This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
The duties listed are GS-11 Full Performance Level, GS-9 duties will differ in complexity and level of supervision.
This Social Worker is responsible for program oversight, monitoring and administration, serving as a contact with local Public Housing Authorities, and providing clinical case management services necessary to ensure that Veteran care is coordinated with the VA medical center, community health, or mental health providers, and agencies that serve homeless populations. This position is the primary point of contact for Veterans who have graduated from HUD-VASH case management and have retained their voucher.
Clinical Assessment
* Evaluate each Veteran's situation, abilities, and capabilities, and arrive at a reasoned conclusion including an assessment of vulnerability and priority for re-admission.
* As part of a comprehensive psychosocial assessment, interview the Veteran and their family members to establish facts about the Veteran's situation, presenting problems, their causes, and the impact of such problems on the Veterans functioning and health.
* Review all data, subjective and objective, and make a clinical assessment, identifying needs and strengths.
Comprehensive Case Management
* Ensure graduated Veterans maintain their housing stability by following up at 3, 6, 9, and 12 months following graduation from HUD-VASH Case Management. Contact graduated Veterans annually thereafter at the point of recertification to offer assistance with this process.
* Implement the Housing First model within the HUD-VASH program. The main principle is direct, or nearly direct, placement of targeted homeless Veterans into permanent housing.
* Provide referrals to VA medical and mental health inpatient and outpatient treatment and to other VA services.
* If appropriate, provide referrals or consults, or in some cases direct admission of the Veteran prior to HUD-VASH participation, for other VA programs, including community-based residential treatment such as Grant and Per Diem Programs, Health Care for Homeless Veterans contract residential treatment, Domiciliary, or Psychiatric Residential Rehabilitation and Treatment Programs.
Program Administration and Organization
* Act as the primary POC for PHAs across the catchment area for annual reviews/recertification.
* Develop working relationships and agreements with other organizations and directly coordinates with the designated PHA regarding housing voucher recertification and award process for each Veteran.
* Veterans eligible for HUD-VASH come from a wide geographic area. Their health and mental health needs may be complex and require a high degree of clinical oversight and creative problem solving. The HUD-VASH Program requires continuous coordination with the community's homeless providers and coalitions as well as with other local, state, and federal agencies.
Outreach
* Conduct and participate in outreach activities including field interviews, assessments, and referrals for homeless Veterans contacted in the community, shelters, and "on the streets," as well as those who are referred by VA residential programs, medical centers, or outpatient clinics.
* Transport those Veterans to appointments in the community.
* Maintain current and establish new relationships with other community-based service providers for the homeless.
* Operate a government vehicle to travel in the service area and local community to perform duties. Some overnight travel to remote parts of the catchment area may be required.
* Regularly conduct and participate in outreach activities including field interviews, assessments, and referrals for homeless Veterans contacted in the community.

The Opportunity:
In this role, you will conduct investigations, perform assessments, deliver services, and conduct permanency planning for immediate family, extended family, kin, and children. Worker Core Training (WCT):
As a Social Service Specialist, you are required to successfully complete a mandatory 9.5-week Worker Core Training (WCT). This training provides the foundational knowledge and skills needed to perform core job functions, including assessing risk and safety, supporting child well-being, gathering information, and conducting case and permanency planning. During the training, you will have opportunities to practice these skills through classroom activities, simulation exercises, and debriefs. Upon hire, you will be assigned to the nearest available training cohort. The training includes four (4) weeks at our Seattle training site (scheduled for Weeks 2, 4, 6, and 8). For those eligible for travel from their assigned duty location, DCYF covers hotel accommodations and per diem costs.
The WCT curriculum is designed to prepare Social Service Specialists for the responsibilities of the role, including:
* Understanding the child welfare system
* Familiarity with the Revised Code of Washington (RCW) related to child welfare
* Policies, procedures, and best practices
* Assessing child safety through environmental observation, interviews with children and caregivers, and collateral contacts

The Forensic Housing and Recovery through Peer Services (FHARPS) Team uses the Permanent Supportive Housing and Housing First model to offer housing assistance to individuals who meet the definition of Trueblood Class Members. Participants are provided with meaningful choice and control of housing and support services. The team provides case management to participants with an emphasis on enhancing access to and retention in permanent supportive housing.

