Day (United States of America) The Clinical Dietitian provides clinical nutrition services including medical nutrition therapy to patients. Is a strong member of health care team integrated in all service areas of the hospital. Nutritional practice is research based using current scientific literature. Goals of care are collaborative and incorporates Patient & Family Centered care. Strong skills needed for nutritional assessments with ADIME method. If needed, responsibilities may include nutrition consultations to patients in SSVNA Home Care Division, with home visits. If needed may need to cover in the Cardiac/Pulmonary Rehab department. And if needed, may include covering in the out-patient GDM clinic.
Compensation Pay Range:

ESSENTIAL FUNCTIONS
1 - Screens patients for nutritional risk using: Department standards for the established screening data in EMR; Consults; Daily multi-disciplinary rounds; Meals Rounds. Collaborates with Nutr. Guest Reps, nurses and medical providers and SLP to identify patients needing an assessment or re-assessment.
2. Appropriately prioritize patients for the shift. Documents which patients need assessment notes and the reasons; Documents daily multi-disciplinary care rounds on ‘My Patients' Worklist and files in drawer daily.
3 - Assesses patients appropriately using the Nutrition Care Process. Uses information from: EMR; daily meal rounds; patient and family; physicians, nurses and other clinical providers. Assessments are accurate, appropriate and use evidence based practice. Notes are timely, effective and use the ADIME outline. Calculations are based on N&FS department policies for standards of practice.
RD works efficiently to be productive and complete appropriate number of notes/day.
4 - Documentation of assessment and recommendations uses the appropriate templates in EMR. Documentation is appropriate, timely and accurate.
5 - Effectively communicates departmental, organization and industry information to coworkers and other clinical staff. Delivery is clear and in a timely fashion. Communication is productive and resolves conflicts which may affect relations and productivity.
a - Provides in-service education to RD staff, or other facility employees/students, as requested by CNM.
b - Requests made to CNM are written and given in a timely manner.
6 - Functions proactively to resolve patient problems, and provide operational excellence so the department delivers quality services in an efficient and effective manner. Works cooperatively with other employees to enhance nutritional care to patients. Refers questions of policy to CNM or to N&FS managers.
7 - Initiates research of scientific literature to update educational handouts; or updates policies and procedures, standards of practice or guidelines.
8 - Seeks knowledge, skills and competency, to maintain registration with CDR; MA license; and an approved Professional Portfolio in good standing. Attends lectures, seminars and reviews professional journals in a timely manner.
9 - Actively participates in department and hospital meetings, as requested by CNM. Communicates information from Committee meetings to CNM and coworkers. Attendance is evident on the attendance sheets, filed in the Communication Binder.
10 - Shares in responsibility to provide community educational Fairs and presentations, as assigned by CNM. Is proactive in developing agendas and handouts well in advance of the date. Uses powerpoint when available, or other professional tools.
11 - Embraces technology and electronic tools for work processes, and to improve efficiency. Uses computer for literature searches. Uses Outlook and other technology to communicate within the organization
12 - OUT PATIENT COUNSELING for GDM Clinic. In addition to the above essential functions: Coordinate activities to support overall hospital goal of fiscal accountability by helping to grow the out-patient nutrition volume. Become a provider for medical insures as needed. Office is neat, organized, clean and attractive to patients.
a - Work efficiently and use time management to accomplish administrative duties, document notes and send letters to MDs in a timely manner.
b- Order educational forms from Printshop in advance of need.
c - Complete and Process patient Billing and reconciliation each day.
d - Maximize patient appointment within scheduled hours. Establish good rapport with scheduling and pre-registration; UC receptionists.
e - Responsible for out-patient Performance Improvement plan, as directed by CNM: Assist to set the goals; collect data; summarize data; decide action plan and present to CNM on a quarterly basis.

Minimum Education
Bachelor or Master's Degree in Nutrition from a CDR/A.N.D. approved program.
Completed Dietetic Internship from a CDR accredited program.
Passed RD exam or eligible to take RD exam.
Massachusetts licensure or licensure eligible.

Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.
1. Screens nutritionist status of each hospitalized patient to determine nutritional needs and the level of nutritional intervention required by reviewing the patient's medical record, especially the patient's medical history, drug history, and clinical laboratory data, and attending team rounds.
2. Performs nutritional and metabolic assessments based on clinical, medical and nutritional data in order to identify patients at nutrition risk. Interviews patients for nutritional history and relates problems to current medical history.
3. Assesses patients' nutritional status, develops medical nutrition care plans, monitors and documents the needs for those patients identified at nutritional risk, transitions eternally or parentally fed patients to discharge regimen. Monitors nutrient intake records and nutrient calculations for selected patients.
4. Reviews physicians' diet orders for appropriateness and recommends clarification or changes as indicated based on nutrition history, medical history, socio-economic factors and laboratory data.
5. Assists in development, implementation and monitoring of quality improvement standards for patient meal service delivery.
6. Educates both patient and families regarding modified or normal diets to meet the individual nutritional needs of the patient and documents counseling in medical record. Prepares patient education materials.
7. Develops and individualized diet plan (based on the evaluation) and counsels the patient and, if necessary, the family, including the planning, purchasing, and preparation of foods to meet the patient's individual needs.
8. Follows progress of patient on successive visits and makes appropriate changes in care plan.
9. Maintains the necessary patient records and statistics, including written evaluations, progress notes, etc.
10. Documents and updates nutritional status, care plan, and counseling in patient's medical record.
11. Provides nutritional consultation to the medical and nursing staff and other members of the health care team.
12. Acts as a resource to community groups regarding nutrition and gives presentations and lectures at professional meetings and to community groups.
13. Provides nutritional in service education to other members of the health care team.
14. Communicates nutrition information to patient service assistant to ensure optimal delivery of medical nutrition therapy.
15. Interacts with other members of the patient's health care team to ensure optimal nutritional care for each patient including discharge planning and continuing care conferences and continued nutritional support in the home environment.
16. Acts as preceptor for the FSNC nutrition interns (graduate students) during their clinical affiliations. Supervises their progress in evaluating patients, developing and implementing care plan, and counseling patients and families. Prepares teaching manuals, resources, etc.
17. Participates in the review, update and implementation of departmental policies and procedures, as assigned.
18. Participates in departmental quality improvement programs as assigned, especially those related to improving patient outcome.
19. Performs necessary administrative duties such as scheduling patient appointments, reviewing medical records and recording billing and insurance information.
20. Responsible for meeting assigned patient billing and volume goals.
21. Participates, as required, in nutrition-related studies and projects.
22. Assists in the training of new personnel.

Standard Work Duties: Dietitian I
Evaluate patients nutrition status upon consult and or deemed clinically appropriate per referral from members of the multi-disciplinary team, Clinical Informatics and or as directed per Clinical Nutrition Manager, utilizing the Nutrition Care Process, to include Nutrition Focused Physical Exam.
Documents Malnutrition diagnosis based on ASPEN guidelines.
Implements and monitors nutrition care based on: Nutritional Care: Screening, Assessment, and Plan for the Provision of - CL.0190.
Initiates / provides Medical Nutrition Therapy based upon Licensed Dietitian Ordering Privileges.
Collaborates with and communicates effectively with physicians and members of the multi-disciplinary team to integrate information which refines and contributes to the overall medical care of the patient, with specific emphasis on nutritional care, through routine rounds.
Timely and Accurately completes weekly productivity report.
Educates Medical Residents, Nursing and Nutrition Staff.
Participates in Food Service Quality Initiatives (Test Trays, Meal Rounds, GEMBA Meetings, reports concern to FSD and or CNM, maintains open communication with Clinical Nutrition Manager.
Guide Projects Using Acceptable Standards And ITIL Framework Safety And Performance Improvement
Behaves in a mindful manner focused on self, patient, visitor, and team safety.
Demonstrates accountability and commitment to quality work.
Participates actively in process improvement and adoption of standard work.

Strong analytical skills to resolve complex problems using scientific, mathematical, or technical principles and in-depth, experience-based knowledge.
Effective communication and writing skills to manage complex discussions, conflicting views, and provide clear documentation.
Spanish language skills a plus.
Licensure / Certifications:
Registration by the Commission on Dietetic Registration (CDR) as a Registered Dietitian.
Licensure by the Commonwealth of Massachusetts as a Licensed Dietitian.

Position Summary:
The Pediatric Registered Dietitian II will play a vital role in delivering high-quality nutritional care for our inpatient population, including critical care patients. Current Massachusetts Dietitian license.
CNSC highly preferred.
