Social Worker II Per Diem Days Position Summary
The Social Worker is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient's resources and right to self-determination. The individual in this position has overall responsibility for to assess the patient for transition needs including identifying and assessing patients at risk for readmission. Conducts complex psycho-social assessment and intervention to promote timely throughput, safe discharge and prevent avoidable readmissions. This position integrates national standards for case management scope of services including:
• Transition Management promoting appropriate length of stay, readmission prevention and patient satisfaction
• Care Coordination by demonstrating throughput efficiency while assuring care is the right sequence and at appropriate level of care
• Compliance with state and federal regulatory requirements, TJC accreditation standards and Tenet policy
• Education provided to physicians, patients, families and caregivers
• Leads a population of patients by service line and/or leads the team by being a resource to Tenet performance standards.
Responsibilities
This individual's responsibility will include the following activities:
a) complex psycho-social transition planning assessment and reassessment and intervention,
b) assistance with adoptions, abuse and neglect cases, including assessment, intervention and referral as appropriate to local, state and /or federal agencies,
c) care coordination,

The Organization: The Florence Project was founded in 1989 to respond to a representation crisis for indigent non-citizens detained at the rural Immigration and Customs Enforcement (ICE) Detention Center in Florence, Arizona. Through our programs today, we provide services to thousands of adults, youth, and children detained in Eloy, Florence, Phoenix, and Tucson, Arizona on any given day. In 2001, the Florence Project implemented our Integrated Social Services Program, one of the first of its kind in this field. The Florence Project provides high quality legal and social services and supports initiatives for national changes in immigration law and policy.
Social Worker:
The Children's Program Social Worker, in collaboration with a team comprised of Social Workers, Managing Social Worker, immigration Attorneys, and Legal Assistants, serves immigrant children who have been released from ORR/Federal care to Arizona. The position has extensive client contact and provides case management, advocacy, and administrative duties to support and link children, including indigenous children and children who have suffered abuse, abandonment, neglect, or other emotional and physical trauma with proper services in the greater Phoenix area.
Essential Duties & Responsibilities:
Casework:
Create and maintain a case plan with clear and concise case file documentation.
In collaboration with legal staff, create and implement appropriate case plans and case management for the client's most pressing social needs.
Facilitate client referrals to non-legal services, including housing, medical, mental health, vocational, and educational services.
Act as a liaison between clients and staff attorneys, including assisting clients in scheduling meetings with attorneys and other legal case-related appointments, understand the legal processes, and gather factual information from clients for legal cases, as needed.

The Social Worker is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient's resources and right to self-determination. Assesses patient/family knowledge level, implements plan of instructions, and records the patient's family's demonstrated level of understanding.
Can initiate life saving measures in the absence of Physicians.
Assures the content and correctness of a prepared report and signs the document to validate its content.

Aptive Resources seeks qualified and compassionate Licensed Social Workers to join our team and provide critical services at the Southern Arizona VA Health Care System (SAVAHCS) in Tucson, AZ. This is an opportunity to serve our nation's Veterans by delivering high-quality social work support in a collaborative health care environment.
Work Schedule
Hours: Full-time, 40 hours per week, with schedules determined by staffing needs.
Shifts: Rotating shifts, including Federal holidays, as required by SAVAHCS.
Primary Responsibilities Responsibilities
As a Licensed Social Worker, you will:
Conduct comprehensive biopsychosocial assessments.
Develop and implement plans to address social determinants of health.
Perform suicide risk, mental health, and crisis assessments and interventions.
Document safety plans and care provided in patient medical records.
Educate patients on and assist with completing advanced directives.
Coordinate and complete the process for inpatient psychiatric petitions.
Collaborate with interdisciplinary teams, communicating critical patient information.
Plan and facilitate family and interdisciplinary team meetings, including goals-of-care discussions.
Provide resources for homeless Veterans and address abuse or neglect findings in coordination with Adult Protective Services.
Develop discharge plans, including referrals to rehabilitation, long-term care, or palliative care facilities.
Assist with discharge planning for Veterans returning home, including home health consults.
Manage case assignments for post-discharge case management services.
Educate patients and families, ensuring they understand care instructions.
Support emergency measures and provide life-saving interventions when necessary.
Maintain professional decorum, respect patient rights, and uphold the VA's customer service standards.
Review and utilize electronic medical records to inform service delivery.
Minimum Qualifications Qualifications
Education: Master of Science in Social Work (MSW) from a nationally accredited school.
Experience: Minimum of two years of current social work experience.
Licensure: Current, full, and unrestricted social work license in any U.S. State, Territory, or the District of Columbia.
Certification: Basic Life Support (BLS) certified by an American Heart Association (AHA)-approved vendor

Qualifications
An Average Day:
(Includes, but not limited to)
Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group
Provide an assessment in the patient's identified residence and assistance when this is not safe, and another plan is required
Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect or abuse and plan intervention based on evaluation findings
Counsel patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties; provide crisis intervention when necessary
Assess for, and educate interdisciplinary group, on any special needs related to the culture of the patient and family, including communication, role of family, space, and any special traditions or taboos
Maintain clinical records on all patients referred to social work
Educate patients and families on, and assist in, preparation of advanced directives
Provide information and referral services for organization patients and families/caregivers regarding practical and environmental needs
Provide information to patients and families/caregivers and community agencies
Serve as liaison between patients and families/caregivers and community agencies
Maintain collaborative relationships with organization personnel to support patient care
Maintain and develop contacts with public and private agencies as resources for patient and personnel
Participate in the development of the individualized plan of care, involving the patient and family, and attend regularly scheduled interdisciplinary group meetings, assist the team in recognizing the effects of the psychosocial stresses on symptoms of the terminal illness
Assist physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues
Actively participate in quality assessment performance improvement teams and activities
Assist family and patient in planning for funeral arrangements, financial, legal, and health care decision responsibilities
Perform other duties as delegated by the Clinical Supervisor
Requirements:
Must have a master's degree in social work from an accredited university.
If you hold an undergraduate B.A, Degree in Social Work, you are eligible to apply. Please know the hirable rate of pay will be based on degree type.
Must have minimum of two (2) years of documented supervised experience in health care, hospice experience preferred
Must understand hospice philosophy, and issues of death/dying.
Certified Hospice and Palliative Social Worker is desirable
Must be flexible in work hours and have the ability to travel throughout the assigned Bristol Hospice service area
Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