Landlord outreach and engagement
Employment services
Activities of daily living services
Social and interpersonal relationships and leisure time
Participants have access to short-term housing bridge subsidies to reduce barriers and increase access to housing options. The team links participants to long term providers including mental health and substance use treatment providers, foundational community supports, and other providers as appropriate.
The Mental Health Engagement Specialist for Trueblood operates as a member of a multi-disciplinary team to include Forensic Housing and recovery through Peer Services (FHARPS) and Forensic Projects for Assistance in Transition from Homelessness (FPATH).
The Mental Health Engagement Specialist provides behavioral health services to individuals to generate referrals and promote client engagement. They engage with community partners and local hospitals to assist in the referral process and reduce barriers to Trueblood services. They assist in screening referrals for minimum program criteria and provide crisis intervention and stabilization services in a crisis if needed. Assistance and support include, but is not limited to, engagement, supportive listening, psychoeducation, skill building, crisis assessment and intervention, resource education, community referrals, and short term follow up with individuals in office or community-based settings.
The Mental Health Engagement Specialist is a member of a multidisciplinary team which includes housing specialists, mental health professionals, and certified peer counselors who have lived experience in behavioral health recovery.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Co-outreach with current Trueblood staff to potential Trueblood enrollees in jails and the community to build rapport, explain Trueblood program services, and reduce barriers to enrollment.
2. Conduct regular outreach support including but not limited to the following:
Community-based outreach, jail and hospital-based in-reach, and proactive engagement to FHARPS referrals and enrolled participants;
Referral screening, coordination, and follow-up; and
Locate enrolled participants who are out of contact in order to re-engage in FHARPS services before being discharged for lack of engagement for sixty
(60) Calendar Days if they are still in the community and (ninety (90)
Calendar Days if the participant is re-incarcerated or re-hospitalized.).
3. Attend court with enrollees and, when requested, provide information about the contractor's program and resources.
4. Outreach enrollees as they begin the program and if they disengage from services, re-engage and encourage completion of program.
5. Collaborate with system partners to include FHARPS, FPATH, OCRP, and DSHS Forensic Navigators.
6. Provide any services for FHARPS or FPATH when time allows or as appropriate. Facilitate and co-facilitate client treatment groups.
7. Provide episodic, trauma-informed, person-centered counseling utilizing evidence-based practices within scope of practice.
8. Establish and maintain collaborative working relationships with; clinical, administrative and support staff, community partners, clients, and family/natural supports.
9. Conduct presentations on FHARPS and FPATH process and services to courts or other legal system partners.
10. Maintain client records and all other required documentation in a timely and confidential manner, in accordance with agency policies and procedures.
11. Conduct risk assessments, develop and implement safety plans, and provide crisis intervention and stabilization services as needed, including facilitating access to inpatient facilities for behavioral health care.
12. Assist clients with developing and implementing safety plans and facilitating access to inpatient facilities for behavioral health care.
13. Participate in required staff meetings, including regular supervision and weekly clinical staff meetings.

Full-time Description
SUMMARY: This position is responsible for providing quality intake and referral services, group and individual counseling, case management, and transfer/discharge services
DUTIES AND RESPONSIBILITIES:
Coordinate seamless intake, referral, and transfer/discharge processes between STARS programs, and with outside agencies and referral sources.
Collaborate with multidisciplinary team, community professionals, and community agencies to ensure continuity of care.
Perform other duties as assigned or within program scope including individual and group treatment services and development of service plans for STARS Outpatient Precontemplation track.
Maintain accurate and timely electronic health records of all services as determined by WA State WAC and as outlined in STARS organizational policies and procedures.
Work to uphold organizational values and beliefs, using evidenced-based, person-centered practices when delivering services to program participants.
Performs other related duties as assigned by management.
SUPERVISORY RESPONSIBILITIES:
This job has no supervisory responsibilities.
QUALIFICATIONS:
Master's Degree (MA) or equivalent, or four to ten years of related experience and/or training, or equivalent combination of education and experience.
Certificates, licenses and registrations required: LMHC, LMHCA, LICSW, or LICSWA; SUDP/T (preferred), CPR/First Aid
Computer skills required: Microsoft Office Suite; EHR software.
Other skills required:
Knowledge of and experience with case management, service referral, and available community behavioral health resources.
Experienced SUD counselor for full range of SUD treatment services.
Knowledge and experience with ASAM LOC and Decision-making rules
Knowledge of HIPPA and 42-CFR
