    JOB PURPOSE: The case manager is responsible for using sound professional judgment and best practices to ensure quality case management services are provided to the service population especially in crisis situations while maintaining compliance with established state and federal standards. Monitors assigned caseload of clients participating in a short-term life management program; establishes program plan/goals and evaluates client's progress by conducting routine meetings with client and/or staff; serves as advocate for client in order to acquire services that will enable them to functionally cope with their environment. The Case Manager is responsible for intake, coordinated assessment, individualize housing plan, and making appropriate community referrals for additional services. The Case Manager maintains appropriate contact with the client and ensure quality services is provided and that the individualize housing plan is completed timely Qualifications ESSENTIAL JOB RESPONSIBILITIES: Coordinate referrals, service planning and documentation of services for assigned caseload. Interviews applicants to determine eligibility for program enrollment based on established guidelines and requirements; conduct needs assessment, obtains pertinent information and establish program goals for eligible clients; refers applicants to other agencies if not appropriate for program. Develops client intake and case plans for clients; instructs clients in completing necessary paperwork including goals and/or budget forms; completes orientation check-off form and obtains release of information forms when appropriate; completes social history on new clients each week. Meets regularly with clients to discuss and evaluate their progress; prepares accurate and up-to-date records documenting the same; monitors client behavior, group attendance, medication and movement; receives incident reports and enforces disciplinary actions for infractions of the house guidelines. Plans, coordinates, and/or facilitates life management classes to assist clients in obtaining skills thatwill enable them to functionally cope with their environment; obtains speakers and workshop presenters as needed. Consistently serves as a mentor and provides positive examples of proper hygiene, personal appearance, records retention, budget planning, scheduling, etc. Refers clients to appropriate programs or other agencies based on clients' needs and in accordance with their program plan. Completes discharge plans of clients exiting the program; transfers file contents to non-active file. Meet all deadlines required by program supervisor and partners. Meet all federal and state regulatory guidelines and standards that are applicable to this position. Maintain a caseload in accordance with agency policies and procedures contract standards. Participate in workshops, seminars, education programs and other activities that promote professional growth and development. Maintain a minimum number of weekly contacts with clients. Engage and involve client in the casework process. Continually assess ongoing changes in behavior, circumstances or conditions that may affect client safety. Compile, prepare, submit and maintain accurate records, files, forms, statistics and additional information in accordance with agency policies, licensing and/or contract requirements. Participate in weekly case staffing with case managers. Maintain confidentiality in all areas of the service population and program operations. Demonstrate Exceptional customer service, in Everything you do, by placing the child, family, Veteran or client first to support our mission to "Empower people to build better lives for themselves, their families, and their communities." EDUCATION: Bachelor's in Social Work, Psychology, Criminal Justice, Sociology or related field of study. EXPERIENCE: 1+ year related experience and/or training including working in Crisis intervention. Proficiency in Spanish/English (written and spoken language skills), required. ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion. LICENSES: Driver's License with a clear record. This mission is vital to keeping existing satellites safe and to provide critical information to organizations launching new satellites in the future. One of the assets USSF uses to accomplish this mission is the Ground-based Electro Optical Deep Space Surveillance (GEODSS) System.This optical sensor is employed at three (3) different locations around the world (White Sands Missile Range, New Mexico; Diego Garcia, British Indian Ocean Territory; and Maui, Hawaii) and helps track the objects in deep space orbit (altitude of 10,000 to 45,000 km). In this role, you will: + Perform computer security assessments and implement required countermeasures; implement and maintain a site mission system and network Cybersecurity program. + Execute cybersecurity responsibilities protecting and defending information systems by ensuring the availability, integrity, confidentiality, authentication, and non-repudiation of data through the application of cybersecurity measures. + Protect ISs, their operating system, peripherals (media and devices), applications, and the information it contains against loss, misuse, unauthorized access, or modification. + Ensure all users and ISSOs receive COMPUSEC training. + Provide information to update system Assessment and Authorization (A&A) packages. + Review TEMPEST packages and provide documentation for TEMPEST package updates. To be successful in this role, you will have: + An active DoD Secret clearance. + An active DoD IAT Level II compliant certification + A high school diploma/GED + 2+ years' of relevant working experience + Ability to travel up to 10% of the time as needed.
