Required Qualifications:
Master?s degree from a four-year college and/or a professional certification beyond a four-year college
AANC or AANP certified family nurse practitioner or NCCPA certified physician associate
Active Unrestricted Oregon State Nurse Practitioner or PhysicianAssistant License or ability to obtain prior to employment
Current Oregon DEA certificate or the ability to obtain prior to employment

Preferred Qualifications:
2+ years of clinical practice experience

Primary Responsibilities:
Conduct in-home assessments including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
Perform evidence-based practice screenings including point of care testing (as appropriate)
Identify diagnoses to be used in care management and active medical management in the furtherance of treatment
Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment
Communicate findings in your assessment that will be used to inform member's PCP of potential gaps in care
Identify urgent and emergent situations and intervene appropriately
Educate members on topics such as disease process, medication, and compliance

Required Qualifications:

Job Summary
Each Psychiatric Nurse Practitioner or PhysicianAssistant (PA) is an important part of the Atlas Medical team. Atlas specializes in providing care for home health and hospice patients in senior living communities and in home settings. Job Responsibilities:
Provide face to face psychiatric services to patients living in long term care, assisted living, and group home settings
Assess patient's mental status based on symptoms
Appropriately diagnose mental disorders and develop treatment plans
Apply appropriate medical decision-making skills for mental health issues
Consult with and educate the patient/family
The provider is independent in his or her management of the assigned patients; Atlas will provide quality measures that will be expected in the care
The provider will work closely with facility staff, patient families, and Atlas/Aegis support staff to provide patient centered care
Maintain confidentiality in all aspects of the job
Skills & Qualifications:
Possess a National Board Certification as a Nurse Practitioner or PhysicianAssistant with a focus in mental health
Current license as a NP/PA in the State of Arizona
Minimum of two (2) year experience
Strong communication and interpersonal skills
Valid Driver's License (clean record) & reliable transportation.
Proof of current auto insurance & registration

Primary Responsibilities:
Conduct comprehensive assessments
Manage patients through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventions
Ensure accurate and complete documentation, including ICD 10 conditions
Communicate and collaborate with the interdisciplinary care team
Conduct advanced illness and advanced care planning conversations
Provide patients and caregivers with counseling and education
Perform comprehensive assessments and document findings in a concise/comprehensive manner that is compliant with documentation requirements and Center for Medicare and Medicaid Services (CMS) regulations
Facilitate agreement and implementation of the patient's plan of care by engaging the facility staff, families/responsible parties, primary and specialty care clinicians
Coordinate care as patients transition through different levels of care and care settings
Review orders and interventions for appropriateness and response to treatment to identify most effective plan of care that aligns with the patient's needs and wishes
Function independently and responsibly with minimal need for supervision

What You?ll Do:
Provide primary, acute, and specialty healthcare services in patients' homes.

Educate patients and families on lifestyle changes, disease prevention, and treatment plans.
Collaborate with physicians and other care team members.
Build trusting relationships through consistent, compassionate care.
Requirements:
MSN degree required
Valid Florida APRN license or PhysicianAssistant license
Board certification in your specialty area
5?7 years of relevant clinical experience preferred
NRP and BLS certifications required
Reliable transportation for travel to patient homes
Skills & Competencies:
Strong clinical judgment and decision-making skills
Excellent verbal and written communication
