Under the general supervision and direction of the Physician, Office Manager, or designee, and working collaboratively with patients and families (as defined by the patient), perform episodic illness visits and follow-up care visits [preventative care] which may require laboratory testing, radiology testing and/or immunizations.
Formulate diagnosis based on clinical assessment and establish an appropriate/ timely treatment plan.
Select and order appropriate medication for treatment of medical conditions.
Review diagnostic tests and procedures ordered, as it relates to follow-up care.
Utilize an Electronic Medical Record (EMR) and other related software to update and/or access patient information, as appropriate.
Maintain patient privacy and confidentiality.
Qualifications
Physician Assistant: Bachelors Degree in a related field is required.
Graduate of an A.M.A.-approved Physician Assistant program is required.
Certification from a National accrediting body to practice as a Physician Assistant is required.
Nurse Practitioner: Masters of Science Degree in Nursing is required.
Certification from a National accrediting body to practice as a Nurse Practitioner is required.
Current MA RN registration is required.
Nurse Practitioner Certificate/Physician Assistant Certificate is required.
Current MA Nursing License or Physician Assistant License is required.
Bilingual a plus.

Our hours of operation are Monday-Friday from 8:00 AM to 5:00 PM. This NP will be off on Tuesdays and required to work on Monday's and Fridays - the other days are flexible.

Major Responsibilities:
1. Provides diagnostic and assessment services. Secures a health history from patient and/or family, records findings, and evaluates them. Performs a complete in-depth physical examination. Orders/performs pertinent diagnostic tests based on age and history.
2. Analyzes data collected to determine health status and identify differential diagnoses based on history, physical exam, and clinical findings.
3. Partners with physicians regarding proposed plan of care. Formulates this plan of care with the patient.
4. Manages therapeutic regime as outlined by established protocols for patients with acute illnesses. (Protocols are guidelines concerning patient care that are established between the physician and nurse practitioner. The guidelines outline subjective and objective findings, treatment plan and follow-up). Reassesses and modifies plan as necessary to achieve medical and health goals. Confers with physician for clinical direction as outlined by protocols.
5. Documents patient care outcomes to determine effectiveness of plan of care. Communicates outcomes with physician.
6. Signed delineation of privileges at hire between NP and collaborating physician. Must renew delineation of privileges each credentialing cycle including:
scope of departmental practice
treatment of emergencies
physician consultation
writing orders/prescription
7. Prescribes medications from Schedules II-VI. The form of prescription may be written or verbal via telephone order. Written prescriptions may be initiated by the nurse practitioner after obtaining proper registration numbers from the Massachusetts Department of Public Health and Drug Enforcement Agency.
8. Provides patient education. Acts as coordinator for admitting and discharging assigned patients. Works collaboratively with other health agencies and families of patients in the transition from admission to discharge. Confers with other health care providers including consultations and referrals to prepare a comprehensive patient care plan.
9. Within established protocols, under the supervision of the physician, nurse practitioners will assist with procedures in the operating room and with endoscopic procedures.
10. Enhances professional growth and development through participation in educational programs, current literature, in-service meeting and workshops.
11. Participates in department driven practices including on call scheduling, rotating weekends, etc.
Ambulatory Care Nurse Practitioners:
Completion of accredited program pertinent to the departmental needs (OB/GYN, Pediatrics, etc.) and has passed the appropriate certification exam.
Partners with collaborating physician or designee as necessary with complexity of problems or plan of care
Formulates plan of care with patient and multidisciplinary/interdisciplinary healthcare team
Maintains department specific competencies that are degree specific, age specific to the discipline and license specific per certifying authority.

Graduate of an approved Acute Care Nurse Practitioner program (ACNP) and has passed the ACNP board certification.

Responds to sick calls of patients and treats them according to the acuity of illness.

Performs procedures such as central line placement, insertion of arterial catheters, lumbar punctures, paracentesis and thoracentesis.
Provides patient education and therapeutic management of the family in crisis related to critical illness.
Maintains competencies as directed by the division.

Major Responsibilities:
1. Provides diagnostic and assessment services. Secures a health history from patient and/or family, records findings, and evaluates them. Performs a complete in-depth physical examination. Orders/performs pertinent diagnostic tests based on age and history.

