portal, patients, other healthcare team members, and/or mental health/substance abuse providers to ensure transition to MH/SA services.
Develop, implement, and update service plans for patients receiving MH/SA care coordination services.
Obtain and maintain consents/ ROIs for HIV and Mental Health/Substance Abuse information for the behavioral health team.
Contact external MH/SA providers annually and as needed to update internal records, exchange information, and coordinate care.
Assist with removing barriers and facilitating access to mental health/substance abuse care.
Provide crisis intervention and motivational interviewing skills, including de-escalation techniques, to patients in emotional crisis.
Serve as consultant to staff regarding communication, boundaries, and difficult emotional situations with patients.
Assist Behavioral Health Program staff with development and maintenance of mental health program information tools (brochures, resource lists, bulletin boards, etc)
Provide re-engagement services/ care coordination follow up to patients who have fallen out of mental health/ substance abuse in house counseling services care, both internally and externally.
Monitor Mental Health Counselor caseload, ensuring engagement efforts for patients who have lapsed in care, and program closure as needed.
Participate in routine/weekly care coordination and case conference meetings and other meetings as warranted.
Manage mental health/substance abuse program waiting list for counseling or psychiatric services by carrying a small caseload.
Facilitate support and educational groups for patients day and evening providing back up for primary counselors.
Participate in quality improvement activities within the Behavioral Health Program and other Division of HIV Medicine Programs.
Provide education to patients/families to help them cope with illness and promote wellness as they navigate through mental health services.
Participation in mental health/substance abuse provider community meetings
Utilize and continuously update Excel spreadsheet and maintain data to monitor referral status, program enrollment, program closure, and other data regarding patients receiving mental health and substance abuse services.
Carry out responsibilities in accordance with AMC and DHIVM policies as well as NYS AIDS Institute MH standards of Care and CASAC.
