About the Role
As a Behavioral Health Specialist (LCSW), you will support older adults enrolled in a comprehensive healthy aging and wellness program. This full-time, community-based role blends clinical social work, behavioral health counseling, and care coordination to help participants age safely and independently while managing complex medical, behavioral, and social needs.
Key Responsibilities

This is an exempt temporary/part-time position that reports to the Director for the Center for Families.

* Identify economic, cultural, physical, and environmental factors which support or limit a child's functioning

* Evaluate developmental and behavioral screenings and make referrals where needed

* Monitor in-kind documentation for children who are receiving special needs services with outside agencies

* Track and review Individual Education Plans (IEP) and Individual Family Service Plans (IFSP) for children and ensure that staff are developing activities and goals around these plans
* Develop community resources around Mental Health Agencies in community meet with staff when approved to discuss Mental Health or Disability issues
* Perform other duties as assigned or delegated by the Director for the Center for Families.
REQUIRED QUALIFICATIONS:
* Bachelor s degree in mental health for children, special education OR related field.
* Two years of work experience in mental health or special education early childhood setting
DESIRED QUALIFICATIONS:

PHYSICAL WORK ENVIRONMENT:
Work is typically performed in an office setting. The ability to sit at a desk/computer station, bend, stoop, walk and generally move around the office to consult with others, work within a busy environment and remain focused on tasks. Inventory management requires some lifting and moving of objects up to 30lbs. Excellent communication skills are essential. COMPENSATION:

* Provide intake, longitudinal care management, and Social Work support to identified patients including behavioral health treatment planning, crisis intervention, transition of care support, resource navigation, community resource procurement, care coordination, emotional and short-term behavioral health support. Evaluate acuity of needs and assist patients in overcoming barriers to optimal health and wellness, promoting graduation from care coordination when appropriate and maintaining appropriate patient panel size.
* Provide advanced assessment and coordination of care for patients with complex behavioral health conditions or significant social challenges. Act as team and organizational resource providing Social Work expertise and perspective. Assist Supervisor with training and onboarding of peers, including mentoring and precepting.
* Actively maintain engaged patient panel utilizing proactive person-centered techniques and approaches such as critical thinking, motivational interviewing, case finding, SMART goal setting, health coaching, patient-empowerment, relationship-building and proactive independent collaboration. * Utilize the biopsychosocial perspective to administer and interpret screenings and assessments. Complete any payer contract requirements including verification of patient eligibility, coordination of appointments, attending required trainings, administering and documenting screenings within required timeframes.

Job Duties
STATEMENT OF ESSENTIAL JOB DUTIES
* Conducts investigations and evaluations of individuals for involuntary commitment and detains individuals when legal criteria are met and commitment is appropriate.
* Assesses behavioral health emergencies and provides crisis intervention for individuals experiencing a behavioral health crisis in the community.
* Works collaboratively with behavioral health agencies, psychiatrists, emergency departments, inpatient units, residential facilities and other referral sources to assure the best outcome for each individual in need of behavioral health services.
* Coordinates and facilitates arrangements necessary for the holding of probable cause hearings for patients hospitalized in our county in need of continued involuntary treatment; provides court testimony concerning patient's need for further involuntary treatment.
* Performs ongoing community regarding involuntary treatment.
* Contacts appropriate hospitals for admissions, provides screening information and arranges for transport as needed.
* Participates in the ITA Court process and provides relevant information to prosecuting attorneys and others authorized through the judicial process.
* Maintains necessary records and files as well as statistical information on all program activities.
* Performs related duties as required.
Minimum Qualifications
A master's degree in social work, clinical psychology, psychiatric nursing or related field; PLUS, two (2) years of experience in the direct treatment of mentally ill clients; OR, any equivalent combination of training and/or experience which provides the required knowledge and abilities. PLUS, a valid and current license as a Licensed Mental Health Counselor, Licensed SocialWorker or Licensed Marriage and Family Therapist. Must pass job related tests.

Responsibilities
What You'll Do:
The Behavioral Health Specialist (BHS) manages and maintains the safety of the individuals and staff. Takes vitals and assists individuals in meeting their needs while on the unit by providing exceptional customer service. Documents services provided and significant interactions in the Electronic Health Record (EHR).
* Conducts security screening and inventories property of individuals upon admission to the facility. Provides orientation to the Observation Unit and Short-term Inpatient Unit. Educates individuals on their rights under Washington State regulations, grievance process, provides expectations, and explains rules and regulations.
* Maintains safety of the unit.
* Intervenes with escalated individuals using therapeutic techniques for de-escalation and performs restraints or seclusions (if applicable) when needed to ensure the individual's safety and the safety of others. POSITION SUMMARY:
Holt International finds and supports permanent, loving families for children who are orphaned, abandoned or at serious risk of separation from their family. The master's level socialworker represents Holt in their community and ensures the successful adjustment of child and family by conducting an assessment of the family, and providing appropriate services throughout the adoption process. The master's level socialworker works cooperatively with families, the Branch Office and Adoption Teams headquartered in Eugene, Oregon. Note: The Adoption SocialWorker is primarily a home-based position. The Adoption SocialWorker position requires travel (most often by car) to different locations within the assigned area to complete the essential functions of the position.
ESSENTIAL JOB FUNCTIONS (to be completed within timeframes outlined by the Branch Director):
1. Determines a family's ability and capacity to parent an adopted child by conducting pre-home study meetings as requested, reviewing and assessing agency-requested information, evaluating the family's knowledge and perceptions of adoption, teaching adoption education courses as needed, and conducting psycho-social assessments of family members.
2. Contributes to the delivery of high-quality adoption services by producing adoption home studies and post placement reports that comply with provided guidelines and meet current agency, state, country and USCIS requirements.
3. Provides support and case management of all assigned families by maintaining quarterly contacts throughout process, communicating information to the family, referring family to appropriate community support systems when necessary and documenting all correspondence in Holt's database.
4. Prepares family for travel and assists with post placement adjustment by providing training and identifying resources available to help meet the child's needs and aid in the child's adjustment to the family. Conduct home visits as required by state law and country program guidelines, assist in early identification of problematic issues, and make a determination regarding the stability and appropriateness of the placement.
5. Assures child's physical and emotional well-being and eases family's adjustment by providing counseling to the child and family and/or helping arrange for counseling with a clinician outside the agency if needed. In case of an adoption disruption or dissolution, maintain relationship with family to ensure transition plan is developed that meets the child's needs. Assist child and family with transition of child to new placement.
6. Contributes to public knowledge about Holt and supports Holt programs by participating in activities arranged by Holt including picnics or parent groups, representing Holt at adoption meetings, coalitions and organizations, and performing other outreach activities as appropriate.
7. Maintains professional knowledge by attending relevant seminars and course work, keeping informed about State regulations, legislation, adoption issues and child-related issues overseas; maintaining license, if required.