6. Signed delineation of privileges at hire between NP and collaborating physician. Must renew delineation of privileges each credentialing cycle including:
scope of departmental practice
treatment of emergencies
physician consultation
writing orders/prescription
7. Prescribes medications from Schedules II-VI. The form of prescription may be written or verbal via telephone order. Written prescriptions may be initiated by the nurse practitioner after obtaining proper registration numbers from the Massachusetts Department of Public Health and Drug Enforcement Agency.
8. Provides patient education. Acts as coordinator for admitting and discharging assigned patients. Works collaboratively with other health agencies and families of patients in the transition from admission to discharge. Confers with other health care providers including consultations and referrals to prepare a comprehensive patient care plan.
9. Within established protocols, under the supervision of the physician, nurse practitioners will assist with procedures in the operating room and with endoscopic procedures.

In this Float - Nurse Practitioner job, float nurse practitioners care for patients at 15 Primary Care practices, which include Family Medicine, Health Centers, Pediatric Care and Adult Internal Medicine.
The practice locations range from Main campus in Boston, and in nearby towns which may include Jamaica Plain, Chestnut Hill, Brookline, Norwood, and Foxboro. Patient population is primarily adult however may include some pediatrics at certain practice sites.
The teams at each practice may vary but the goal is still the same: to provide high quality care for patients whether you are conducting a nurse only visit in clinic or providing care telephonically.
Job Summary
GENERAL OVERVIEW
The Nurse Practitioner is an advanced practice registered nurse who has acquired specialized knowledge and skills through completion of a nurse practitioner program. The practitioner has the credentials to function in an expanded nursing role and is responsible for the daily management of selected patients. The practitioner has responsibility for assessment and management of patients including diagnostic and therapeutic interventions, and continuous implementation and evaluation of an appropriate plan of care. The Nurse Practitioner works with her/his collaborating physician in accordance with mutually developed practice guidelines, members of the care team and is responsible to the Nursing Director or Executive Nursing Director for the area for issues specific to the discipline of nursing.
PRINCIPAL DUTIES AND RESONSIBILITIES
A. Patient Care Organization
1. Provides direct care, counseling, and teaching to a select patient population.
2. Performs physical examinations and provides preventive health measures appropriate to patient needs.
3. Orders, interprets, and evaluates appropriate lab and diagnostic tests to assess patients' clinical problems and health care needs.
4. Arranges for appropriate plan of care and follow-up based on outcome of diagnostic, lab, and physical assessment findings.
5. Performs laboratory tests appropriate for specific patient population.
6. Orders medications and writes prescriptions according to organizational and regulatory policies and procedures.
7. Records pertinent patient care findings in the medical record.
8. Sets priorities for appropriate and efficient management of patient care that reflects cost effective management of time, available human resources, supplies and equipment.
9. Maintains interdisciplinary approach to planning and communicating patient care information by discussing patient data with appropriate physicians and other health care professionals.
10. Incorporates the concepts of health maintenance, prevention, and promotion into daily practice through patient education and counseling.
11. Demonstrates awareness of community resources and assists staff, patients, and families to effectively utilize them.
B. Quality of Care
1. Assists in the development, implementation and revision of nurse practitioner guidelines and monitoring requirements for prescription writing in collaboration with their designated physician and Nursing Director or Executive Nursing Director.
2. Evaluates outcomes of own patients' care and makes necessary revisions in care plans.
3. Seeks and provides consultative services as required.
4. Participates in departmental and organization quality improvement program.
5. Utilizes quality improvement data and research findings to enhance patient care.
C. Coordination
1. Demonstrates appropriate collaborative and consultative relationships with other providers within the institution and community.
2. Serves on Nursing and Hospital Committees and assumes responsibility for disseminating and communicating information to staff.
D. Budget
1. Participates in setting productivity metrics and in cost effective management of personnel, time, supplies, and equipment in his/her area.
2. Collaborates with the Nursing Director/Practice Manager/Executive Nursing Director in the recommendation of appropriate staffing patterns for their clinical areas.
E. Personnel Management
1. Participates in selection process of new employees in group or team practice as requested.
2. Provides input into evaluation of other staff as appropriate.
F. Education
1. Acts as preceptor for nursing students and staff as requested.
2. Participates in orientation of new staff.
3. Participates in Departmental and organization continuing education programs.
4. Identifies own learning needs and updates clinical knowledge and skills through formal and informal education and review of current literature.
5. Educates lay and/or professional groups through lectures, papers, publications, and educational materials.
G. Planning
1. Develops annual objectives and reviews with Nursing Director or Executive Nursing Director and collaborating M.D. at time of performance review and at appropriate intervals. Completes annual credentialing process through the Nursing Department credentialing program.
2. Assists in determining, evaluating, and/or implementing clinical area goals and objectives.
3. Maintains interdisciplinary approach for planning and communicating information for patient care and operation of practice.
4. Maintains current knowledge of legislation that may affect the health care delivery system.
H. Research
1. Participates in nursing research at a level consistent with educational background and experience.
2. Maintains understanding of current research of others in related clinical areas through review of relevant publications.
3. Participates in grant writing as appropriate.
Qualifications
Education:
* Master's in Nursing (MSN) (or) DNP (or) PhD Degree in Nursing required.
* Completion of a Master's or post Masters' degree in Adult or Family NP program accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC).
* Satisfactory completion of an approved formal education program which has as its objective the preparation of professional nurses to practice as nurse practitioner.
* Current Adult/Gerontology (AGNP) or Family Nurse Practitioner (FNP) Certification by an accredited APRN certifying body required.
* Current APRN License to practice in Massachusetts Required.
Experience:
* 1. Five (5) + years of recent experience in office-based Primary Care as an NP required.
* 2. Bilingual English/Spanish preferred.
* 3. Pediatric experience preferred.
* 4. Demonstrated ability to care for a diverse patient population, including acute and chronic disease management as an NP required.
* 5. Demonstrated ability to care for a broad range of ages as a nurse practitioner required (ages 16 and up).
* 6. Previous experience working in a float or temporary/seasonal position required.
* 7. Experience managing inter-visit care via asynchronous modalities including phone visits, electronic communication and in-basket management as an NP required.
* 8. Prior experience working in a team-based care environment required.
Other:
* ANA or other appropriate nursing certification in area of practice and authorization by the Massachusetts Board of Registration in Nursing to practice in the expanded role. DEA for prescription writing.

Major Responsibilities:
1. Provides diagnostic and assessment services in Epilepsy Clinic. Secures a health history from patient and/or family, records findings, and evaluates them. Performs a complete in-depth physical examination. Orders/performs pertinent diagnostic tests based on age and history.
2. Analyzes data collected to determine health status and identify differential diagnoses based on history, physical exam, and clinical findings.
3. Partners with physicians regarding proposed plan of care. Formulates this plan of care with the patient.
4. Manages therapeutic regime as outlined by established protocols for patients with acute illnesses. (Protocols are guidelines concerning patient care that are established between the physician and nurse practitioner. The guidelines outline subjective and objective findings, treatment plan and follow-up). Reassesses and modifies plan as necessary to achieve medical and health goals. Confers with physician for clinical direction as outlined by protocols.
5. Documents patient care outcomes to determine effectiveness of plan of care. Communicates outcomes with physician.
6. Signed delineation of privileges at hire between NP and collaborating physician. Must renew delineation of privileges each credentialing cycle including:
scope of departmental practice
treatment of emergencies
physician consultation
writing orders/prescription
7. Prescribes medications from Schedules II-VI. The form of prescription may be written or verbal via telephone order. Written prescriptions may be initiated by the nurse practitioner after obtaining proper registration numbers from the Massachusetts Department of Public Health and Drug Enforcement Agency.
8. Provides patient education. Acts as coordinator for admitting and discharging assigned patients. Works collaboratively with other health agencies and families of patients in the transition from admission to discharge. Confers with other health care providers including consultations and referrals to prepare a comprehensive patient care plan.
9. Within established protocols, under the supervision of the physician, nurse practitioners will assist with procedures in the operating room and with endoscopic procedures.
10. Enhances professional growth and development through participation in educational programs, current literature, in-service meeting and workshops.

In 2015, NEW Health expanded services into Charlestown with the opening of a Public Housing Primary Care site, offering high quality, accessible care to the residents of the largest public housing development in New England.
Nurse Practitioners at NEW Health are key members of our primary care team. These positions are fully onsite at our North End location. Each NP manages their own patient panel, typically 300-900 patients, and provides comprehensive care that includes chronic disease management, preventive services, and same day access for urgent needs. They work closely with physicians, behavioral health providers, community health workers, nurses, and support staff to deliver coordinated, team-based care that is accessible and patient-centered.
Located on historic Hanover Street in Boston's North End, this site serves a patient population that is primarily English-speaking, between the ages of 25 and 55, and generally has a moderate to high level of health literacy. Our North End site offers accessible, high-quality care in a close-knit community setting, with a strong focus on continuity, preventive health, and chronic disease management.
